Vitamin A supplementation for preventing morbidity and mortality in very low birthweight infants.

BACKGROUND: Vitamin A is necessary for normal lung growth and the ongoing integrity of respiratory tract epithelial cells. Preterm infants have low vitamin A status at birth and this has been associated with increased risk of developing chronic lung disease. Several studies have been undertaken to assess whether vitamin A supplementation beyond that routinely given in multivitamin preparations can reduce the incidence of this outcome. OBJECTIVES: To assess the benefit of supplementation with vitamin A in very low birthweight infants. SEARCH STRATEGY: Searches were made of the Oxford Database

Path analysis on the risk of mortality in very low birth weight infants.

OBJECTIVE: The purpose of this work is to clarify by path analysis which of the predictor factors of mortality in a cohort of 423 singletons VLBW infants born at Beilinson Medical Center during the years 1980-1990 (pre-surfactant era) exert direct or indirect effect on mortality. A direct risk factor was defined as a factor having an immediate effect on the outcome without working through an intermediate variable. By contrast, an indirect risk factor was defined as a factor that does not have a direct effect on the risk of mortality but instead works through an intermediate variable. Evaluatio

Morbidity and mortality among Egyptian neonates and infants: rates and associated factors.

Among 2,128 women residing in two Egyptian governorates with differing socio-economic backgrounds, who were interviewed after giving birth between 1994 and 1996, 181 experienced an early foetal death, 34 had stillbirths, and 1,908 each had a live birth. Among the live births, there were 35 neonatal deaths and 42 post-neonatal deaths. A high percentage of the women sought prenatal care, beginning early in pregnancy, from both government health facilities (GHF) and private physicians. Problem-based care was typically directed to private physicians whereas routine care was largely directed to GHF

Sociodemographic factors identify US infants at high risk of injury mortality.

OBJECTIVES: To identify sociodemographic predictors of infant injury mortality and to compare trends in injury mortality rates for high- and low-risk US infants from 1985 to 1991. DESIGN: Historical cohort. SETTING/STUDY PARTICIPANTS: The National Center for Health Statistics linked US infants (<1 year) born from 1985 to 1991 with death certificates. MAIN OUTCOME MEASURES: Multivariate regression was used to identify sociodemographic factors associated with injury mortality. The adjusted relative risks (RRs) of maternal age, education, marital status, number of other children, and infant bir

Very low birth weight infants--mortality and predictive risk factors.

AIMS: To determine the survival rates and risk factors associated with mortality in premature very low birth weight or VLBW (< or = 1500 grams) infants. METHODS: This is a part-retrospective and part-prospective study of VLBW infants admitted into the Special Care Nursery, University Hospital Kuala Lumpur, between August 1994 and July 1996. RESULTS: Of the 184 infants without lethal congenital malformations, 144 (78%) infants survived till discharge. The causes of death included respiratory diseases (63%), infections (30%), gastrointestinal abnormalities (5%) and intracerebral haemorrhage (2

Mortality among infants with high-risk congenital diaphragmatic hernia in Singapore.

Several factors suggested to predict mortality in congenital diaphragmatic hernia (CDH) have not always been applicable in different centers. A retrospective review was conducted of 19 consecutive neonates in Singapore in whom CDH was diagnosed within 12 hours of birth to identify factors associated with mortality. Of the 19 cases, 15 (79%) were diagnosed using antenatal ultrasonography. Eight (42%) underwent primary repair at a median age of 23 hours (range, 12 to 50 hours). Of the 19 infants, 15 died (mortality rate, 79%). Survivors until hospital discharge were compared with nonsurvivors. A

[Perinatal and neonatal mortality and morbidity in newborn infants up to 1,000 g].

World-wide intensive efforts have been made for more than 20 years to reduce the rate of preterm delivery, but so far without remarkable success. Preterm delivery accounts for 6-9% of all deliveries and very-low-birth-weight infants (< or = 1000 g) total up to about 60% of perinatal mortality. This study reports on a retrospective analysis of fetal outcome with 33 very-low-birth-weight-infants (< or = 1000 g) delivered at the Women's Hospital (University of Rostock) during the time from 1986 to 1992. The rate of early mortality was at 45.2%, survival rate was at 33% (n = 11). Caesarean section

Mortality and morbidity in infants born between 20 and 28 weeks gestation.

Of 149 infants with ultrasound evidence of gestational age, born in Ninewells Hospital at between 20 and 28 weeks gestation over a 5-year period, 50 were alive at birth. Of these infants, 21 died within 1 week, a further three died within 1 month and a further two within 18 months. Of the 24 survivors, 8 (33%) have significant handicap. The obstetric factors leading to delivery and the mode of delivery are discussed.

Persistent morbidity and mortality of protein calorie malnutrition in young infants with CF.

Young infants with cystic fibrosis (CF) are at risk for developing symptomatic protein-calorie malnutrition (PCM) characterized by hypoalbuminemia, edema, and anemia. We reviewed the hospital charts of all infants less than 12 months of age referred to our CF clinic between 1979 and 1982 and found nine patients with PCM (13%). Their courses were characterized by age less than 7 months (nine patients), frequent use of soy formula (eight patients), presence of gastrointestinal or respiratory symptoms before being diagnosed with CF (eight patients), severe respiratory distress often requiring mec

Perinatal factors, periventricular haemorrhage and mortality in very low birthweight infants.

In a population of 225 very low birthweight infants born over a 21 month period the cerebroventricular system was scanned by ultrasound. One third of the infants developed a periventricular haemorrhage; in 41% of infants the haemorrhage was detected before an hour of age and 66% of all haemorrhages occurred within the first 24 hours. Statistically significant associations with periventricular haemorrhage included vaginal delivery, endotracheal intubation and intravenous sodium bicarbonate when this was administered in the first 24 hours. In a stepwise regression analysis, however, these and ot

Predicting mortality in low-birth-weight infants with pulmonary interstitial emphysema.

Due to the high mortality associated with pulmonary interstitial emphysema in the low-birth-weight infant, a method was developed to predict the infants most at risk for death. This information will allow the discriminant selection of patients for future trials of an alternative method of mechanical ventilation, high-frequency ventilation. During a 3-year-period (July 1, 1979 through June 30, 1982), 70 infants were retrospectively analyzed to determine the clinical parameters important in predicting mortality. The infants at highest risk for death included those of younger gestational age, tho

[Late mortality among small premature infants. Significance of respiratory distress and bronchopulmonary dysplasia for early and late mortality among infants with birth weights lower than 1,501 g].

We have studied neonatal and late mortality rates among children with a birthweight of less than 1.501 grams. The sample consisted of children admitted to the neonatal intensive care unit at the Dept. of Pediatrics in Trondheim during 1985 and 1986. 58 children were admitted, three of whom had lethal malformations (two triploids, one trisomi 18). Of the remaining 55 children, 12 died in the neonatal period. This gives a neonatal mortality rate of 21.8%, which is comparable with the results reported from other centers. However, six children died after the neonatal period, giving a late mortalit

Gestational age- and birthweight-specific declines in infant mortality in Canada, 1985-94. Fetal and Infant Health Study Group of the Canadian Perinatal Surveillance System.

We studied infant mortality rates in Canada within specific gestational age and birthweight categories after using probabilistic techniques to link information in Statistics Canada's live births data base (1985-94) with that in the death data base (1985-95). Gestational age- and birthweight-specific mortality rates in 1992-94 were contrasted with those in 1985-87 with changes expressed in terms of relative risks with 95% confidence intervals [CI]. Statistically significant reductions in infant mortality were observed beginning at 24-25 weeks of gestation and extended across the gestational age…

Mapping Infant Mortality Inequities in Mexico: Ten Years (2014-2023) of Individual and Population Level Challenges.

Infant mortality is a key indicator of population health and health system performance. This national study analyzed infant mortality in Mexico, examining temporal trends, leading causes of death, territorial inequalities, and indicators of surveillance quality. Using official mortality microdata for 224 212 deaths, we assessed patterns by cause of death, sex, health system affiliation, city size, and state of residence. Infant mortality remained largely stable during the study period (10.7 per 1000 live births in 2014 to 10.6 in 2023), despite an increase in reported medical attendance at dea…

Has the decline ended? A concerning shift in infant mortality trends in Europe: an urgent analysis.

BACKGROUND: Recent years have shown an unexpected increase in infant mortality in Spain, raising concerns about maternal, socioeconomic, and healthcare factors. METHODS: We performed a retrospective analysis of national and European infant mortality data from 2009 to 2024. Joinpoint regression was used to detect trend changes. Socioeconomic, maternal age, and healthcare access factors were analysed to identify potential contributing elements. RESULTS: The analysis identified a significant upward trend in infant mortality from 2019 onwards. Delayed maternal age, socioeconomic disparities, and…

Mortality Trends for Infant Intracranial Hemorrhage in the United States, 1999-2020: A 22-year Analysis.

BACKGROUND: Intracranial hemorrhage (ICH) is a major cause of infant mortality and neurodevelopmental complications, yet national mortality trends and demographic disparities remain poorly characterized. METHODS: A retrospective analysis using the CDC WONDER Multiple Cause of Death database (1999-2020) identified infant (<1 year) deaths with ICH (ICD-10 P52) as an underlying or contributing cause. Mortality rates per 100,000 live births were calculated by demographic and geographic factors, and Joinpoint regression estimated annual and average annual percent changes (APC, AAPC) with 95% confi…

Hypertensive Disorders of Pregnancy, Preterm Delivery, and Infant Size: Which Mothers Have Highest Cardiovascular Disease Mortality?

BACKGROUND: Research on new-onset hypertensive disorders of pregnancy (HDP) and long-term maternal cardiovascular disease (CVD) death has focused on mothers of small-for-gestational-age infants rather than large-for-gestational-age infants. OBJECTIVES: We further explored this focus by investigating CVD death in mothers with HDP by gestational age at delivery across the full spectrum of infant birth size. METHODS: We used data from the Medical Birth Registry of Norway, the Norwegian National Population Register, and the Norwegian Cause of Death Registry, with information on mothers giving bi
