Association between oral candidiasis and bacterial pneumonia: A retrospective study.

OBJECTIVE: To investigate associated risk factors for oral candidiasis in elderly patients hospitalized in a community-based acute-care hospital with no dental units. METHODS: Two hundred and twenty-eight elderly patients (male: 105, female: 123), who were hospitalized with several systemic diseases in a community-based acute-care hospital from May 2014 to October 2016, were retrospectively analysed by multiple logistic regression. RESULTS: Multiple logistic regression analysis shows that bacterial pneumonia has a statistically strong relationship with oral candidiasis (p = 0.000, OR: 5.173,

The Italian Community-based Pulmonology Manifesto: a national survey exploring the current state, critical issues, and perspectives for a national reference model.

In Italy, demographic aging, increasing multimorbidity, and the growing burden of chronic respiratory conditions have highlighted the need for a reorganization of community-based respiratory services, as outlined by the Italian Ministerial Decree 77/2022, which promotes integrated, proximity-based models of care. Community-based pulmonology care represents a potentially pivotal component of healthcare systems for the management of chronic and complex respiratory diseases. A national, anonymous, web-based survey was conducted among Italian pulmonologists working in territorial and community hea

Removing a Disabled Person from Her Treasured Independent Living.

Ms. X is a person with cerebral palsy and schizophrenia. She has intractable bedsores that are a result of her immobility and to poor wound care related to her delusional thinking. Despite intensive community support, the wounds have worsened to the point that she has needed multiple hospitalizations to prevent systemic sepsis, a life-threatening condition. She is capable of placement decisions and wishes for independence at home but is incapable of making wound care decisions and does not appreciate that immediately returning home from the hospital, instead of going into a special care facili

Patient-Reported Outcome and Experience Measures (PROMs and PREMs) in Pressure Injury Prevention and Care Across European Hospital and Community Nursing Settings: A Scoping Review.

Pressure injuries are a frequent and largely preventable adverse event across the whole care continuum, from intensive and acute hospital wards to community and home settings. Internationally, they are graded with the objective European Pressure Ulcer Advisory Panel and National Pressure Injury Advisory Panel (EPUAP/NPIAP) staging system. Staging describes the tissue rather than the person. How patients experience each stage is captured by patient-reported outcome measures (PROMs), and how they experience prevention is captured by patient-reported experience measures (PREMs). Both may be recor

Viewpoint from facilities to communities: Breastfeeding promotion in Baby-Friendly Provinces in Türkiye.

BACKGROUND: While Baby-Friendly Hospital initiatives play a critical role in the initiation of breastfeeding and the prevention of prelacteal feeding, growing evidence highlights the need for context-specific, equitable, and community-based breastfeeding support strategies that extend beyond the early postnatal period. MAIN BODY: In Türkiye, the scope of baby-friendly practices has progressively expanded from hospital settings to encompass the broader health system and community level. This expansion includes the introduction of the Baby-Friendly Family Medicine Initiative, the Baby-Friendly

Impact of infections on the incidence of acute inflammatory demyelinating polyneuropathy in children.

ObjectivesAcute inflammatory demyelinating polyneuropathy (AIDP) is the leading cause of acute flaccid paralysis in children and hypothesized to be triggered by antecedent infection. We sought to determine the association between AIDP and commonly acquired community infections in children. We utilized the reduction in these infections due to measures during coronavirus disease 2019 (COVID-19) to serve as a natural experiment and determine their contribution to AIDP.MethodsThis cross-sectional study used administrative and billing data from children's hospitals contributing to the Pediatric Hea

The National Palliative Care Research Center and the Center to Advance Palliative Care: a partnership to improve care for persons with serious illness and their families.

The elimination of suffering and the cure of disease are the fundamental goals of medicine. While medical advances have transformed previously fatal conditions such as cancer and heart disease into illnesses that people can live with for many years, they have not been accompanied by corresponding improvements in the quality of life for these patients and their families. Living with a serious illness should not mean living in pain or experiencing symptoms like shortness of breath, nausea, or fatigue. Yet, multiple studies over the past decade suggest that medical care for patients with advanced

Rapid molecular detection of respiratory pathogens in patients admitted with suspected community-acquired pneumonia: secondary analysis of a randomized controlled trial.

UNLABELLED: In a randomized trial, we demonstrated that implementing point-of-care (POC) PCR detection of respiratory pathogens in an emergency department (ED) setting did not change overall antibiotic use but led to more targeted treatment and a clinically relevant, albeit non-significant, reduction in length of stay. This study aimed to assess the diagnostic impact of POC-PCR testing in patients with suspected community-acquired pneumonia (CAP) in the ED. This study is a secondary analysis of a Danish multicenter randomized controlled trial (RCT) that included patients aged ≥18 years admitte

'There was no other way things could have been.' Greenlandic women's experiences of referral and transfer during pregnancy.

The World Health Organization (WHO) has stated that referral practices along with midwifery care are a means of heightening the quality of perinatal care and lessening perinatal mortality and morbidity. In 2002, in response to high perinatal mortality and morbidity, a referral system was instituted nationally in Greenland, transferring all at-risk pregnancies to its national referral hospital. Little or no current research has focused on evaluation of the perinatal referral system or on the thoughts, beliefs, opinions and challenges faced by women and Greenlandic families themselves. The aim o

Passageway: A Novel Approach to Success of Conditional Release - Principles and Constructs of the Model Residential Program for the Forensic Mentally III Patient.

With the advent of psychotropic medications and with the deinstitutionalization of psychiatry starting in 1968, patients were prematurely discharged from forensic state hospitals. Due to lack of resources, psychiatric forensic patients ended up in the correctional system or homeless with the reduction of psychiatric beds in forensic and civil state hospitals. Lacking proper training and medication management, the recidivism rate of this population was close to 10% for rearrest and about 35% for revocation of conditional release (CR; Manguno-Mire et al., ). A new treatment modality was created

"Access to healthcare is a human right": a constructivist study exploring the impact and potential of a hospital-community partnered COVID-19 community response team for Toronto homeless services and congregate living settings.

BACKGROUND: Individuals experiencing homelessness face unique physical and mental health challenges, increased morbidity, and premature mortality. COVID -19 creates a significant heightened risk for those living in congregate sheltering spaces. In March 2020, the COVID-19 Community Response Team formed at Women's College Hospital, to support Toronto shelters and congregate living sites to manage and prevent outbreaks of SARS-CoV-2 using a collaborative model of onsite mobile testing and infection prevention. From this, the Women's College COVID-19 vaccine program emerged, where 14 shelters wer

[Palliative Care at Home by Anesthesiologists].

Governmental policies recommend the palliative care at home for cancer patients. However, there are some barriers to discharge cancer patients from the hospital who want to receive end-of-life care at their homes. Anesthesiologists whose main job is to perform general anesthesia in the operating theater usually have little contact with general practitioners giving community palliative care. So it is important to communicate each other to make opportunities to discuss an organized system and care plan for these patients, and to improve information transfer.

Social Risk Screening and Referrals in the Pediatric Inpatient Setting.

BACKGROUND/OBJECTIVES: Many professional organizations recently recommended inpatient screening for social risks. Although prior studies have explored social risk screening rates in children's hospitals, many excluded community hospitals and lacked detail on screening practices. We aimed to describe national social risk screening and referral practices among hospitals caring for children. METHODS: We conducted a cross-sectional survey (August 2023-November 2023) through the Pediatric Research in Inpatient Settings Network. One clinical leader per institution was surveyed on inpatient social r

Hospital Human Resource Managers' Perspectives on Organizational Readiness for Generative AI Skills: Qualitative Descriptive Study.

BACKGROUND: Generative AI (GenAI) is increasingly entering health care through documentation support, communication tools, educational content generation, and other knowledge-intensive functions. However, organizational adoption remains uneven, and concerns related to privacy, security, output reliability, governance, workflow fit, and infrastructure continue to limit broader implementation. Although the literature increasingly discusses the skills health care workers may need in the GenAI era, less is known about how hospital managers view the organizational readiness required before such ski

Modeling Culvert Failure and Community Accessibility Impacts During Hazards.

Culverts (or road-stream crossings) are prone to failure during extreme events such as flooding. However, culverts are considered small-scale infrastructure and have been understudied and overlooked. The impact of culvert failure could be far-reaching and is especially damaging in rural networks where there is less redundancy in the transport system when compared to a dense urban network. Culvert failure can block road infrastructure and disconnect community members from places of interest such as hospitals. Current work on culverts has focused on improving component level condition ratings. T

The hidden microbiome of hospital infection surveillance testing: biomarkers of health outcomes in MRSA and VRE colonization.

BACKGROUND: Hospital-acquired infections present a major concern for healthcare systems in the U.S. and worldwide. Drug-resistant infections result in increased costs and prolonged hospital stays. Methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE) are responsible for many drug-resistant infections in the U.S. We undertook two parallel studies aimed to investigate the differences in the microbial communities of individuals colonized with MRSA (or VRE) as compared to their respective non-colonized counterparts matched for age, sex, race, ethnicity, uni

A scoping review of generative AI and digital twins in tackling loneliness during hospital-to-home transitions.

Loneliness is an important psychosocial concern for older adults and may be intensified during the transition from hospital to home, when established routines, professional contact, functional capacity, and social participation can change simultaneously. This scoping review examined how generative artificial intelligence (GenAI), AI-enabled technologies, and digital twins (DTs) have been investigated in relation to loneliness, social connectedness, and psychosocial support for older adults, with particular attention to their relevance to hospital-to-home transitions. Design of an Event-Driven, Low-Power Wearable Fall Detection Device for Hospital Use in Older Adults: A Proof-of-Concept.

As life expectancy increases, a growing proportion of hospitalized older adults require continuous medical supervision during inpatient care. Falls among hospitalized older adult inpatients are a major patient-safety challenge, associated with increased injury rates, prolonged hospital stays, and higher healthcare costs. Adoption of existing fall detection solutions has been limited by patient privacy concerns, high implementation costs, inconsistent detection performance, such as false alarms, limited battery life, and discomfort associated with intrusive wearable designs. This study presents
