[Internal screwed plate for recent fractures of the humeral diaphysis in adults].

PURPOSE OF THE STUDY: Can medial plating of the humerus, through an antero lateral approach, diminish incidence of iatrogenic radial palsies? MATERIAL AND METHODS: We carried out a prospective study of medial plating of humeral shaft fractures through an antero lateral approach between 1988 and 1997. 41 fractures were fixed, 36 were followed up for a mean period of 5.8 years. The indications were multiple injuries (10), displaced fractures (23), and failure of conservative treatment (3). Road traffic accidents and sports injuries were the cause in 68 per cent of cases. Two fractures were open

Nonoperative treatment for minimally and nondisplaced lateral humeral condyle fractures in children.

The radiographic and clinical records of 95 children with nondisplaced or minimally displaced fractures of the lateral humeral condyle treated on a nonoperative protocol were reviewed. Acute nondisplaced or minimally displaced fractures were defined as < 24 h old on initial evaluation and displaced < 2 mm in three radiographic planes (anteroposterior, lateral, and internal oblique). Closed treatment and close follow-up in a long-arm cast or splint resulted in a union rate of 98% in 3-7 weeks. Two of the fractures displaced required open reduction and internal fixation and then subsequently wen

Tips of the trade #41. Modified coaptation splint for humeral shaft fractures.

A technique for applying a modified coaptation splint for humeral shaft fractures is presented. Using this method, the splint extends from the base of the neck, passes over the shoulder and along the front and back of the arm, and returns toward the axillary fold. To prevent slippage, the proximal end of the splint is secured to the chest with a strap.

Comminuted intraarticular fractures of the distal humeral condyles. Surgical vs. nonsurgical treatment.

Forty-two comminuted intraarticular fractures of the humeral condyles treated between 1975 and 1981 were analyzed in order to compare the results of operative versus nonoperative management. Twenty-nine patients were treated by open reduction and internal fixation; the remainder were treated by nonoperative techniques. Functional results were evaluated by the method of Bickel and Perry. Of the patients treated by open reduction and internal fixation, 76% had an excellent or good result. Of the nonsurgically-treated patients, only eight percent had a satisfactory result. Anatomic restoration by

Delayed union of humeral shaft fractures treated by closed flexible intramedullary nailing.

Ten patients with humeral shaft fractures and no clinical or radiographic signs of healing after at least six weeks' immobilisation were treated by flexible intramedullary nailing using a closed retrograde technique. Bone grafting was not performed, and active movement was encouraged after operation. Nine fractures healed; the mean time to union was 10.5 weeks (range 6 to 22 weeks). One patient needed compression plating and bone grafting at 22 weeks, and another required re-operation for distal migration of the rods. There were no infections, nerve palsies or other complications. Stiffness of

Design and topographic evaluation of a canine humeral anatomic plating system (CHAPS) for humeral condylar fracture repair in spaniels.

OBJECTIVE: To design a precontoured plating system for humeral condylar fracture repair in spaniels with humeral intracondylar fissure (HIF) and quantify accuracy of fit of the system on 3D printed bone models. STUDY DESIGN: Experimental on 3D printed models. METHODS: The plating system was based on morphometric data obtained from CT. Plates were applied to models and impression material was measured under the plates at points of interest to define plate bone gap (PBG). Data on locking screw interference on plate application were also obtained. RESULTS: Eleven elbows models were plated. Sta

Unusual Case of a Traumatic Proximal Humeral Physeal Fracture in an Adolescent.

Shoulder injuries, specifically proximal humeral fractures, are uncommon in skeletally immature patients. The anatomic characteristics of the humerus are determined by its ossification development, which is quite particular in the proximal segment where four principal segments have been outlined as fracture components in adults. Hereby, we present the case of an 18-year-old female who suffered a traffic accident that elicited a proximal humeral injury through the physeal line of the anatomical neck, as well as the treatment given and her clinical outcome with an effort to highlight the complex

Humeral Head Fracture Dislocation with Displacement into the Mediastinum: A Case Report.

CASE: A 70-year-old woman pedestrian struck by a motor vehicle presented with multiple orthopaedic injuries including a humeral head fracture dislocation with a large segment of humeral head located in the mediastinum. Thoracic surgery personnel performed a minimally-invasive video-assisted thoracoscopic extraction of the humeral head, and the patient underwent subsequent reverse total shoulder arthroplasty. CONCLUSION: Intramediastinal displacement of the humeral head is a rare, yet serious traumatic injury that necessitates early recognition and comanagement with cardiothoracic or thoracic

Isolated fracture of the humeral trochlea: a case report and review of the literature.

INTRODUCTION: Isolated fracture of the trochlea is a rarely reported entity. To the best of our knowledge, only 15 cases have been published. We report the case of a patient with an isolated fracture of the trochlea and discuss through a literature review the underlying mechanisms and the clinical, radiological and therapeutic features of this lesion. This work will significantly advance our understanding of this particular fracture. CASE PRESENTATION: A 21-year-old Caucasian man received an elbow injury. An anteroposterior radiograph showed only an irregularity of the medial joint space, but

Comparison of the results of treatment of humeral shaft fractures by different methods.

The aim of this study was to compare the functional results of non-operative and different operative techniques of managing humeral shaft fractures. The average follow-up was one year after the trauma in 61 patients with united fracture, where the initial method of treatment was the definitive one. The patients were divided into four groups: 22 were treated with hanging cast, 20 with intramedullary nails, 10 with dynamic-compressive plates and screws and 9 with external fixators. We estimated the presence of bone union by radiographic and clinical evaluation and compared the results with Const

Humeral fracture between a total elbow and total shoulder arthroplasty.

This article presents a case of a 71-year-old woman with a humeral fracture between a cementless reverse total shoulder arthroplasty and a cemented total elbow arthroplasty and discusses our treatment plan. Surgical treatment was performed after the patient was informed of possible complications and the benefits of surgery including: early, complete restoration of arm anatomy, greater functional improvement of the adjacent joints, and increased risk of nonunion with nonoperative treatment. The fracture was comminuted and extended proximally around the shoulder prosthesis. Through the posterior

Anatomic and reverse shoulder prostheses in fracture sequelae of the humeral head.

OBJECTIVES: Arthroplasty for the proximal humerus problems secondary to fractures is troublesome, because of necrosis of the tubercles and the resulting insufficiency of the rotator cuff. The aim of this study was to investigate whether better results can be achieved with the differential use of anatomic and reverse shoulder prostheses, in comparison to the preoperative status. METHODS: Fifty-five patients with secondary fracture prostheses due to sequelae of fractures of the humeral head were followed. Anatomic prostheses were implanted in 36 cases (fracture sequelae types 1 and 2 according

Comminuted periprosthetic humeral fracture after reverse shoulder prosthesis.

Periprosthetic fractures represent a severe complication after joint replacement. A case of comminuted displaced humeral fracture around the stem of a SMR reverse shoulder prosthesis is reported. The patient was a 81-year-old lady who had had a total shoulder replacement 28 months previously. The surgical solution consisted of a partial revision of the modular implant with conservation not only of the glenoid component but also of the prosthetic humeral body, which was well fixed in the humeral metaphysis. The humeral stem was removed and a long uncemented revision stem was implanted providing

A prospective study of a new locking plate for proximal humeral fracture.

Displaced and unstable extra-articular fractures of the proximal humerus are treated by operative reduction and fixation using various techniques such as fixed plates and intramedullary implants. This prospective study evaluates the clinical efficacy of a novel implant that is attached to the humeral head with fixed-angle locking screws. Between October 2002 and November 2005, 99 people (mean age 62.8 years) with proximal humeral fractures received treatment involving this plate in our department. The mean follow-up time was 17.8 months. Outcome was assessed with radiography, the Constant-Murl
