Predictors of outcome of floating knee injuries in adults: 89 patients followed for 2-12 years.

Of the 98 floating knee injuries that were consecutively treated from 1987 to 1997, 89 patients were available for analysis. There were 80 males and 9 females, ranging from 15 to 70 years old. Average follow-up was 5 (2-12) years. Injury severity scores ranged from 18 to 45. 21 fractures were intra-articular. 55 fractures were open. Substantial comminuted and segmental fractures occurred in 57 cases and 35 cases, respectively. Multivariate analysis showed that increasing age was associated with delays in bony union and full weight bearing ability. An increase in the number of pack years smoked…

Change in diagnosis among orthopedists compared to non-orthopedists in the management of acute knee injuries.

OBJECTIVE: Uncertainty regarding diagnosis is associated with lower patient satisfaction and can lead to delays in definitive treatment and to inappropriate use of resources. We sought to compare change in diagnosis among orthopedists and non-orthopedists caring for a community based cohort of individuals with incident acute knee injuries. METHODS: We conducted a longitudinal investigation of a population based cohort of Olmsted County residents with their first episode of acute knee injury occurring between January 1, 1993, and December 31, 1995. We reviewed the entire (inpatient and outpati…

Function, post-traumatic sequelae and current concepts of management of knee meniscus injuries: a review article.

Long thought to be expendable or insignificant for function, the menisci may well be very important structures in the knee joint. Menisci are of critical importance from the standpoint of stability, shock absorption, load transmission, and lubrication, and also play a role in preventing synovial impingement, and in defining the endpoints of flexion and extension. In the wake of meniscectomy, certain predictable time-related events afflict the joint, i.e., instability and degenerative arthrosis. These eventualities are less pronounced following partial resection, and lend credence and support t

Arthroscopy in acute traumatic hemarthrosis of the knee. Incidence of anterior cruciate tears and other injuries.

In a prospective study, all injured knees that had traumatic hemarthrosis and absent or negligible instability on clinical examination underwent arthroscopy and examination under anesthesia. Eighty-five knees (eighty-three patients) were examined over a 125-week period. Some degree of disruption of the anterior cruciate ligament was found in sixty-one (72 per cent) of the knees (a partial tear in 28 per cent and a complete tear in 44 per cent), frequently associated with an injury of varying severity to other joint structures. These included minor ligament sprains without laxity in 41 per cent

A clinical decision-making algorithm for posterolateral corner injuries of the knee: Development and internal validation.

PURPOSE: Posterolateral corner (PLC) injuries of the knee remain a complex and heterogeneous clinical entity, with substantial variability in surgical management and no widely accepted criteria to guide treatment selection. The purpose of this study was to develop and internally validate a clinically applicable decision-making algorithm for PLC injuries and to evaluate the outcomes of a minimally invasive reconstruction strategy in a consecutive cohort of patients. METHODS: A retrospective cohort of 120 patients undergoing surgical treatment for PLC injuries, including both isolated PLC recon

What's New: Sub-5-minute Knee Magnetic Resonance Imaging- Spectrum of Sports Injuries and Overuse Conditions.

Knee injuries are one of the most common complaints in sports medicine. Magnetic resonance imaging is an essential adjunct to clinical evaluation for many traumatic injuries and overuse conditions. Given the heavy use of knee magnetic resonance imaging, developing faster magnetic resonance imaging acquisition methods and deployment in clinical practice would be valuable. In this article, we illustrate a spectrum of knee abnormalities from our clinical practice, utilizing a recently developed, publicly available sub-5-minute knee magnetic resonance imaging protocol with super-resolution image r…

Editorial Commentary: Knee Hyperextension Injuries: What Are We Really Diagnosing and How to Solve the Problem?

Knee hyperextension injuries remain a poorly defined entity, often functioning more as a descriptive label than a true mechanistic diagnosis. Rather than representing isolated sagittal-plane events, these injuries reflect the final position of complex multiplanar loading patterns, resulting in highly variable structural damage that frequently extends beyond the cruciate ligaments. Emerging clinical observations suggest that patients with subtle hyperextension mechanisms may present with persistent pain and instability despite largely normal imaging and standard ligament testing. This highlight

Risk factors associated with dairy cattle lameness and hock and knee injuries on Iranian dairy farms.

Both lameness and hock and knee injuries represent significant welfare and economic concerns on dairy farms. However, there is a notable lack of data from certain housing systems, including bedded-pack farms and a few studies that compare free-stalls with bedded-pack farms. The objective of this study was therefore to utilise data collected during a welfare assessment to identify key risk factors for lameness and leg injuries in these housing systems. A generalised linear mixed model was used to identify risk factors associated with hock and knee injuries, whereas for lameness a binomial regre…

Investigating the incidence of concomitant lower-extremity and pelvic fractures in patients with multiligament knee injuries.

BACKGROUND: Multiligament knee injuries (MLKIs) often result from high-energy trauma in polytrauma patients. They may coincide with other musculoskeletal injuries, especially fractures of the ipsilateral lower extremity (LE) or pelvis. Understanding these fracture patterns can guide surgical planning and improve patient outcomes. The study aim is to describe the ligamentous injury patterns of combined ipsilateral LE or pelvic fractures with surgically treated MLKIs. METHODS: A retrospective cohort study was conducted from April 2008 to August 2024. Patients who sustained tibial plateau (TP), …

Reconstructive surgery for gunshot injuries of the knee: experience from the Russo-Ukranian War 2022-2024.

The invasion of Ukraine by the Russian Federation in February 2022 has witnessed the first peer-on-peer full-scale European conflict since World War 2. We have noted that the vast majority of injuries affect the extremities. Within that group, injuries to the joints pose a huge clinical challenge. Specifically, 17.1% of all gunshot injuries (GSWs) involved the joints, which represents 22.3% of all limb injuries. 55.6% of all GSW to the joints involved the knee; 10% had a concomitant vascular injury and 15%-20% had a nerve injury. The surgical management of ballistic knee injuries includes initi…

Causal Relationships of Ligamentous Injuries in the Knee on Corticospinal Tract Structure: A Mendelian Randomization Analysis.

BACKGROUND: The association between ligamentous knee injuries and corticospinal tract (CST) structure has attracted attention; however, any causal relationship remains uncertain. We performed Mendelian randomization (MR) analysis to identify the causal effects of ligamentous knee injuries on the CST. HYPOTHESIS: Ligamentous knee injuries impair CST microstructure (ie, by reducing fractional anisotropy [FA] and increasing mean diffusivity [MD]). STUDY DESIGN: MR analysis. LEVEL OF EVIDENCE: Level 2. METHODS: MR uses genetic variants as instrumental variables to infer causal relationships be…

Performance of PROMIS Computer Adaptive Testing As Compared With Established Instruments for Multiple-Ligament Knee Injuries.

BACKGROUND: The Patient-Reported Outcomes Measurement Information System (PROMIS) computer adaptive testing (CAT) is a patient-reported outcome metric that has been validated for anterior cruciate ligament (ACL) injuries, ACL reconstructions, and meniscal injuries. Thus far, the system has not been validated for multiligament knee injuries. The Multiligament Quality of Life (MLQOL) questionnaire is a validated, disease-specific patient-reported outcome instrument for this population that can serve as a gold standard for validation of the newer PROMIS. PURPOSE/HYPOTHESIS: The purpose of this s…

Bilateral disruption of soft tissue extensor mechanism of knee: functional outcome and comparison to unilateral injuries.

BACKGROUND: Previous studies suggest that Dupuytren's disease is caused by fibroblast and myofibroblast contractility. Cell contractility in smooth muscle cells is caused by calcium-dependent and calcium-independent signaling mechanisms. In the calcium-dependent pathway, calcium/calmodulin activates myosin light chain kinase (MLCK). In this study, the effects of calcium/calmodulin inhibition with the FDA-approved drug fluphenazine on Dupuytren's fibroblast contractility and MLCK expression were tested. METHODS: Fibroblast lines from the palmar fascia of patients with Dupuytren's disease were

Knee injuries related to sports in young adult males during military service - incidence and risk factors.

The population-based incidence and risk factors for knee injuries in young adults were assessed in Finnish male conscripts performing their compulsory military service (n = 128,584). The main outcome variables were (1) hospitalization due to knee injuries overall and (2) hospitalization due to knee disorders as categorized into specific International Classification of Diseases, tenth revision diagnoses (cruciate and collateral ligament tears, meniscal tears, traumatic chondral lesions, and patellar dislocations). Person-time injury-incidence rates were calculated by dividing the number of pers…
