Candida auris Urinary Tract Colonization and Nosocomial Infection Control Strategies in Shanghai.

BACKGROUND: Candida auris is a multidrug-resistant pathogenic fungus that exhibits regional variation and is emerging in new clades worldwide. C. auris predominantly colonizes the urinary tract and respiratory system, thereby increasing the risk of bloodstream infection. OBJECTIVE: To evaluate the epidemiological characteristics of C. auris urinary tract colonization in Shanghai and assess the efficacy of prevention and control strategies using molecular epidemiology and clinical interventions to provide evidence-based guidance for hospital infection control. METHODS: Cases were identified u…

Case-Control Study on Risk Factors of CRE Infection in the ICU.

INTRODUCTION: In recent years, the increasing prevalence of carbapenem-resistant Enterobacteriaceae (CRE) poses a serious threat to clinical anti-infection therapy. This study aims to identify risk factors for CRE infection among intensive care unit (ICU) patients. METHODOLOGY: An unmatched case-control study based on a hospital was performed. Data on the general condition of patients, disease scores, invasive procedures, and the use of antibiotics before infection were collected. Univariate and multivariate Logistic regression were performed to analyze the risk factors for CRE infection. RE

Type I interferon signaling promotes early innate control of Borrelia burgdorferi infection.

Borrelia burgdorferi, the causative agent of Lyme disease, elicits a robust type I interferon (IFN-I) response that has been strongly associated with chronic inflammatory manifestations such as Lyme arthritis. Although IFN-I is induced early after infection, its functional contribution during the initial stages of B. burgdorferi infection has remained unclear. Here, we identify a critical protective role for IFN-I signaling in the early control of B. burgdorferi. At 5 days post-infection, mice lacking the IFN-I interferon receptor (IFNAR1) exhibited markedly elevated spirochetal burdens at the

Resilient gut microbiome is linked to immune control in HIV-1 infection.

BACKGROUND: HIV-1 affects millions of people globally, and in rare cases, individuals known as “elite controllers” (EC) maintain undetectable viral load without antiretroviral therapy (ART). Given the crucial role of the gut microbiota in maintaining immune system homeostasis, this study investigated its composition and functionality in people living with HIV (PLHIV) (including viremic individuals, ART-treated individuals, and ECs) and healthy donors (HD). Fecal samples were collected for quantification of short-chain fatty acids (SCFAs), 16S rRNA sequencing, and bioinformatic analysis of taxo

Influenza coinfection inhibits control of mycobacterial infection in a human challenge model.

Mycobacterium tuberculosis infection is a dynamic continuum. Clinical outcomes reflect complex host-pathogen interactions. Epidemiological and animal studies have suggested influenza coinfection as a risk factor for progression from contained infection to active disease, but human studies have been lacking. Using a whole blood luminescent mycobacterial growth inhibition assay within a human influenza challenge study, we show that influenza infection reduces immunological control of mycobacterial growth. Transcriptome-wide RNA sequencing, cytokine and cellular analyses of subjects' blood before

Advances in burn wound management: innovative strategies for healing and infection control.

Burn wounds are insults to the skin that can be caused by various sources, including thermal, electrical, or chemical sources, and even natural sources such as the sun. A burn wound is conventionally categorized into 3 distinct zones: (1) coagulation, (2) ischemia/stasis, and (3) hyperemia. In addition to the potential for physiological scarring, burn wounds can lead to microbial infections, such as pneumonia and methicillin-resistant Staphylococcus aureus, that are difficult to treat using conventional antimicrobial therapy. Patients whose burn wounds trigger a systemic inflammatory response

Neutrophils, not macrophages, aid phage-mediated control of pulmonary Pseudomonas aeruginosa infection.

BACKGROUND: The increasing prevalence of multidrug-resistant (MDR) bacteria has reduced the effectiveness of standard antibiotics, prompting renewed interest in bacteriophage (phage) therapy as an alternative or adjunctive treatment. Phage therapy offers high specificity, self-amplification at infection sites, and minimal disruption to the gut microbiota. However, clinical implementation is challenging, due to the risk of phage resistance and uncertainties regarding optimal dosing and immune interactions. METHODS: Previously, we demonstrated that a two-phage cocktail exhibited low immunogenic

Antibody-mediated control of anellovirus infection: evidence from people who inject drugs.

Infections with viruses belonging to the family Anelloviridae are widespread among humans. Although generally considered a commensal, there is evidence to suggest that these infections may be controlled by host immune responses. However, the mechanism of immune control remains unclear. Previous research has also suggested a possible role of anellovirus capsid spikes in immune evasion. To investigate the role of antibodies in controlling infection, we used AnelloScan to profile plasma collected every 6 months over 2 years from 10 persons who inject drugs (PWID). Participants were selected based

Infection control and wound care innovations in plastic surgery: a multidisciplinary approach.

An infection that develops after plastic or reconstructive surgery is one of the most common and difficult consequences to deal with. In addition to compromising functional recovery and cosmetic outcomes, these infections can greatly lengthen hospital stays, raise patient morbidity, and slow wound healing. Infection risk in plastic surgery is complex and multifaceted, necessitating an all-encompassing and preventative strategy. Strict aseptic surgical procedures, perioperative antibiotic prophylaxis based on procedure type and patient risk profile, and comprehensive preoperative skin preparati

Lichen Planus Following COVID-19 Infection and Vaccination. Matched Case-Control Study.

BACKGROUND: New-onset lichen planus (LP) development following COVID-19 infection/vaccination is reported. Since case series cannot be used to study exposure-outcome associations, we designed this matched case-control study to investigate whether COVID-19-related events and de novo LP are associated. METHODS: Patients with histologically confirmed LP, newly diagnosed at the National Research Center of Dermato-Venereology of Moscow (September 2020-December 2022), were considered. Sex/age/ethnicity-matched controls attending the same Center in the same period for consultations on conditions unr

STAT1-IFITM3 promotes autophagy in epithelial cells to control Cryptosporidium parvum infection.

Cryptosporidium parvum infects intestinal epithelial cells, causing cryptosporidiosis in humans and neonatal livestock. Autophagy is an important component of the defense against C. parvum infection. IFITM3 is an IFN-stimulated gene that regulates viral infection through autophagy; however, its role in C. parvum infection is unknown. Herein, IFITM3 levels increased significantly after C. parvum infection and promoted autophagy. IFITM3 overexpression up-regulated LC3B II/LC3B I, Beclin-1, ATG7, and ATG5, but down-regulated p62 during infection. In C. parvum-infected HCT-8 cells, IFITM3 interact

Deficiency in astrocyte CCL2 production reduces neuroimmune control of Toxoplasma gondii infection.

Toxoplasma gondii is an obligate intracellular parasite that infects one-third of the world's human population and establishes infection in the brain. Cerebral immune cell infiltration is critical for controlling the parasite, but little is known about the molecular cues guiding immune cells to the brain during infection. Activated astrocytes produce CCL2, a chemokine that mediates inflammatory monocyte recruitment to tissues by binding to the CCR2 receptor. We detected elevated CCL2 production in the brains of C57BL/6J mice by 15 days after T. gondii infection. Utilizing confocal microscopy a

CD4, but not Cxcr6, is necessary for control of Pneumocystis murina infection.

CD4+ T cells are critical to control of Pneumocystis infection, and Cxcr6 has been shown to be upregulated in these cells during infection, but the roles of CD4 and Cxcr6 in this setting are undefined. To explore this, mice deficient in CD4 or Cxcr6 expression were utilized in a co-housing mouse model that mimics the natural route of Pneumocystis infection. Organism load and anti-Pneumocystis antibodies were assayed over time, and immunohistochemistry, flow cytometry, and quantitative PCR were used to characterize host immune responses during infection. CD4 was found to be necessary for cleara
