Responses of lymphocytes to Epstein-Barr virus in patients with primary immunodeficiencies.

Several reports have demonstrated that the responses of B-cells to Epstein-Barr virus (EBV) are variable in common variable immunodeficiency (CVID). In this study in patients with selected primary immunodeficiencies, i.e., Bloom's syndrome, Wiskott-Aldrich syndrome or IgA deficiency, the responses of peripheral blood mononuclear cells (PBMCs) to EBV were investigated. In the two patients with Bloom's syndrome, PBMCs stimulated with EBV showed decreased proliferation and immunoglobulin production, suggesting a mild abnormality of B-cells. In patients with Wiskott-Aldrich syndrome, the responses…

[Blood Transfusion in a Patient with Severe Anemia and Immunoglobulin A Deficiency during an Emergency Total Hysterectomy on a Holiday].

A 45-year-old woman diagnosed with immunoglobu- lin A (IgA) deficiency during blood donation and undiagnosed with anti-IgA antibodies posses … Intraoperatively, 155 g bleeding was noted, and the patient was discharged uneventfully. Selective IgA deficiency is the most common primary hypogamma- globulinemia and is less frequent in the Japanese than in Caucasians. Up to 40% of patients with IgA defi- ciency had anti-IgA antibodies that can cause anaphy- lactic reactions to IgA in transfused blood. Blood …

Celiac disease and severe vitamin D deficiency: the case for anti-tissue transglutaminase antibody screening.

UNLABELLED: Vitamin D-deficient Saudi adolescent girls were screened for anti-tissue transglutaminase (IgA-tTG) antibodies to determine whether the presence of severe vitamin D deficiency was associated with celiac disease. All 9 participants who were positive for IgA-tTG antibodies had severe vitamin D deficiency (25(OH)D < 12.5 nmol/l), suggesting that this population should be screened for celiac disease. PURPOSE: The current cross-sectional study aimed to see if severe vitamin D deficiency is associated with celiac disease (CD) among Saudi adolescent girls. METHODS: A total 200 adolescen

The role of anti-IgA antibodies in causing adverse reactions to gamma globulin infusion in immunodeficient patients: a comprehensive review of the literature.

Anaphylactic reactions to immunoglobulin infusions in immunodeficient patients with undetectable IgA have been attributed in several reports to IgG or IgE anti-IgA antibodies. However, other reports have not supported an association between such antibodies and the development of severe reactions. We have reviewed the articles reporting reactions to immunoglobulin products in IgA-deficient patients, as well as those describing the presence of such antibodies in the absence of reactions to infusions. A variety of factors might influence the association of adverse reactions with anti-IgA antibodi

Salivary antigliadin and antiendomysium antibodies in coeliac disease.

The definitive diagnosis of coeliac disease is based on typical changes in the small intestine biopsy specimens. To screen individuals for c … taneously for the presence of IgA and IgG AGA and IgA EmA. None of patients studied had a selective IgA deficiency. There was no significant difference in salivary IgA AGA levels between the three groups tested and there was no correlation between the individual serum and salivary values of IgA AGA. Salivary IgG AGA levels were very low or undetectable. Serum Ig …

Implementation of National Institute for Health and Care Excellence (NICE) guidance to measure immunoglobulin A with all coeliac screens: can an affordable solution be devised?

There has been a dramatic increase in requests for coeliac disease (CD) serological screening using immunoglobulin (Ig)A tissue transglutaminase antibodies (IgA-tTG). Recently, the UK National Institute for Health and Care Excellence has revised its guidance, recommending that total IgA should also be measured in all samples. This is justified, as false-negative results may occur with IgA deficiency. However, implementation of this guidance will incur considerable expense. Tests that measure IgA-tTG antibodies can detect IgA deficiency, indicated by low background signal. This provides an oppo…

MLL1 directs gut-associated antibody responses to helminth and bacterial infections.

Soil-transmitted helminths are one of the most common infections globally, yet how to promote effective gut-associated humoral responses is not well understood. We identify the histone methyltransferase MLL1 as a key target to promote IgA-driven responses. Mll1 was increased in germinal center B cells in gut-associated lymphoid tissues, and Mll1-deficiency led to changes in the histone modification H3K4me3 on key B cell and immune-regulatory genes. Correspondingly, MLL1-deficient B cells had defective germinal centers and IgG1 in response to the helminth Trichuris muris. Yet Mll1f/fCd23cre/+ m

Serum IgG subclass levels in patients with primary immunodeficiency syndromes or abnormal susceptibility to infections.

Serum IgG subclass levels were measured using an indirect competitive immunoenzymatic assay with monoclonal antibodies in 221 patients affected with definite immunodeficiency (ID) syndromes and 229 patients presenting with infection patterns suggestive of ID, but with normal immunoglobulin class levels and no clear evidence of ID. In common variable ID and IgG-IgA deficiency with normal or high IgM, subclass imbalance (mostly IgG1-IgG3 or IgG2-IgG4 deficiency) was the rule, with a higher incidence of severe infections in IgG2-IgG4 defects. One-fifth of patients with IgA deficiency, especially …

Cooperative control of IgA synthesis and secretion by MZB1 and the J chain.

Immunoglobulin A (IgA) is the most abundantly produced antibody in mammals, with its secretory dimeric form playing a central role in maintaining intestinal homeostasis. Although the J chain is known to be essential for IgA transcytosis, its precise role in IgA biosynthesis and secretion is not fully understood. Here, using CRISPR/Cas9-edited J558 plasmacytoma cells, a mouse line that secretes IgA, we demonstrate that MZB1 and the J chain act sequentially to ensure proper IgA assembly. MZB1 stabilizes α-heavy chain-light chain complexes (HL complexes), thereby enabling their efficient associat

[Primary antibody deficiency syndrome. Improved life expectancy and immunoglobulin G substitution].

Recurrent pyogenic infections are the leading clinical symptom in patients with antibody deficiency syndromes. In accordance with the WHO-classification at least nine different diseases are differentiated. In some cases diagnosis can be made simply by determining the immunoglobulin serum levels. Some diseases, such as Ig-heavy chain gene deletions, are clearly characterized by molecular biological methods, while other terms, such as common variable immunodeficiency, cover a collection of different diseases with changes predominantly of B-cell functions. The main features of the antibody defici

Fc receptor function and circulating immune complexes in gluten sensitive enteropathy--possible significance of serum IgA.

The capacity to clear IgG containing immune complexes from the circulation was studied in patients with coeliac disease (n = 13), dermatitis herpetiformis (n = 8), and coeliac disease with concomitant serum IgA deficiency (n = 4). A small group of patients with active ulcerative colitis (n = 4) was included as a bowel disease control group. Clearance was estimated by measuring the disappearance rate of a bolus dose of intravenously injected IgG coated autologous erythrocytes. The mean T1/2 of clearance was prolonged in both coeliac disease (86 (24) minutes) and dermatitis herpetiformis (111 (3…

[Selective deficit of IgA and infantile rheumatoid arthritis. Case reports].

Three children with selective deficiency of serum IgA associated with oligoarticular juvenile rheumatoid arthritis (JRA) are presented. Before the appearance of JRA, all 3 children had shown frequently some respiratory infections. Indeed IgA deficiency may be asymptomatic or may cause a higher frequency of respiratory, gastrointestinal, allergic or autoimmune diseases, among which, JRA is one of the most important conditions.

Causative mechanisms and clinical impact of immunoglobulin deficiencies in ataxia telangiectasia.

BACKGROUND: Ataxia telangiectasia (AT) is characterized by cerebellar ataxia, telangiectasia, immunodeficiency, and increased cancer susceptibility and is caused by mutations in the ataxia telangiectasia mutated (ATM) gene. The immunodeficiency comprises predominantly immunoglobulin deficiency, mainly IgA and IgG2, with a variable severity. So far, the exact mechanisms underlying the immunoglobulin deficiency, especially the variable severity, remain unelucidated. OBJECTIVE: We characterized the clinical impact of immunoglobulin deficiencies in AT and elucidated their mechanisms in AT. METHO

Jejunal secretion of secretory immunoglobulins and gliadin antibodies in celiac disease.

The aim of this study was to determine the secretion of secretory immunoglobulins and gliadin antibodies in the small bowel in celiac disease. Twenty-four patients were investigated by perfusion of a defined jejunal segment. Four of the patients studied had a serum IgA deficiency and had no measurable amounts of secretory IgA in the perfusion fluid. The other patients demonstrated a significant increase in the jejunal concentration of secretory IgA (median 28.5 mg/liter) compared with healthy controls (median 16 mg/liter, N = 16) and of IgM, celiac (median 12.3 mg/liter) compared to healthy co…

IgA-antitissue transglutaminase: validation of a commercial assay for diagnosing coeliac disease.

We have evaluated a commercial assay for serum IgA class antibodies to tissue transglutaminase, the enzyme identified as the major endomysial autoantigen in coeliac disease (CD). Sera were available from 130 adults diagnosed with CD in Southern Derbyshire between 01 01 97 and 31 12 99. Sera from 100 patients without villous atrophy on small intestinal biopsy were controls. The ability of the assay to detect abnormally low total IgA levels was assessed using sera from 18 subjects with IgA deficiency. Sensitivity and specificity of this IgA-anti tissue transglutaminase (tTGA) assay (86.2%, 91.0%…

Immunoglobulins on the surface of lymphocytes. IV. Distribution in hypogammaglobulinemia, cellular immune deficiency, and chronic lymphatic leukemia.

The distribution of peripheral blood lymphocytes that contain surface Ig has been studied by means of immunofluorescence in humans. Normal individuals, individuals with sex-linked and acquired agammaglobulinemia, selective IgA deficiency, cellular immune deficiencies, and individuals with chronic lymphatic leukemia (CLL) were studied. Approximately 28% of the peripheral blood lymphocytes from normal individuals contained surface Ig. On an average 15% contained IgG, 6%, IgA, and 8%, IgM; and the kappa: lambda ratio was 2:1. Lymphocytes from patients with CLL appeared to be "monoclonal" in that …

Origin of poorly galactosylated IgA1 other than mucosa: a viewpoint from a report on patient with IgA vasculitis.

Patients presenting monoclonal gammopathy of renal significance (MGRS) and IgA vasculitis collectively have rarely been reported. This study reports one patient with monoclonal IgA and λ, and with IgA positive-mesangial proliferative glomerulonephritis. The patient manifested slight chronic nephritic syndrome, and his serums tested positive for M protein (monoclonal IgA and λ). As a result of bone wear, the plasma cell ratio of these patients was confirmed to be mildly increased in peripheral blood smears. Just like typical IgA nephropathy or IgA vasculitis patients, serum poorly galactosylate
