Effect of Chinese Herbal Medicine on Male Infertility.

Male infertility normally refers a male's inability to cause pregnancy in a fertile female partner after 1 year of unprotected intercourse. Male infertility in recent years has been attracting increasing interest from public due to the evidence in decline in semen quality. There are many factors contributing to the male infertility including abnormal spermatogenesis; reproductive tract anomalies or obstruction; inadequate sexual and ejaculatory functions; and impaired sperm motility, imbalance in hormone levels, and immune system dysfunction. Although conventional treatments such as medication

Current medical management of endocrine-related male infertility.

Male factor contributes to 50%-60% of overall infertility but is solely responsible in only 20% of couples. Although most male factor infertility is ascertained from an abnormal semen analysis, other male factors can be contributory especially if the sample returns normal. Male infertility can be due to identifiable hormonal or anatomical etiologies that may be reversible or irreversible. This manuscript will highlight existing guidelines and our recommendations for hormone evaluation for male infertility and empiric therapies including multivitamins, estrogen receptor modulators (clomiphene),

Robotic surgery in male infertility and chronic orchialgia.

PURPOSE OF REVIEW: The use of robotic assistance during microsurgical procedures is currently being explored in the treatment of male infertility and patients with chronic testicular pain. Whether the addition of this technology would allow a corresponding improvement in outcomes as when the operating microscope was introduced in microsurgery is yet to be seen. RECENT FINDINGS: The present review covers new robotic microsurgical tools and applications of the robotic platform in microsurgical procedures such as vasectomy reversal, varicocelectomy, denervation of the spermatic cord for chronic

[Cases of Leydig cell tumor in male infertility].

1745 patients at the Department of Urology of Tenon Hospital (Paris, France) from 1990 to 2006 and at the department of urology of Tbilisi State Medical University (Georgia) during last several months have been examined and counseled on male infertility. Leydig cell tumor was found in 4 patients, among them 3 by palpation and testicle echography, fourth patient (at the age of 33) with bilateral varicocele (III stage at the left, II stage at the right) was more interesting for us. Leydig cell tumor was found out at the scrotum exploration. Oligoteratozoospermia (OATS) has been distinguished fro

Androgen receptor polymorphisms and mutations in male infertility.

Normal spermatogenesis depends on a sequential cascade of genetic events triggered by factors encoded by sex chromosomes. To determine the contribution of genetic aberrations to male infertility, the X-linked androgen receptor (AR) gene was examined for mutations and polymorphisms in a large cohort of infertile men. Genetic screening of over 400 patients and controls showed that defects in the AR gene lead to the production of dysfunctional receptor protein in up to 10% of males with abnormally low sperm production and male infertility. The dozens of mutations and polymorphisms uncovered were

[Sexual dissatisfaction and somatic complaints in male infertility].

Several relevant sociodemographic and psychological variables were assessed in addition to clinical and andrological examinations in order to find predictors for sexual satisfaction and somatisation in 68 infertile men of involuntarily childless couples. Subjects reported relatively high average levels of both sexual satisfaction and somatisation, with considerable variance between subjects. Multiple regression analyses revealed that neither age, attitudes towards sexuality, nor andrological findings had an influence on sexual satisfaction. However, an inverse relationship between sexual satis

[Intra-uterine inseminations with hyperstimulation in male infertility].

Our aim is to evaluate the efficacity of intrauterine insemination in male infertility after controlled ovarian hyperstimulation. 96 cycles were carried out. 16 pregnancies were achieved, the pregnancy rate per cycle is 16.7% (43.3% per couple) and 12.5% ongoing pregnancies. Six cycles were proposed and the higher cumulative probability of pregnancy is obtained at the 4th cycle. The best pregnancy rate per cycle is obtained with 5.10(6) to 10.10(6) inseminated. The success rate is not associated with the number of sperm abnormalities in our study.

Diagnostic value of bioactive FSH in male infertility.

Bioactive FSH and immunoreactive FSH were determined in 193 infertile men and in 23 men with proven fertility using the Sertoli cell aromatase bioassay for bioactive FSH measurement and a two-site fluoroimmunoassay for immunoreactive FSH measurement. Overall bioactive and immunoreactive FSH levels correlated well (r = 0.74, p less than 0.001) but were significantly different from fertile men (bioactive FSH: 6.2 +/- 0.3 U/l; immunoreactive FSH: 4.1 +/- 0.4 U/l) in patients with Klinefelter's syndrome (24.1 +/- 6.1; 26.9 +/- 3.0), non-obstructive azoospermia (25.1 +/- 4.3; 22.2 +/- 4.0), maldesc

[Male infertility with chromosomal abnormalities. I. XYY syndrome].

Chromosomal abnormalities are found in a considerably high percentage of cases of male infertility, in particular azoospermia. We report a case of the XYY syndrome and review the literature. A 36-year-old man, a factory hand, presented with infertility. He was safely delivered at term as a fourth child when his father was 41 years old and his mother 38. He had no delinquent or criminal record. His height was 179 cm, weight 75 kg and distance of extended hands 184 cm. No gynecomastia was noticed. Both testes were 8ml in size and growth of pubic hair and penis were normal. Severe oligozoospermia

Male infertility: etiologic factors in 385 consecutive cases.

During a three years period, evaluation of etiologic factors in 385 consecutive infertile men revealed chronic prostato-vesiculitis as the commonest cause (25.7%) followed by varicocele (21.8%). In 77% of the patients with chronic prostatitis the authors found a decreased spermatozoa motility rate. AN obstruction of the sperm duct was found in 7,8%, autoagglutination in 5.7%. Idiopathic infertility was diagnosed in 10.9% of the patients. The significance of these findings and the contributions of other causes in this series are discussed.

Unraveling the complex interplay: HIV and male infertility.

This review elucidates the complex interplay between HIV infection and male infertility, underscoring the multifaceted mechanisms through which HIV undermines male reproductive health. These mechanisms encompass diminished semen quality, orchitis, hypogonadism, and leukocytospermia. Concurrently, while antiretroviral therapy (ART) is salvific, it may pose additional fertility challenges. The introduction of highly suppressive ART has revolutionized the reproductive landscape for HIVserodiscordant couples, enabling them to plan for children with minimal risk of HIV transmission, thereby justify

Obesity and Male Infertility: True, True, and Unrelated?

While the prevalence of obesity has rapidly increased worldwide, there has also been a notable decline in semen parameters over the last several decades. While obesity can negatively impact reproductive hormones, many studies have sought a link between rising obesity and decreased male fertility potential. Nonetheless, few data support a direct link between the two. The focus on obesity as a causative factor in male infertility can potentially result in patient harm through delayed fertility treatment and missed diagnoses. This review investigates the associations between obesity and male infe

Environmental pollutants and male infertility: Effects on CatSper.

Infertility is a growing health concern among many couples worldwide. Men account for half of infertility cases. CatSper, a sperm-specific Ca2+ channel, is expressed on the cell membrane of mammalian sperm. CatSper plays an important role in male fertility because it facilitates the entry of Ca2+ necessary for the rapid change in sperm motility, thereby allowing it to navigate the hurdles of the female reproductive tract and successfully locate the egg. Many pollutants present in the environment have been shown to affect the functions of CatSper and sperm, which is a matter of capital importan

Achilles' heel of male infertility: good LEGO players.

In a recent journal article, Chen et al. identified a germ cell-specific cofactor, STYXL1, associated with male fertility function. Deletion of STYXL1 prevents the LEGO player CCT complex from properly folding key microtubule proteins of the sperm flagellum, which affects sperm motility and male fertility function.

Sperm RNA code in spermatogenesis and male infertility.

Spermatozoa are traditionally thought to be transcriptionally inert, but recent studies have revealed the presence of sperm RNA, some of which is derived from the residues of spermatocyte transcription and some from epididymosomes. Paternal sperm RNA can be affected by external factors and further modified at the post-transcriptional level, for example N6-methyladenosine (m6A), thus shaping spermatogenesis and reproductive outcome. This review briefly introduces the origin of sperm RNA and, on this basis, summarizes the current knowledge on RNA modifications and their functional role in sperma

Circular RNAs: novel noncoding players in male infertility.

Infertility is a global problem being associated with emotional and financial burden. Recent studies have shown contribution of a group of non-coding RNAs, namely circular RNAs (circRNAs) to the etiology of some infertility conditions. CircRNA are transcribed from exons and form a circular RNA molecule, being abundant in eukaryotes. Traditionally classified as non-coding RNA, these transcripts are endogenously produced through either non-canonical back-splicing or linear splicing, typically produced from precursor messenger ribonucleic acid (pre-mRNA). While during the canonical splicing proce

[Research progress on male infertility caused by prostatitis].

Prostatitis, as a common disease in urology, accounts for one-fourth of the outpatient volume in urology clinics. The number of patients is increasing year by year. In particular, chronic prostatitis not only affects the quality of life of patients but also often poses challenges for doctors in outpatient clinics. In recent years, male health issues have also attracted much attention, especially male infertility. Studies have shown that prostatitis lead to male infertility through a variety of mechanisms. However, there were few comprehensive discussions on male infertility caused by prostatit
