Aromatase inhibitors for male infertility.

PURPOSE: Testosterone-to-estradiol ratio levels in infertile men improve during treatment with the aromatase inhibitor, testolactone, and resulting changes in semen parameters. We evaluated the effect of anastrozole, a more selective aromatase inhibitor, on the hormonal and semen profiles of infertile men with abnormal baseline testosterone-to-estradiol ratios. MATERIALS AND METHODS: A total of 140 subfertile men with abnormal testosterone-to-estradiol ratios were treated with 100 to 200 mg. testolactone daily or 1 mg. anastrozole daily. Changes in testosterone, estradiol, testosterone-to-est

Physiopathological determinants of human infertility.

Successful management of infertility involves the use of appropriate diagnostic tests and treatments, and knowledge of prognostic factors such as age of the female partner and duration of infertility. The assessment of infertility may reveal disorders that cause morbidity in the normally fertile population, which might or might not contribute to the infertility. Endocrine dysfunction is a significant cause of infertility due to amenorrhoea and dysfunctional uterine bleeding, and hirsutism is a familiar problem in the normal population. Some genetic abnormalities cause infertility in males and …

Fibroids, infertility and pregnancy wastage.

Uterine fibroids are often found in women of reproductive age. Different types of fibroids may affect reproductive outcome to a different extent, with submucous, intramural and subserosal fibroids being (in decreasing order of importance) a cause of infertility and pregnancy wastage. Fibroids may also produce a number of complications during pregnancy. Women who are scheduled for assisted conception should be advised to have submucous and possibly intramural fibroids removed prior to IVF. Large fibroids (>5 cm), wherever their location, should be considered individually, with the reproductive …

Y chromosome and male infertility.

Male factor infertility accounts for about half the cases of couple infertility. In more than 60% of cases the origin of reduced testicular function is unknown but they may have an unidentified genetic anomaly. Microdeletions of the long arm of the human Y chromosome are associated with spermatogenic failure and have been used to define three regions of Yq (AZFa, AZFb and AZFc) that are recurrently deleted in infertile males. Several genes have been identified within this region and have been proposed as candidates for infertility. Many of these genes encode proteins involved in post-transcrip…

Infertility following pelvic inflammatory disease.

OBJECTIVE: To assess the frequency of infertility after pelvic inflammatory disease (PID) and factors important in postinfectious tubal damage in an urban population at high risk for sexually transmitted diseases. METHODS: From a cohort of 213 women with PID documented by laparoscopy and/or endometrial biopsy, 58 women (27% of the initial cohort) were interviewed by phone 2 to 9 years after an index episode of PID. Data regarding the initial history, physical examination, microbiology, laparoscopic, and serologic findings, and data concerning interval contraception, subsequent pregnancy, subs…

Genetic contribution to male infertility.

Worldwide, of couples trying for a child, 2-7% fail to conceive. Extensive screening programmes of men attending infertility clinics show that chromosomal and gene disorders make a significant contribution to spermatogenic impairment. It appears that an orderly genome is essential for normal germ cell development, since numerical and structural chromosome abnormalities are found in association with germ cell breakdown. The most recent research indicates that genes on the Y chromosome and autosomes are involved in spermatogenic control.

Motherhood, motherliness, and psychogenic infertility.

This paper revives an issue dormant in recent psychoanalytic literature--that of psychogenic causes of infertility. Modern technology to overcome anatomic or physiological blocks to fertility has probably contributed to the assumption that the psychogenic issue is obsolete. However, a broader perspective on the vicissitudes of pregnancy and motherhood in women who have particular psychological problems warrants reconsideration of the rationale for psychoanalytic evaluation and treatment. Three case presentations illustrate similar psychological issues that may interfere with the ability or wis…

Treatment of endometriosis-associated infertility.

The choice of treatment options for endometriosis-associated infertility has been both controversial and complex, largely because of lack of data. In the last 10 years, better data from numerous studies with improved design support laparoscopic ablation and/or resection of lesions as the most successful for both minimal/mild and moderate/severe/extensive disease. Laparotomy should be performed when necessary. Observation alone is sometimes indicated in young women with minimal/mild disease. Hormonal suppression has no identifiable role, except perhaps for severe/extensive disease, before IVF o…

Infertility due to genital tuberculosis.

Tuberculosis is one of the most common causes of infertility in underdeveloped countries. In the United States and developed countries, in contrast, it is one of the rarest causes of infertility. A woman with primary infertility was diagnosed with extensive genital and intraperitoneal tuberculosis by laparoscopic exploration, and culture of tubercules and peritoneal fluid. The tubercules covered the viscera so extensively that it was impossible to identify the organs. Antituberculosis treatment almost completely resolved the disease.

Transrectal ultrasound in male infertility.

In the evaluation of the subfertile man who has severe oligospermia or azoospermia associated with a low volume ejaculate, the high resolution transrectal ultrasound (TRUS) has, in recent years, been a very important non-invasive imaging technique to detect abnormalities in the seminal vesicles, ejaculatory duct and the status of the prostate. We performed TRUS in 30 of these patients from January to December 1994 followed by scrotal exploration with vasogram using methylene blue, inspection of the epdidymis for dilatation and fibrosis together with testicular biopsy to evaluate testicular spe

Sexually transmitted diseases and infertility.

In both men and women, STD-associated genital infections may cause permanent damage to the reproductive tract resulting in sub- or infertility. In men, the wide zone between sterility and normal fertility makes it difficult to demarcate the precise role of infection on post-infection fecundity, but it seems less important than in women. The reproductive events were studied in a cohort of 1,309 pregnancy-seeking women, < or = 35 years of age, after laparoscopically verified acute salpingitis, and 451 women with normal laparoscopy. Tubal factor infertility (TFI) was diagnosed in 12.1% of the pat…

Preventable causes of male infertility.

The initial evaluation of the infertile man includes a diligent search for testicular factors, gonadotoxins, and coital factors so as to identify existing causes of impaired fertility and to prevent further dimunition in fertility. There are also prophylactic measures in the treatment of all men that can prevent future infertility, such as prompt correction of cryptorchidism, testicular torsion, genital infection, and adolescent varicocele and proper precautions to limit occupational, medical, and recreational gonadotoxins.

Relative infertility: modeling clinical paradoxes.

OBJECTIVES: To assume that a cause of relative infertility will decrease the monthly chance of conception (fecundability) in a dose-dependent manner and, by a mathematical model, to identify common clinical observations and paradoxes that are explainable within this hypothesis. DESIGN: An empirically based assumption of a population-mean fecundability of 0.2 and the accumulating probability of pregnancy equations and projections were used to examine over time the effects of diminishing such fecundability to one half, one fifth, and one twentieth of normal, and then reversing this effect with …

Testicular biopsy in male infertility.

A total of 134 testicular biopsies from infertile men were studied. The potentially treatable causes of infertility viz. Normal spermatogenesis with post-testicular obstruction, maturation arrest and hypospermatogenesis together constituted about 54 percent of all cases. The incurable causes viz., germ cell aplasia, tubular fibrosis and Klinefelter's syndrome together constituted about 42 percent of all cases. Testicular biopsy in male infertility is a very useful procedure since it gives a lot of information on the diagnosis and helps in planning treatment.

[Immunological causes of male infertility].

Sperm (spermatozoa and the various secretions of the accessory glands) with its very complex antigenic system is only produced from puberty and thus at a time when the body has already had a for a long time the facility to respond to an antigenic stimulation with an immune reaction. Because of this, the spermatozoa, in contrast to other cells of the organism, are considered as strangers and may behave as auto-antigens. However, the existence of a haemato-testicular barrier, that prevents all contact between the sperm and the immune system must be recognized in man, its rupture has the conseque

[Mycoplasma infections in marital infertility].

Evaluation of 302 infertile married couples has identified Mycoplasma infection in 106 (35.5%) of women and 96 (31.8%) of men. The infection was associated primarily with cervical and endometrial abnormalities in the women and sperm abnormalities in the men. Tetracycline treatment resulted in clinical improvement both in the women and men. This study suggests Mycoplasma infection to be an etiological factor in infertile marriage.

Sulphasalazine-induced reversible male infertility.

A male patient suffering from ulcerative colitis, presented with primary infertility due to sulphasalazine therapy. Sulphasalazine was discontinued and the patient was treated with a 5-aminosalicylic acid preparation (Salofalk) in the form of an enema. After 3 months, the semen quality was dramatically improved and a successful pregnancy ensued. The sulphapyridine moiety of sulphasalazine seems to be responsible for male infertility and depressed semen quality. The metabolite 5-aminosalicylic acid is proposed as a suitable alternative to sulphasalazine in cases of male infertility.

Gonadotropin therapy of female infertility.

Anovulatory infertility in 134 women was treated with gonadotropins for a total of 318 cycles. The patients were classified into WHO group I, hypothalamic-pituitary failure (72 patients), and WHO group II, hypothalamic-pituitary dysfunction (62 patients). All patients in this group had failed to achieve pregnancy with clomiphene citrate therapy in repeated cycles. The pregnancy rate in group I was 72.2% vs 17.7% in group II. The 'take home' baby rate was 57.1% in group I vs 13.1% in group II. The rate of miscarriages was 14.3% without any significant difference between the groups. Multiple pre…
