BACKGROUND: Psychiatric side effects are common during interferon-alfa (IFN-alfa) therapy and often responsible for early treatment discontinuation, thus limiting its therapeutic potential. Depression is considered the hallmark of these side effects. However, irritability, anger/hostility, and manic/hypomanic episodes have also been reported, suggesting that these symptoms are important features of IFN-alfa-induced neuropsychiatric side effects.
OBJECTIVE: The aim of this prospective study was to use item-by-item analysis to thoroughly characterize neuropsychiatric symptoms occurring during e

Functional somatic syndromes are defined as several related syndromes that are characterized more by symptoms, suffering, and disability than by structural or functional abnormality. These syndromes include irritable bowel syndrome, tension-type headache, chronic fatigue syndrome, and fibromyalgia. Such syndromes have similarities in terms of definition, diagnosis, etiology, and treatment. To elucidate the pathogenesis of functional somatic syndromes, it is important to focus on gender-related factors, comorbidities of depression and anxiety, physiological responses like autonomic nervous func

The authors assessed the prevalence of Diagnostic Criteria for Psychosomatic Research (DCPR) clusters in a community sample and the association between these syndromes and psychosocial variables. A group of 347 consecutive subjects from the general population were administered the semistructured interview for DCPR and a self-rating scale (the Psychosocial Index). A DCPR syndrome was identified in 59% of subjects. These subjects showed more stress and distress and less well-being than those without a DCPR syndrome. Some DCPR syndromes (alexithymia, Type A behavior, irritable mood) are frequentl…

Irritable bowel syndrome (IBS), a functional gastrointestinal disorder, is present in 10% to 20% of the U.S. adult population. The syndrome is best defined as chronic abdominal discomfort with changes in stool frequency, consistency, and passage, with associated symptoms such as abdominal bloating or presence of mucus in stools. Several studies have shown that up to 70% to 90% of patients with IBS who seek treatment have psychiatric comorbidity, most notably mood and anxiety disorders. Recent studies have shown a high prevalence of IBS in psychiatric patients who seek treatment, with a prevale

BACKGROUND: To test the factor structure of oppositional defiant disorder (ODD) symptoms and to study the relationships between the proposed dimensions and external variables in a community sample of preschool children.
METHOD: A sample of 1,341 3-year-old preschoolers was randomly selected and screened for a double-phase design. In total, 622 families were assessed with a diagnostic semi-structured interview and questionnaires on psychopathology, temperament and executive functioning completed by parents and teachers.
RESULTS: Using categorical and dimensional symptoms of ODD it was possibl

BACKGROUND: Chronic pain (CP) as a long-lasting stressor can often cause mood disorders, such as depression or anxiety. The comorbidity of CP and mood disorders poses challenges for treatment and increases healthcare costs. Acupuncture has emerged as a widely utilized approach to alleviate both CP and mood disorders. However, there is a lack of well-established bibliometric analyses in this area of research. Therefore, this study aimed to identify the current hotspots and research trends regarding the use of acupuncture for treating CP-related depression or anxiety.

Prominent apathy and/or irritability are frequently observed among individuals infected with the human immunodeficiency virus (HIV). Although these symptoms often occur as part of a mood disorder, compelling evidence suggests that they may occur independently of depression in neurologic disease/disorder. The current study examined the prevalence of both apathy and irritability among a sample of HIV-infected individuals and explored the degree to which these neuropsychiatric (NP) phenomena were associated with performance on neuro-cognitive measures thought to be sensitive to the potential CNS

INTRODUCTION: To aid in the diagnosis and management of premenstrual syndromes, dozens of symptom measurement instruments have been created and several methods for classifying clinically important change in symptoms have been defined. While the diagnosis of premenstrual dysphoric disorder (PMDD) has become standardized through the application of research criteria, consensus amongst investigators as to the instruments best able to confirm the diagnosis and measure treatment effects has yet to be reached.
OBJECTIVE: To determine the performance and inter-correlations of three prospective sympto

Although for years it was felt to be almost nonexistent, very-early-onset bipolar disorder (before age 12) has become an important member of the mood disorder spectrum of youths. Whether there is a separate subtype characterized by severe irritability or whether the problem of emotionally explosive children is better explained by other conditions is still being resolved. The diagnosis of bipolar I disorder requires an episode of mania, and understanding those manic symptoms is the source of differences in interpretation.

OBJECTIVE: According to DSM-IV, criterion (A) for diagnosing a hypomanic/manic episode is mood change (ie, elevated, expansive, or irritable mood). Criterion (A) was redefined in DSM-5, adding increased energy or activity in addition to mood change. We sought to investigate the effect of adding increased energy or activity to criterion (A) for the diagnosis of hypomania/mania and, thus, bipolar disorder.
METHODS: This analysis of prospectively collected data from the Bipolar Collaborative Network (1995-2002) includes 907 DSM-IV-TR-diagnosed bipolar outpatients (14,306 visits). The Young Mania…

BACKGROUND: CDKL5 deficiency disorder (CDD) is an early-onset developmental and epileptic encephalopathy. While a subset of individuals is believed to experience comorbid behavioral disorders, none have reported well-defined affective disorders. Though there is a documented association between epilepsy and mood disorders, they may go undetected in the CDD population due to difficulty assessing mood in the presence of severe/profound intellectual disability and disease-related sleep dysregulation. We aimed to identify the clinical characteristics of an individual with CDD who presented with a m

BACKGROUND: Neurofeedback and neuromodulation treatments are of increasing clinical interest, but their neurocognitive targets are poorly understood.
METHODS: In this review, we will use Jaspers' phenomenological psychopathology combined with modern network analysis to identify neurocognitive treatment targets by focussing on distinctive and necessary symptoms of mood disorders as well as their subsyndromal and prognostic variations.
RESULTS: We discuss the early descriptions of Kraepelin's mixed affective states and suggest a model of four mood states (depressed, anxious, irritable, and ela

BACKGROUND: The concept of bipolar spectrum disorders has opened therapeutic opportunities for patients with atypical and complex affective conditions. The literature has recently described several commonalities in pathophysiological processes of bipolar disorders and dementia. However, this connection has been insufficiently appreciated at the clinical level, in part because affective dysregulation in the elderly and, particularly in the dementia setting, is typically attributed either to secondary depressive states or otherwise relegated to a neurologically understandable behavioral complica

Most women of reproductive age have some physical discomfort or dysphoria in the weeks before menstruation. Symptoms are often mild, but can be severe enough to substantially affect daily activities. About 5-8% of women thus suffer from severe premenstrual syndrome (PMS); most of these women also meet criteria for premenstrual dysphoric disorder (PMDD). Mood and behavioural symptoms, including irritability, tension, depressed mood, tearfulness, and mood swings, are the most distressing, but somatic complaints, such as breast tenderness and bloating, can also be problematic. We outline theories

BACKGROUND: Menopausal symptoms are common during the menopausal transition and can substantially affect women's physical and psychological health. These symptoms often co-occur and interact, but their complex interrelationships and the symptoms that play central roles within the symptom network remain unclear.
OBJECTIVE: Explore the network structure of menopausal syndrome and identify core symptoms.
METHODS: A cross-sectional study of 402 perimenopausal women aged 40-60 years in Shanghai, China, November 2023 to April 2024. The Greene scale was used for evaluation. Perform a network analys

Irritability and executive function, which continue to develop from late childhood to early adolescence, predict important functional impairments. Although correlated, longitudinal research has often focused only on how executive function affects irritability, overlooking the reverse pathway. This study proposes that irritability may also contribute to later executive function difficulties and investigates their bidirectional associations across three components, including inhibitory control, working memory, and cognitive flexibility, as well as whether these associations vary by sex. Irritability in children and adolescents is an important mental health concern that presents across a range of disorders. Family accommodation refers to the way families alter their behavior to prevent or reduce child distress. Despite its clinical relevance, the role of family accommodation in pediatric irritability remains underexplored. This study developed and validated a novel parent- and child-report form of the Family Accommodation Scale for Irritability (FASI). We recruited 131 youths (61.83% male, 36.64% female, 1.53% unknown sex; 63.36% White, 12.21% multiracial, 9.92% Black or Afric
