PURPOSE: Intraoperative lung protective ventilation and, in inhalational anesthesia, minimal fresh gas flow (FGF) are recommended. It remains unclear how these recommendations are transferred to clinical routine. We aimed to assess corresponding anesthesia practices in Germany and raise awareness of optimal ventilation settings.
METHODS: We performed a pragmatic prospective observational national cross-sectional assessment of intraoperative ventilation settings in Germany, followed by an educational intervention and follow-up assessment. The intervention consisted of a pocket card designed in

BACKGROUND AND OBJECTIVE: Pulmonary artery (PA) injury during anatomic lung resection remains a rare but potentially catastrophic complication. With the increasing adoption of robotic-assisted thoracoscopic surgery (RATS), the absence of tactile feedback and the growing use of neoadjuvant therapies introduce unique challenges. This review aims to summarize the epidemiology, risk factors, prevention strategies, and intraoperative management of vascular injuries in the era of minimally invasive thoracic surgery.
METHODS: A narrative review of the literature was performed, with inclusion of Engl

BACKGROUND: According to the National Clinical Database of Japan, the incidence of bile duct injury (BDI) during laparoscopic cholecystectomy has hovered around 0.4% for the last 10 years and has not declined. On the other hand, it has been found that about 60% of BDI occurrences are due to misidentifying anatomical landmarks. However, the authors developed an artificial intelligence (AI) system that gave intraoperative data to recognize the extrahepatic bile duct (EHBD), cystic duct (CD), inferior border of liver S4 (S4), and Rouviere sulcus (RS). The purpose of this research was to evaluate

OBJECTIVES: To evaluate iatrogenic urinary tract system injuries in obstetrics and gynecology operations and compare the results with the literature.
PATIENTS AND METHODS: We examined the records of patients who had obstetric and gynecology operations at the Ministry of Health, Dr. Zekai Tahir Burak Women's Health, Training and Research Hospital between June 2007 and June 2010. All the patients who were diagnosed as having urinary system injuries in either the intraoperative or postoperative period were determined.
RESULTS: During this period, 25,998 gynecologic and obstetrical operations we

It is now widely established that systematic intraoperative location and diligent dissection of the recurrent inferior laryngeal nerve during thyroidectomy are the keystones to assure its anatomic and functional preservation. The possibility of abnormal routes, like a non-recurrent cervical course of the inferior laryngeal nerve is an additional major argument for its systematic identification to avoid surgical damage. In 2517 cervicotomies performed between 1992 and 1997 for at least right thyroid lobe excision or parathyroid glands exploration, 20 cases of non recurrent laryngeal nerve were

Two neonates undergoing arterial switch procedure developed life-threatening pulmonary hypertension intraoperatively. In one patient, bradycardia, hypotension, and electrocardiographic (ECG) evidence of myocardial ischemia suddenly occurred 20 minutes after uneventful weaning from cardiopulmonary bypass. Lifting a palpably hypertensive main pulmonary artery (MPA) resulted in reproducible hemodynamic improvement. Because the patient was already on full ventilatory support and a nitroglycerin infusion, the MPA was suspended onto the anterior chest wall. In the other patient, after removal of int

BACKGROUND: Lymphorrhea is a rare condition with a paucity of reports in the gynaecologic literature. The most frequent causes are invasive procedures and surgical interventions.
CASE: A multiparous woman underwent a total abdominal hysterectomy with prophylactic bilateral salpingectomy and abdominal sacrocolpopexy for pelvic organ prolapse. During retroperitoneal dissection, clear fluid discharge was encountered. Ureteric injury was subsequently ruled out. A sample of the fluid was taken to confirm lymphatic injury. Ligation suture and closing the peritoneum slowed fluid drainage.

The possibility of urinary tract injury should always be considered in the course of anterior resection or combined abdominoperineal resection of the lower colon and rectum. Controlled studies of ureteral and other injuries and fistulas cannot be made; but fortunately, unanticipated damage to the lower urinary tract does not occur often. The surgeon operating in the pelvis should be aware of the problems rarely encountered and should be capable of their management. If a urologic surgeon is available, consultation often is desirable. Prompt intraoperative recognition is most important so remedi

Appendiceal duplication is an exceptionally rare anomaly, with an incidence ranging from 0.004% to 0.009%. Most cases are asymptomatic and remain undetected on imaging, with diagnosis usually established incidentally during laparotomy performed for other intra-abdominal conditions. The differential diagnosis includes cecal diverticulum, mesenteric adenitis, epiploic appendagitis, and neoplasms of the colon or appendix. We report the case of a 22-year-old man whose preoperative findings and intraoperative appearance suggested appendiceal duplication. Histopathological examination, however, demo

This narrative review examines the multifaceted realm of postoperative infections in gynaecology, addressing their significance, types, risk factors, prevention, management, and emerging trends. Postoperative infections, encompassing surgical site infections, urinary tract infections, and pelvic inflammatory disease, pose considerable challenges in patient care, warranting comprehensive exploration. Strategies for prevention include preoperative patient assessment, antimicrobial prophylaxis, and aseptic techniques. Intraoperative measures encompass infection control and instrument sterilizatio

Mazabraud's syndrome represents rare benign disorder characterized by simultaneous occurrence of fibrous dysplasia of bone and intramuscular myxomas within surrounding soft tissue. Mutations of GNAS1 gene were proven to be causative for this condition. Here, we present a case report of a patient with unusual manifestation of this disease, who developed a pathological fracture of the femur in the setting of monostotic fibrous dysplasia. The intramuscular myxoma of the thigh was discovered during the following orthopedic operation, where the intraoperative diagnosis became a pitfall of the case,

BACKGROUND: While variation within the anterior cerebrovascular circulation is common, an infraoptic course of the proximal anterior cerebral artery (ACA), or infraoptic A1, is a relatively rare cerebrovascular anomaly. Associations with suprasellar neoplasms may occur, and accurate identification of this aberrant vessel during dissection is crucial to preventing vascular injury or stroke.
CASE DESCRIPTION: We present the first reported case of surgically confirmed bilateral infraoptic A1 arteries associated with a craniopharyngioma. We review the relevant magnetic resonance imaging (MRI), an

BACKGROUND: Squamous papillary craniopharyngioma is a distinct entity, and its cytologic features may be misleading. Because of the rarity of this tumor, this case is being reported with a note on the cytologic features.
CASE: A 56-year-old Malay man who had 1-month history of generalized lethargy was admitted for altered sensorium. On examination, he was found to have neck stiffness, bilateral papilledema and generalized atrophy of muscles, with reduced power in all limbs. Magnetic resonance imaging of the brain showed a solid mass in the third ventricle causing obstructive hydrocephalus. In

Iatrogenic ureteric injury is an uncommon but serious complication of gynaecological surgery, with significant potential for renal impairment, fistula formation, prolonged hospitalisation, and medico-legal consequences. The ureter is at risk not only in complex pelvic procedures but also during apparently straightforward operations, and injuries may go unrecognised at the time of surgery, contributing to delayed diagnosis and worse clinical outcomes. We present a case series of five female patients aged 33, 59, 45, 46, and 47 years, respectively, who underwent complex gynaecological surgery fo

BACKGROUND: Intraoperative imaging is often used to aid pedicle screw placement during scoliosis operations. Higher rates of cancer and death have been observed in orthopaedic surgeons and radiation technologists, including a fourfold higher rate of breast cancer in female orthopaedic surgeons. The purpose of this study was to evaluate variability in intraoperative radiation during spinal fusions for both adolescent idiopathic scoliosis (AIS) and neuromuscular scoliosis (NMS).
METHODS: A retrospective review of posterior spinal fusion and segmental spinal instrumentation for scoliosis perform

Primary tumors arising from within the arterial wall are extremely uncommon and often present at advanced stages, making timely intervention challenging. Surgeons need to be aware of these rare conditions, as early detection and treatment can significantly improve patient outcomes. Angiosarcoma of the abdominal aorta is a particularly rare and aggressive malignancy, often resembling an aneurysm at risk of rupture. A 59-year-old woman presented with sudden onset of severe abdominal pain and was diagnosed with a 42 mm saccular abdominal aortic aneurysm, which was urgently repaired under the pres

INTRODUCTION: We report on a case of congenital unilateral atresia of the vas deferens encountered during a robotic-assisted transabdominal preperitoneal (TAPP) inguinal hernia repair.
CASE REPORT: Our 65-years-old male patient was scheduled for a bilateral robotic-assisted TAPP inguinal hernia repair because of bilateral symptomatic groin hernia. Standard intraoperative dissection obtaining a critical view of the myopectineal orifice did not allow for an identification of the vas deferens (VD) on the left side. On the right side, a normal VD was identified. There was no suspicion of an intra

BACKGROUND: Echocardiography can detect aortic regurgitation (AR) that may interfere with the adequate delivery of cardioplegia solution to the myocardium during cardiac operation. When aware of this lesion, the surgeon can modify the operative technique accordingly. We sought to evaluate the ability of intraoperative transesophageal echocardiography to detect AR and to correlate the severity of the lesion with the need for retrograde cardioplegia administration.
METHODS: Eighty-four consecutive patients undergoing coronary artery bypass grafting were evaluated. When AR was noted by intraoper

Splenogonadal fusion (SGF) is a rare congenital anomaly that can mimic testicular malignancy, potentially leading to unnecessary orchiectomy. We report a 3-year-old boy with bilateral cryptorchidism who underwent bilateral orchiopexy via inguinoscrotal approach. Intraoperatively, a reddish, firm extra-testicular mass was identified at the upper pole of the left testis. Due to its uncertain nature, the mass was not manipulated to avoid orchiectomy, and the testis was descended uneventfully. Postoperative Doppler ultrasound showed a hypervascular, hypoechoic heterogeneous mass suggestive of SGF.
