Two kinds of immunosuppression must be distinguished: malignant diseases on one hand, and acquired immunodeficiency syndrome on the other hand. Ocular infections can involve any part of the eye and they can lead to blindness. They usually attest of a systemic infection. Their treatment consist of an induction course to provide complete healing of the lesions, immediately followed by a maintenance course to prevent relapses. Retinitis are the most common posterior involvement, especially the retinitis due to cytomegalovirus and Toxoplasma gondii. Zoster ophtalmicus is the most common anterior i

The manuscript collates indications for vaccination in immunocompromised adults based on recommendations from leading international institutions. These individuals have increased vulnerability to vaccine-preventable infectious diseases, and their immune response to vaccination is often weaker than that of immunocompetent individuals. Therefore, whenever possible, it is important to assess vaccine response (serological or other). Emphasis is placed on adapting vaccination to the net state of immunosuppression, following a careful evaluation of risks, indications, contraindications, and the opti

Non-tuberculous mycobacteria (NTM) are ubiquitous environmental organisms that can cause significant disease in both immunocompromised and immunocompetent individuals. The incidence of NTM pulmonary disease (NTM-PD) is rising globally. Diagnostic challenges persist and treatment efficacy is variable. This article provides an overview of NTM-PD for clinicians. We discuss how common it is, who is at risk, how it is diagnosed and the multidisciplinary approach to its clinical management.

Blood stream infections and pneumonia caused by Pseudomonas aeruginosa is associated with high mortality, especially in an immunocompromised host. A large section of the palliative care patient population has varied forms of compromised immunity due to advanced cancer or cancer treatment, organ failures, chronic autoimmune disorders, degenerative conditions, and acquired immunodeficiency syndrome. The lung is one of the most frequently involved organs in a variety of complications in an immunocompromised host and infection is the most common complication. P. aeruginosa is one of the most commo…

Histoplasma capsulatum is a dimorphic pathogenic fungus and causative agent of histoplasmosis, which is a respiratory and systemic infection that is particularly severe in immunocompromised hosts and represents the fungal homolog of tuberculosis. In highly endemic regions, the majority of individuals have been infected and carry the organism in a persistent latent form that is a danger for reactivation if host defenses are suppressed. H. capsulatum has been a model organism for intracellular pathogenesis and fungal morphogenesis for decades. New genomic information and application of approache…

Histoplasmosis occurs throughout the world but is more common within the endemic areas of North America, particularly in fertile river valleys. Disease manifestations range from asymptomatic infection in the normal host with low-inoculum exposure to rapidly fatal, disseminated infection in the severely immunocompromised host, emphasizing the importance of cellular immunity in defense against Histoplasma capsulatum. Diagnosis depends on a high index of suspicion, knowledge of the clinical and epidemiologic features of the infection, and a thorough understanding of the uses and limitations of fu…

The immunocompromised host was first defined around the host abnormalities and consequent infections that were associated with congenitally acquired deficiencies. Although the impact of the deficiency on the affected child was considerable, the numbers of such children remained small. With the advent of immunosuppressive therapy and cytotoxic regimens for the effective treatment of cancers and autoimmune diseases, an increasingly larger number of children became immunocompromised and thus subject to serious infectious complications. More recently a new population of immunocompromised children …

Invasive aspergillosis (IA), generally considered an opportunistic infection in immunocompromised hosts, is associated with high morbidity and mortality. IA commonly occurs in the respiratory tract with isolated reports of aspergillosis infection in the nasal sinuses, central nervous system, skin, liver, and urinary tract. Extra-pulmonary aspergillosis is usually observed in disseminated disease. To date, there are a few studies regarding primary and disseminated gastrointestinal (GI) aspergillosis in immunocompromised hosts. Only a few cases of primary GI aspergillosis in non-immunocompromise…

Antimicrobial prophylaxis is an important cornerstone for reducing morbidity and mortality of cancer patients. Important strides have been made in recent years in vaccination, drug prophylaxes and the use of growth-factor support. We detail these changes to the respective recommendations here.Patients with malignant disease are recommended to receive vaccinations against common respiratory pathogens (COVID-19, influenza, pneumococci, and RSV). For both influenza (now trivalent vaccine) and pneumococci (now PCV20), the preferred vaccine has changed. A VZV vaccination using an inactivated virus-

Mucormycosis is an aggressive and frequently lethal disease. Most patients with mucormycosis have poorly controlled diabetes mellitus and rhino-orbito-cerebral disease. Patients with hematologic malignancy and transplant recipients mostly present with rhino-orbito-cerebral or pulmonary disease. Prompt recognition of clinical symptoms and radiographic features of mucormycosis is required to establish timely diagnosis and initiate targeted therapy. Diagnosis is, historically, made by direct microscopy, culture, and pathology of biopsy tissue, but molecular methods are increasingly playing a role

OBJECTIVES: Given the complex role of immunity in dengue severity, we aimed to review the clinical course of dengue infection in immunocompromised patients.
METHODS: We conducted a systematic review of studies reporting outcomes among immunocompromised patients with laboratory-confirmed dengue infection. Meta-analysis using the Mantel-Haenszel method (fixed effects) was performed for studies with control groups. We registered the study with PROSPERO (No. CRD42021258930).
RESULTS: We included 115 studies. Among these, 30 studies compared immunocompromised (cases) and nonimmunocompromised (con
