Hepatitis B Core-Related Antigen and New Therapies for Hepatitis B.

The hepatitis B core-related antigen (HBcrAg) is an unprecedented novel HBV biomarker that plays an essential role in reflecting covalently closed circular DNA (cccDNA) in chronic hepatitis B (CHB) because its levels correlate with intrahepatic cccDNA and serum HBV DNA. In this review, we describe the clinical application of serum HBcrAg in CHB patients, with a particular focus on new therapies targeting intrahepatic HBV replication. (1) HBcrAg can be detected in clinical cases where serum HBV DNA is undetectable during anti-HBV therapy. (2) A highly sensitive HBcrAg assay (iTACT-HBcrAg) may b…

New Treatments for Chronic Hepatitis B Virus/Hepatitis D Virus Infection.

Chronic hepatitis D virus (HDV) infection is the most severe form of viral hepatitis with high rates of end-stage liver disease and hepatocellular carcinoma. Therefore, effective antiviral treatment strategies are needed desperately. Until recently, antiviral treatment was limited to pegylated interferon-alpha. With the conditional approval of the entry inhibitor bulevirtide by the European Medicines Agency, new treatment options are now available. In addition, multiple other antiviral compounds are currently tested in clinical phase II and III trials and represent promising agents for the tre…

Comparative Pathology of Hepatitis A Virus and Hepatitis E Virus Infection.

Hepatitis A virus (HAV) and hepatitis E virus (HEV) cause acute, self-limiting hepatic infections that are usually spread by the fecal-oral route in humans. Naturally occurring and experimental infections are possible in a variety of nonhuman primates and, in the case of HEV, a number of other species. Many advances in understanding the pathogenesis of these viruses have come from studies in experimental animals. In general, animals infected with these viruses recapitulate the histologic lesions seen in infected humans, but typically with less severe clinical and histopathological manifestatio…

Hepatitis A virus-associated fulminant hepatitis with human immunodeficiency virus coinfection.

Hepatitis A virus (HAV) commonly causes acute hepatitis in humans and is transmitted through the fecal-oral route or by ingestion of contaminated food or water. HAV infection generally follows a self-limiting course; it can seldom cause fulminant hepatitis that increases the risk of mortality. To the best of our knowledge, this is the first reported fatal case of fulminant hepatitis caused by HAV in a 40-year-old male with human immunodeficiency virus (HIV) infection. The HAV genotype in this case was IA, which has recently become common globally among people living with HIV (PLWHIV), intraven…

Autoimmune Hepatitis and Seronegative Hepatitis Associated With Myelodysplastic Syndrome in Children.

An association between hepatitis and aplastic anemia (AA) is known as hepatitis-associated AA, and is characterized by an acute attack of hepatitis followed by the development of AA. We report 2 clinical cases of acute seronegative hepatitis in which pancytopenia with mild dysplasia developed after 3 months; however, neither of our cases fulfilled the histological criteria of AA, but rather myelodysplastic syndrome. This novel association bears considerable resemblance to hepatitis-associated AA, and raises the question of whether hepatitis-associated dysmyelopoiesis should be included in the …

Hepatitis E Virus Superinfection and Clinical Progression in Hepatitis B Patients.

Hepatitis E virus (HEV) infection may cause acute hepatitis and lead to hepatic failure in developing and developed countries. We studied HEV seroprevalences in patients with hepatitis B virus (HBV) infection to understand the consequences of HEV superinfection in a Vietnamese population. This cross-sectional study was conducted from 2012 to 2013 and included 1318 Vietnamese patients with HBV-related liver diseases and 340 healthy controls. The case group included patients with acute (n = 26) and chronic hepatitis B (n = 744), liver cirrhosis (n = 160), hepatocellular carcinoma (n = 166) and p…

Serum lactonase and arylesterase activities in alcoholic hepatitis and hepatitis B.

BACKGROUND: PON1 is an HDL-associated enzyme having antioxidant activity. PON1 is synthesized in the liver, and there is decreased synthesis … with liver disease out of which 40 were having chronic alcoholic liver disease and 20, acute viral hepatitis B, and 60 were healthy controls. Serum PON1 lactonase activity was measured manually using dihydrocoumarin, and AE activity was measured using phenylacetate as substrate. Liver function tests (bilirubin, albumin, AST, ALT, alkaline phosphatise) were …

Vietnamese community screening for hepatitis B virus and hepatitis C virus.

Asian Americans represent an important cohort at high risk for viral hepatitis. To determine the prevalence of Hepatitis B virus (HBV) and Hepatitis C virus (HCV) infection and HBV vaccination in a Vietnamese community, a total of 322 Vietnamese subjects from a local doctor's office and annual Vietnamese Health Fair were included in this study. Demographic and clinical data were collected. 2.2% of the screened cohort tested positive for anti-HCV and 9.3% tested positive for HBsAg. Unlike HBV-positive subjects, HCV-positive subjects had significantly higher liver enzymes (P = 0.0045 and P = 0.0…

Hepatitis C virus infection increases hepatitis risk during anti-tuberculosis treatment.

OBJECTIVE: To study the impact of hepatitis B virus (HBV) and hepatitis C virus (HCV) co-infection on clinically significant transaminase elevation during short-course anti-tuberculosis treatment. DESIGN: Retrospective observation study. RESULTS: During standard anti-tuberculosis treatment of 295 patients with active pulmonary tuberculosis (TB) and normal baseline liver biochemical tests, 25 (8.5%) developed hepatitis and had a significantly higher mortality rate (32% vs. 7%, OR 6.22, 95%CI 2.0-17.6, P = 0.001). Multivariate analysis showed that HCV co-infected individuals were more likely t…

Hepatitis B virus and hepatitis C virus in pregnant Sudanese women.

BACKGROUND: The epidemiology of viral hepatitis during pregnancy is essential for health planners and programme managers. While much data exist concerning viral hepatitis during pregnancy in many African countries, no proper published data are available in Sudan. AIM: The study aimed to investigate the sero-prevalence and the possible risk factors for hepatitis B virus (HBV) and hepatitis C virus (HCV) among antenatal care attendants in central Sudan. METHODS: During 3 months from March-June 2006, sera were collected from pregnant women at Umdurman Maternity Hospital in Sudan, and they were …

Postinfantile giant-cell hepatitis associated with ulcerative colitis and autoimmune hepatitis.

Postinfantile giant cell hepatitis (PGCH) is rare. It is characterized by the presence of multinucleated giant cells in liver biopsy, and although it has been associated with several etiological agents, in many cases its etiology remains unclear. The case is presented herein of an adult woman with PGCH in the setting of ulcerative colitis and autoimmune hepatitis. The presence of autoimmune hepatitis in the patient is consistent and supports the autoimmune pathogenesis of PGCH in a subgroup of patients. Furthermore, this finding, along with others, suggests that PGCH may be included in the lis…

[Syndrome of overlap: Chronic hepatitis C/autoimmune hepatitis: fact or fancy?].

Non-organ specific autoantibodies are common in patients with chronic hepatitis C, making differential diagnosis difficult between viral, autoimmune forms and hepatitis C/autoimmune hepatitis overlap syndrome. The lack of additional criteria of autoimmunity in most patients leads to the definition of a "false" hepatitis C-autoimmune hepatitis overlap syndrome, while the "true" overlap syndrome occurs in a very few number of subjects. In patients with "false" overlap syndrome the first choice therapy is based on the administration of interferon plus ribavirin. On the contrary, first-line therap…

Alcoholic intake predisposes to more interface hepatitis in chronic hepatitis C.

Progression of liver disease in chronic hepatitis C depends on several factors related to the host, virus and the environment which deserves further investigations. 120 candidates for blood donation with hepatitis C virus were divided into three groups according to alcohol intake: abstainers, light drinkers and heavy drinkers. Liver histopathology alterations, namely architectural staging, periportal and lobular inflammation as well as portal inflammatory infiltrate were graded from 0 to 4 and afterwards divided into light (0 to 2) and severe (3 to 4). There were more drinkers among men (83.5%…

[Chronic hepatitis C/autoimmune hepatitis overlap syndrome: report of two cases].

The coexistence of non-organ specific autoantibodies and chronic hepatitis C features raises the dilemma as to whether the pathogenesis is viral or autoimmune and consequently to be treated with interferon or corticosteroids. Two cases with autoimmune characteristics and chronic hepatitis C are reported. We discuss the possibility of classifying this overlap form in three distinct entities: viral disease with an autoimmune epiphenomenon ("false hepatitis C/autoimmune hepatitis overlap syndrome"), viral disease associated to an autoimmune pathogenic component ("true hepatitis C/autoimmune hepat…

[Autoimmune hepatitis and autoimmune manifestations in patients with chronic viral hepatitis].

The study reviews pathogenesis of autoimmune hepatitis and role of hepatotropic viruses in this disease. For a long time it was not known for certain which antigen was autoantigen on the hepatocyte. The discovery of human asialo glycoprotein receptor as an autoantigen enabled distinguishing autoimmune hepatitis from other chronic active hepatitis of unknown etiology. Autoantibodies and other tissue antigens ensured differentiation of autoimmune hepatitis in regard to severity of clinical manifestations in patients of different age and sex. The paper deals with autoantibodies and their occurren…
