CASE REPORT: A 73-year-old woman presented with bilateral generalized posterior keratoconus. The corneal thickness measured 0.320 mm paracentrally and 0.440 mm toward the periphery. The corneal specimen revealed absence of Bowman's layer and focal homogeneous oval stromal deposits that stained for amyloid.

PURPOSE: To report a keratoconus case with bilateral horizontal Vogt's striae.
METHOD: The clinical findings of the patient and the development of the direction of striae are discussed.
RESULTS: Vogt's striae, defined as vertical stress lines, are rarely horizontal. One patient with unilateral horizontal stress lines on his left eye has been reported in the literature. Our patient has horizontal Vogt's striae in both eyes.
CONCLUSION: Horizontal Vogt's striae may be seen in keratoconus as a rare slit-lamp biomicroscopic finding.

OBJECTIVE: To report the concomitant presentation of keratoconus and macular corneal dystrophy in two cases.

In a histochemical and electron microscopic study of 45 cases of keratoconus, oxytalan fibres, a member of the elastic fibre family, were found in the extracellular tissues of the cornea. Oxytalan fibres were identified beneath the epithelium over the thinned central region of the conical cornea as well as in areas of stromal scarring. In a number of instances the apparently thickened Descemet's membrane was shown to include an oxytalan-containing retro-corneal layer. Oxytalan fibres were found to be immunostained by antibodies raised against collagen type VI.

In the past, keratoconus patients have been custom fit with multi-curve spherical contact lenses.
