Local anaesthetic systemic toxicity complicating intraoperative intercostal nerve blocks: What do clinicians need to know to prevent similar occurrence?

Local anaesthetic systemic toxicity is a life-threatening adverse event that may occur after administration of local anaesthetics through a variety of routes. Local anaesthetic systemic toxicity is always a potential complication and may occur with all local anaesthetics and in any route of administration. Local anaesthetic systemic toxicity primarily affects the central nervous system and the cardiovascular system and may be fatal. The following is a case of local anaesthetic systemic toxicity complicating surgeon-performed intraoperative intercostal nerve blocks at multiple levels, with a mi

Utility of intraoperative neuromonitoring and outcomes of neurological complication in lower cervical and upper thoracic posterior-based three-column osteotomies for cervical deformity.

OBJECTIVE: For severe and rigid adult cervical deformity, posterior-based three-column osteotomies (3COs) are warranted, but neurological complications are relatively high with such procedures. The performance measures of intraoperative neuromonitoring (IONM) during cervicothoracic 3CO have yet to be studied, and there remains a paucity of literature regarding the topic. Therefore, the authors of this study examined the performance of IONM in predicting new neurological weakness following lower cervical and upper thoracic 3CO. In addition, they report the 6-month, 1-year, and 2-year outcomes o

Safety and Efficacy of Real-Time Intraoperative Ultrasound-Guided Posterolateral Thoracic Diskectomy: Using Technology to Minimize Complication and Improve Surgical Outcome.

BACKGROUND AND OBJECTIVES: Surgical Treatment of thoracic disk herniation (TDH) can be challenging due to the thoracic spine's relatively narrow canal with spinal cord blocking 0direct access and visualization of the TDH. This study evaluates the efficacy and safety of real-time intraoperative ultrasound (IOUS)-guided posterolateral diskectomy for TDH. METHODS: A retrospective analysis was conducted on patients with symptomatic TDH who underwent IOUS-guided surgery between May 2020 and February 2025 treated by a single surgeon at a major academic center. Patient demographics and perioperative

Severe intraoperative vascular bleeding as main complication of acetabular fractures treated with plate osteosynthesis via the modified Stoppa approach-Letter to editor.

We read with great interest the article "Severe intraoperative vascular bleeding as the main complication of acetabular fractures treated with plate osteosynthesis via the modified Stoppa approach" by Julia Riemenschneider et al [1]. We appreciate the authors' efforts in describing intraoperative vascular bleeding in reduction acetabular fractures using the modified Stoppa approach. However, we had several concerns about the study results and believe that the authors' responses may help to address them.

Predictive value of deformity angular ratio (DAR) for intraoperative neuromonitoring (IONM) signal loss and neurological complications in spinal deformity surgery: a systematic review and meta-analysis.

PURPOSE: Spinal deformity surgery carries a high risk of intraoperative neuromonitoring (IONM) signal loss and neurological complications. The Cobb angle has traditionally been used to assess spinal deformity severity, but the Deformity Angular Ratio (DAR) has been proposed as a potentially more precise predictor of surgical risks. This systematic review and meta-analysis aimed to compare the preoperative DAR to the Cobb angle to predict the loss of IONM signal and the presence of postoperative neurological complications in spinal deformity correction. METHODS: A systematic literature search

Intraoperative Blood Pressure Lability Acts as a Key Mediator in the Impacts of Goal-Directed Fluid Therapy on Postoperative Complications in Patients Undergoing Major Spine Surgery.

Objective Although goal-directed fluid therapy (GDFT) has been proven to be effective in reducing the incidence of postoperative complications, the underlying mechanisms remain unknown. The aim of this study was to examine the mediating role of intraoperative hemodynamic lability in the association between GDFT and the incidence of postoperative complications. We further tested the role of this mediation effect using mean arterial pressure, a hemodynamic indicator. Methods This secondary analysis used the dataset of a completed nonrandomized controlled study to investigate the effect of GDFT o

Complication rates after intraoperative tranexamic acid use in total joint arthroplasty patients with a history of solid organ transplantation: A retrospective cohort study.

BACKGROUND: Solid organ transplant (SOT) recipients undergoing total joint arthroplasty (TJA) may face elevated perioperative risk because of chronic immunosuppression, medical comorbidity, and organ-specific physiologic considerations. Although intravenous tranexamic acid (TXA) is widely used during TJA to reduce perioperative blood loss, evidence regarding outcomes among SOT recipients receiving TXA remains limited. METHODS: A retrospective review was performed of 29,240 consecutive primary TJA patients who all received intraoperative intravenous TXA between June 2011 and March 2025. Patien

Limited effect of sugammadex on postoperative pulmonary complications in patients undergoing spine surgery in prone position intraoperatively: A retrospective analysis of matched cohort data.

Postoperative pulmonary complications (PPCs) increase postoperative mortality, hospital stays, and healthcare costs. Whether the use of sugammadex could reduce PPCs remains controversial. This study aimed to determine if sugammadex could more effectively reduce PPCs than acetylcholinesterase inhibitor (AChEi) in patients who had undergone spine surgery, in prone position intraoperatively. From March 2019 to February 2021, adult patients who underwent elective spine surgery were eligible. Primary outcomes were PPCs (including atelectasis on chest radiograph, pneumonia, acute respiratory distres

Does Hypotension Prediction Index limit the occurrence of postoperative complications associated with intraoperative hypotension? A systematic review and model-averaged Bayesian meta-analysis of statistically sound studies.

Intraoperative hypotension (IOH) is considered a potential contributing factor to postoperative complications. In 2018, a machine-learning algorithm to predict hypotension has been included in the Hemosphere™ platform (Edwards Lifescience, USA), of which the Hypotension Prediction Index (HPI) is the marker. We conducted a systematic review and Bayesian model-averaged meta-analysis to evaluate whether the limitation of IOH had a limiting effect on postoperative cardiac, pulmonary and renal complications and mortality. Nine randomized controlled trials and one propensity score analysis were incl
