OBJECTIVE: The purpose of this article is to describe findings on MRI in the evaluation of knee injury in pediatric patients.
CONCLUSION: Injury patterns in the pediatric knee overlap and differ from adults. Differences include open physes, changing mechanics, and differences in ligamentous support. Awareness of normal variants, common incidental findings, and normal evolution of bone marrow aid in the interpretation.

Spontaneous osteonecrosis of the knee presents with acute onset of severe, pain in elderly patients, usually female and usually without a history of trauma. Originally described as idiopathic osteonecrosis, the exact etiology is still debated. Evidence suggests that an acute fracture occurs as a result of chronic stress or minor trauma to a weakened subchondral bone plate. The imaging characteristics on MR reflect the age of the lesion and the symptoms. More appropriate terminology may be " subchondral insufficiency fracture of the knee" or "focal subchondral osteonecrosis".

Mastocytosis is a rare pathology with possible bone involvement. This report describes an unusual case of solitary osteolytic lesion of the proximal right tibia, revealing an osseous mastocytosis, treated by bone cement injection. Follow-up showed a good clinical and radiological outcome, and the patient remains symptom-free 9 months after her surgery. To our knowledge, this is the first case reported in the literature.

Lipoma arborescens (diffuse articular lipomatosis) is a rare, benign intra-articular lesion of unknown aetiology. It is characterised by villous proliferation of the synovium and diffuse replacement of the subsynovial tissue by mature fat cells. It forms part of the differential diagnosis for a slowly progressive chronically swollen knee. We present a very rare case of bilateral involvement of the knee and discuss the symptoms, diagnosis and treatment of this condition.

In spite of considerable research efforts, knee injuries continue to plague athletes. The rate of occurrence may actually be increasing. Particularly vulnerable to injury because of its anatomic location, the knee sustains a tremendous amount of force during sports. Physicians caring for these patients must be familiar with the mechanism of these injuries. This report aims to familiarize rheumatology physicians with an overview of the anatomy, function, physical examination, and treatment of the various anatomic structures that are commonly injured.
