Intestinal protozoa.

Giardia is the best known cause of protozoal gastrointestinal disease in North America, producing significant but not life-threatening gastrointestinal distress and diarrhea. Although diagnosis of giardiasis may be challenging, treatment is usually successful. Entamoeba histolytica poses a rarer but far more difficult clinical challenge. Dysentery caused by E. histolytica may be the most feared intestinal protozoal infection, although Cryptosporidium parvum, Balantidium coli, Isospora belli, Sarcocystis species and other newly described protozoa also may cause diarrhea in healthy individuals a

Intestinal permeability.

Damage to the mucosal barrier may be assessed, non-invasively by use of sugar probes, which permeate through the transcellular or paracellular (tight junction) routes. A standardised test, with analysis of a five hour urine collection has proved useful in studying the sequelae of non-steroid anti-inflammatory drug (NSAID) administration, untreated coeliac disease, and enteric infections. Choice of probe molecule is crucial and lactulose/l-rhamnose seem to be satisfactory, in contrast with polyethylene glycol. Significant correlations have been seen between permeability and plasma IgA concentra

Intestinal transplantation.

Intestinal transplantation is often the only alternative form of treatment for patients dependent on total parenteral nutrition for survival. Although a limited number of intestinal transplantations have been performed, results with FK 506 immunosuppression are comparable to those for other organ transplants. The impact of successful intestinal transplantation on gastroenterology will likely be similar to the impact of kidney and liver transplantation on nephrology and hepatology.

Intestinal endometriosis.

Two cases with intestinal endometriosis, one with a right iliac fossa mass and the other with subacute intestinal obstruction, are presented. In both the cases, the diagnosis was not suspected clinically and was reached only on histological examination of the resected specimens.

Intestinal capillariasis.

Intestinal capillariasis caused by Capillaria philippinensis appeared first in the Philippines and subsequently in Thailand, Japan, Iran, Egypt, and Taiwan, but most infections occur in the Philippines and Thailand. As established experimentally, the life cycle involves freshwater fish as intermediate hosts and fish-eating birds as definitive hosts. Embryonated eggs from feces fed to fish hatch and grow as larvae in the fish intestines. Infective larvae fed to monkeys, Mongolian gerbils, and fish-eating birds develop into adults. Larvae become adults in 10 to 11 days, and the first-generation …

The carcinoid of the intestine.

From 1956 to 1985, 38 patients with a carcinoid of the intestine were treated. Tumours originating from the duodenum, ileum and colon had the worst prognosis. Fourteen out of 15 patients showed lymph-node metastases, and eight liver secondaries. Metastatic spread was rare (9%) if the primary tumour was small or an incidental finding (appendix). Even with metastatic spread there may be a longer survival due to the slow growth pattern of carcinoid metastases, which often become manifest several years after resection of the primary tumour. Metachronous liver metastases should be considered for su

[Intestinal parasitoses].

While intestinal parasites are infrequent causes of acute diarrhea, investigations in travellers, children, homosexuals and immunocompromised persons who have had diarrhea for more than 2-4 weeks should include stool analyses for intestinal parasites. Intestinal helminthiases are treated with pyrantel pamoate, mebendazole, thiabendazole, albendazole, praziquantel or niclosamide. For treatment of intestinal protozoa, ornidazole (or other benzimidazoles), diloxanide furoate and co-trimoxazole are drugs of first choice; at present spiramycin is recommended for treatment of cryptosporidiosis in AI

The intestinal tract.

In this chapter, we have considered angiographic applications for diagnosis and management of patients with gastrointestinal bleeding and intestinal ischaemia. For patients with upper gastrointestinal bleeding, diagnostic angiography should be performed only when and if endoscopy is not available or non-contributory. For therapeutic purposes, angiography is applied in patients who continue to bleed despite conservative measures and intervention becomes unavoidable. Therapeutic interventions include the intra-arterial infusion of vasopressin and/or transcatheter embolization. In patients with r

Intestinal endometriosis.

Endometriosis involving the intestine usually takes the form of asymptomatic, small, superficial serosal implants on segments of bowel lying in the pelvis in proximity to the genital organs. Deeper and more extensive intestinal wall involvement may result in obstruction and occasionally bleeding and requires distinction from a neoplasm or other inflammatory bowel process. Intestinal endometriosis should be considered in the differential diagnosis of recurring lower abdominal pain and other episodic bowel symptoms in women of child-bearing age. The diagnosis may be suspected based on the patien

Intestinal atresia.

Forty-five patients with intestinal atresia, including 20 with duodenal, 21 with jejunoileal, and 3 with colonic obstruction, were encountered at the Yale-New Haven Hospital between 1970 and 1976. The overall survival rate in the 43 operated cases was 93 per cent. The major reasons for the excellent operative survival with this malformation include: (1) the care received in a regional neonatal center; (2) the early recognition and appropriate case selection, denying operation unless mandated in babies with duodenal atresia and trisomy 21; (3) primary repair using modern surgical techniques whi

[Intestinal pneumatosis].

Fourteen cases of intestinal pneumotosis were found in a review of 1 477 plain abdomen X-ray films taken because of different conditions. These 14 cases were associated in most instances with septicemia, gastroenteritis and paralytic ileus. The mortality was high. The treatment should be directed to the management of the primary condition: intestinal pneumatosis, septicemia, gastroenteritis, paralytic ileus.

Intestinal anastomosis.

The long-term results of 370 cases of intestinal anastomosis are described. 121 cases were studied in detail. There were 61 duodenal, 43 small intestinal, and 17 colon anastomoses. The follow-up examinations permitted the following conclusions: 1. The long-term results of duodenoduodenostomy and duodenojejunostomy are similar. 2. Resection of a duodenal membrane was often followed by stenosis. 3. End-to-end anastomosis of small intestine and colon give good long-term results, but after side-to-side anastomoses, 24% of the cases will develop a closed loop syndrome.

Intestinal gas.

Patients with functional bowel disease commonly complain of abdominal pain, bloating, and excessive flatulence and eructation. Pain and bloating may be primarily caused by abnormal intestinal motility rather than by excessive intestinal gas. As yet there are no data available that prove excessive flatulence is actually caused by the presence of excessive intestinal gas. A study of the composition of intestinal gas provides insight into whether it is derived from swallowed air or from intraluminal metabolism. Therapy aims primarily at excluding the presence of organic disease as a cause and rea
