Concurrent ipsilateral fractures of acetabular posterior wall and femoral shaft associated with posterior hip dislocation: A case report and literature review of a rare injury.

INTRODUCTION: A floating hip injury involving the acetabulum and femur, often complicated by hip dislocation, necessitates a timely and appropriate management strategy to preserve the extremity and patient well-being. CASE PRESENTATION: We present a case of a 20-year-old male with concurrent fractures of the acetabular posterior wall, a comminuted femoral shaft, and posterior hip dislocation. Reduction of the dislocated hip posed a challenge due to the femoral shaft fracture. We successfully employed an innovative technique, using pins proximal and distal to the shaft fracture in conjunction

Editorial Commentary: Managing Hip Pain, Athletic Pubalgia, Sports Hernia, Core Muscle Injury, and Inguinal Disruption Requires Diagnostic and Therapeutic Expertise.

Pubalgia means pubic pain. This is different from core muscle injury (implying muscular pathology) or inguinal disruption (different anatomic region). Athletic pubalgia includes a myriad of pathologic conditions involving the pubic symphysis, adductors, rectus abdominis, posterior inguinal wall, and/or related nerves. Moreover, growing evidence supports a link between femoroacetabular impingement (FAI) and pubalgic conditions. Constrained hip range of motion in flexion causing obligatory transitory, even ballistic, posterior tilting of the hemipelvis may produce pathologic transfer stress to n

Lower limb kinematics and hip extensors strengths are associated with performance of runners at high risk of injury during the modified Star Excursion Balance Test.

BACKGROUND: Reduced trunk and lower limb movement and hip and trunk muscles weakness may compromise the athletes' performance on the modified Star Excursion Balance Test (mSEBT). OBJECTIVE: To investigate the relationship of trunk and lower limb kinematics and strength with the performance on the mSEBT of runners at high risk of injury. METHODS: Thirty-nine runners performed the mSEBT with the dominant limb as the support limb. An Inertial System was used to capture the trunk, hip, knee and ankle movement during the mSEBT. A handheld dynamometer was used to measure the strength of trunk exte

Spontaneous healing of lateral femoral cutaneous nerve injury and improved quality of life after total hip arthroplasty via a direct anterior approach.

PURPOSE: How the symptomatology of lateral femoral cutaneous nerve (LFCN) injury changes after total hip arthroplasty (THA) via direct anterior approach (DAA) is not known. Our hypothesis was that the symptoms of LFCN injury after THA via DAA in longer follow-up periods would resolve spontaneously, leading to an improved quality of life (QOL). The aims of this study were to investigate how the symptom LFCN injury changed after DAA-THA, and how those changes affected QOL. METHODS: We investigated the incidence of LFCN injury after DAA-THA using self-reported questionnaires at two time points (

The Immediate Intramedullary Nailing Surgery Increased the Mitochondrial DNA Release That Aggravated Systemic Inflammatory Response and Lung Injury Induced by Elderly Hip Fracture.

Conventional concept suggests that immediate surgery is the optimal choice for elderly hip fracture patients; however, few studies focus on the adverse effect of immediate surgery. This study aims to examine the adverse effect of immediate surgery, as well as to explore the meaning of mtDNA release after trauma. In the experiment, elderly rats, respectively, received hip fracture operations or hip fracture plus intramedullary nail surgery. After fracture operations, the serum mtDNA levels as well as the related indicators of systemic inflammatory response and lung injury significantly increase

Optimizing Preoperative Venous Thromboembolism Risk Assessment in Elderly Hip Fracture Patients: A Refined Caprini Model Integrating D-Dimer and Injury-to-Admission Time.

BackgroundThe Caprini risk assessment model (RAM) is widely used to evaluate venous thromboembolism (VTE) risk across diverse patient populations. However, the original risk stratifications may require modification for hip fracture patients to improve predictive accuracy. This study aimed to optimize VTE prediction by refining the Caprini model and integrating additional predictive factors.MethodsThis retrospective cohort study included 1114 elderly hip fracture patients screened at Peking Union Medical College Hospital between May 2012 and February 2023. A modified VTE prediction model was de

Tranexamic acid and erythropoietin inhibited inflammatory response and endoplasmic reticulum stress and protected rats against acute lung injury caused by osteoporotic hip fracture.

The objective of this study was to evaluate the protective effects and elucidate the mechanism underlying Tranexamic acid (TXA) and erythropoietin (EPO) in osteoporosis hip fracture (OHF)-induced acute lung injury (ALI). Sprague-Dawley (SD) rats were randomly divided into the control, OHF, OHF + TXA, and OHF + EPO groups. Lung tissue was analyzed at 12, 24, and 48 h post-treatment to assess injury levels through hematoxylin and eosin (H&E) staining, enzyme-linked immunosorbent assay (ELISA), and Western blot analysis. H&E staining showed that TXA and EPO significantly reduced the OHF-induced l

Hip Abductor Rate of Torque Development as Opposed to Isometric Strength Predicts Peak Knee Valgus During Landing: Implications for Anterior Cruciate Ligament Injury.

Peak knee valgus has been shown to predict anterior cruciate ligament injury. The purpose of the current study was to compare peak rate of torque development (RTD) to peak isometric torque as a predictor of peak knee valgus during landing. Twenty-three healthy females participated. Hip abductor muscle performance was quantified using 2 types of isometric contractions: sustained and rapid. Peak isometric torque was calculated from the sustained isometric contraction. Peak RTD was calculated from the rapid isometric contraction (0-50 and 0-200 ms after force initiation). Kinematic data were coll

Effect of a wound protector on soft-tissue injury and early recovery after endoscopic direct anterior approach total hip arthroplasty: a randomized controlled trial.

BACKGROUND: The endoscopic direct anterior approach (Endo-DAA) for total hip arthroplasty (THA) aims to minimize soft-tissue trauma and accelerate recovery. Whether adding a disposable wound protector can further reduce early muscle injury and enhance recovery remains unclear. This randomized controlled trial evaluated the impact of wound protector use on biological, clinical, and functional outcomes after Endo-DAA THA. METHODS: Seventy-six patients undergoing primary Endo-DAA THA were prospectively randomized (1:1) to either a wound protector or control group. The only intraoperative differe

Intraoperative determination of the risky angles and safe distances for preventing deep femoral artery injury during proximal femoral nailing for hip fractures in Asian people.

OBJECTIVE: During proximal femoral nailing, deep femoral artery injury, a rare condition, is often missed and found late, leading to intractable complications such as false aneurysm, hematoma, and anemia. We aimed to determine the novel indicators of the high-risk vertical range and axial angle for deep femoral artery injury that can be easily confirmed intraoperatively using fluoroscopy for hip fracture. METHODS: In a single hospital, the lower extremity computed tomography angiographies of 88 patients (50 men and 38 women) were analyzed. A reference plane was defined as the femoral neck and

Case Report: Trans-Articular External Skeletal Fixation of the Hip for a Highly Comminuted Juxta-Articular Fracture of the Proximal Femur Caused by Gunshot Injury in a Cat.

This report describes a novel surgical technique for trans-articular external skeletal fixation (TA-ESF) of the hip to stabilize a rare, highly comminuted juxta-articular fracture of the proximal femoral segment involving the metaphysis and diaphysis in a cat. A 2-year-old, castrated male, Korean shorthair cat was admitted for a left femoral fracture caused by gunshot injury. Radiographs and computed tomography (CT) scans revealed a highly comminuted proximal femoral diaphyseal fracture that involved the femoral neck and the greater trochanter. The left femoral head was intact on CT. Under flu

Conventional versus lateral fasciotomy for prevention of lateral femoral cutaneous nerve injury in the non-fan-type nerve in total hip arthroplasty with direct anterior approach.

AIMS: Lateral femoral cutaneous nerve (LFCN) injury is a potential complication after the direct anterior approach for total hip arthroplasty (DAA-THA). The aim of this study was to determine how the location of the fasciotomy in DAA-THA affects LFCN injury. METHODS: In this trial, 134 patients were randomized into a lateral fasciotomy (n = 67) or a conventional fasciotomy (n = 67) group. This study was a dual-centre, double-blind, prospective randomized controlled two-arm trial with parallel group design and a 1:1 allocation ratio. The primary endpoint was the presence of LFCN injury, which

High Long-Term Mortality Rate in Elderly Patients with Mild Traumatic Brain Injury and Subdural Hematoma due to Ground-Level Fall: Neurosurgery's Hip Fracture?

BACKGROUND: Moderate-to-severe traumatic brain injury (TBI) is a major source of morbidity and mortality in elderly patients. Little is known about long-term mortality in elderly patients following mild, nonfatal TBI and how the injury mechanism predicts survival. This study aimed to compare long-term mortality in elderly patients with mild TBI and traumatic subdural hematoma (tSDH) due to ground-level fall (GLF) versus those with TBI and tSDH due to another cause (i.e., non-ground-level fall [nGLF]). METHODS: This retrospective study comprised 288 patients ≥60 years old from a single Level I

Comparison of the quality of life in individuals with spinal cord injury wearing either reciprocating gait orthosis or hip knee ankle foot orthosis: a cross-sectional study.

BACKGROUND: The quality of life (QoL) for patients with spinal cord injuries (SCI) is lower than that for healthy individuals. The main purpose of prescribing orthoses for these individuals is to improve their mobility and QoL. The hip knee ankle foot orthosis (HKAFO) has been the conventional choice for such patients, whilst the reciprocating gait orthosis (RGO) is a more contemporary option. Although the impact of these two types of orthoses on the biomechanics of walking has been previously evaluated in patients with SCI, there has been no specific comparison of their relative effects on Qo
