Control of diarrhoeal disease in Tonga 1978-83.

Tonga, like many developing countries, suffers from a shortage of medical staff and a high morbidity and mortality from paediatric diarrhoeal disease. In 1980 a programme was started to train medical assistants and village administrators in the correct use of oral rehydration salt solution for rehydration. The effect on morbidity, mortality, and admission to hospital over the six years 1978-83 was assessed. After the introduction of the scheme the number of deaths due to diarrhoea fell considerably and the state of hydration in children admitted to hospital with diarrhoea greatly improved. It

Stress management consultation to Israeli social workers during the Gulf War.

Social workers operating under emergency conditions may feel overwhelmed and incapacitated by their own stress reactions. During the Gulf War, social work faculty in Israel acted as consultants to help hospital social workers manage stress and regain professional efficacy. The Stress Management Consultation (SMC) was a short-term group intervention designed to enable social workers to work through their stress reactions and to model a method the workers could use with their own target populations. Qualitative feedback from both participants and administrators indicated that the SMC model was e

Maintaining Health Care in Occupied Ukraine: Criminal Collaboration or Conscientious Professionalism?

Following Russia's occupation of Eastern Ukraine, local health care professionals, particularly hospital administrators and public health officials, have faced criminal charges of medical collaboration for taking senior managerial positions in the occupation regime and allocating resources to the Russian army. Although Ukraine is entitled to prosecute collaborators who threaten its national security, the grounds for criminalizing medical administration during military occupation are much weaker and essentially indefensible. During occupation, international humanitarian law requires Russia to m…

The Surgical and Anesthesia Workforce and Provision of Surgical Services in Rural Communities: A Mixed-Methods Examination.

PURPOSE: Rural-urban disparities in the surgical and anesthesia workforce exist. This mixed-methods study describes the distribution of the surgical and anesthesia workforce and qualitatively explores how such workforce and other factors influence rural hospitals' provision of surgical services. METHODS: We calculated provider counts by county from the Area Health Resource File. Using American Hospital Association survey data, we sampled rural hospitals, stratified by critical access status and state policies. We conducted qualitative semistructured interviews with administrators at 16 hospit

Verbal and physical abuse of nurses.

In order to determine the extent to which nurses working in hospitals in the Toronto area experienced abuse, questionnaires were distributed to 2344 nurses working in either a large teaching hospital, a psychiatric hospital or a community hospital. Of the 603 nurses responding to the questionnaire, 199 (33%) had experienced abuse in their last five days at work. There were no statistical differences between the three hospitals in the incidence of abuse. The abuse was mainly verbal, from the patient and during the day shift. Although abuse occurred in all areas of the hospitals and was directed

An Explanatory Mixed-Methods Study of Intensive Care Unit Net Benefit: Triage and Trajectory for Sepsis and Acute Respiratory Failure.

Rationale: Patients with sepsis and/or acute respiratory failure are at high risk for death or long hospital stays, yet limited evidence exists to guide triage to intensive care units (ICUs) or general medical wards for the majority of these patients who do not initially require life support. Objectives: To identify factors that influence how hospitals triage patients with capacity-sensitive conditions and those factors that may account for observed ICU relative to ward, or ward relative to ICU, benefits for such patients. Methods: We conducted an explanatory sequential mixed-methods study. As

Coordinated clinical and financial analysis as a powerful tool to influence vendor pricing.

BACKGROUND: As costs continue to outpace reimbursements, hospital administrators and clinicians face increasing pressure to justify new capital purchases. Massachusetts Health Care Reform has added further economic challenges for Disproportionate Share Hospitals (DSH), as resources formerly available to treat the uninsured have been redirected. In this challenging climate, many hospitals still lack a standardized process for technology planning and/or vendor negotiation. PURPOSE: : The purpose of this study was to determine whether a simple, coordinated clinical and financial analysis of a te…

Understanding the economic impact of introducing a new procedure: calculating downstream revenue of endobronchial ultrasound with transbronchial needle aspiration as a model.

Over the last decade, endobronchial ultrasound (EBUS) evolved into a validated and powerful diagnostic tool. Although it is integral to medical care in some health-care systems, others struggle to justify its purchase based on diminishing reimbursement. In analyzing its value to a health-care system, looking at procedural reimbursement alone will grossly underestimate its economic impact. Downstream revenue has been defined by administrators as revenue captured after patients use one hospital service and then use others. By analyzing consecutive EBUS cases and taking downstream revenue into ac

Fetal versus maternal rights: medical and legal perspectives.

This report deals with a patient's refusal to undergo cesarean section, which was recommended when fetal distress occurred in labor. Faced with this conflict of maternal and fetal rights, the physicians, administrators, and attorneys of the hospital requested a hearing and decision on the issue by the juvenile court. As a result cesarean section was performed as ordered by the court to safeguard the life of the unborn infant in spite of the objections of the mother. This case, in part, illustrates the need for the administrative capability to deal with such a situation even on an emergency bas

Barriers to implementation of pediatric emergency department interventions for parental tobacco use and dependence: a qualitative study using the theoretical domains framework.

BACKGROUND: Pediatric emergency department (PED) and urgent care (UC) professionals can play a key role in delivering evidence-based guidelines to address parental tobacco use and child tobacco smoke exposure (TSE). Understanding PED/UC professionals' perceptions regarding these guidelines is the first step in developing and implementing a TSE screening and counseling intervention in these settings. This study aimed to use the theoretical domains framework (TDF) to identify current screening and counseling behaviors of PED/UC professionals related to parental tobacco use and child TSE, and det
