Klippel-trenaunay syndrome in a child with coexisting lymphangioma, vascular insufficiency, and multiple soft tissue swellings: a case report.

INTRODUCTION: Klippel-Trenaunay syndrome (KTS) is a complex and extremely rare congenital vascular syndrome. The disorder presents with a vascular malformation syndrome involving cutaneous capillaries and venous (hemangiomas and port-wine stains). Lymphatic anomalous development, with hyperplasia of soft tissue and bones, can also occur, which is due to overgrowth occurring as a result of somatic mutations. CASE PRESENTATION: We present a 12-year-old male child with a 12-year history of subcutaneous growths, initially painless. KTS was diagnosed in childhood and associated with a vascular mal

Postoperative limb lengthening following Total Hip Arthroplasty (THA) through a posterior approach--a challenge for the orthopaedist or physiotherapist?

BACKGROUND: As coxarthrosis progresses, pain and mobility limitation exacerbate, usually presenting as a progressive flexion-abduction contracture. This, combined with the degradation of articular ends of bones, is responsible for anatomical and functional shortening of the limb. The sensation of postoperative leg length difference is a significant problem both for the patient and the operator. A sense of the operated limb being longer may be due to excessive length of the femoral segment following total hip arthroplasty. It may also result from a postoperative abduction or flexion-abduction c

Percutaneous nailing in the management of osteogenesis imperfecta.

Marked deformities of the skeleton may develop in the severe form of osteogenesis imperfecta. Internal fixation can be effective in the treatment and prevention of fractures and deformities. Eight years ago, axial correction of deformities of the femora and tibiae by manual osteoclasis was carried out in a 7 year old girl in whom osteogenesis imperfecta had produced severe bowing of the long bones of the legs. Internal metal splints were introduced percutaneously to lie partially in, and partially alongside, the bone, to obtain fixation in the correct position. This treatment resulted in the p

Quadriceps and Hamstrings Activation Peaks Earlier as Athletes Repeatedly Hop, but There are Differences Depending on ACL Reconstruction Technique.

BACKGROUND: After Anterior Cruciate Ligament Reconstruction (ACLR) athletes face the challenge of regaining their previous competitive level while avoiding re-injury and early knee joint cartilage degeneration. Quadriceps and hamstrings strength reductions and neuromuscular alterations potentially related to risk of re-injury are present after ACLR and relate to deficits in muscle activation. DESIGN: Cross-sectional laboratory study. PURPOSE: To examine quadriceps and hamstrings muscle activation during repeated hops in healthy pivoting-sport athletes and those who had undergone ACLR (bone-t

Tuberculosis of the Femur With Intramedullary Abscesses and No Pulmonary Foci in an Immunocompetent Male: A Rare Case.

Tuberculosis of the long bones/femur, especially in an immunocompetent person, is a challenging diagnosis. It is a rare entity, even in endemic settings. The non-specific clinical features, backed by a low suspicion about such presentations even in endemic settings, may result in delayed diagnosis and often unfavorable treatment outcomes. The situation becomes even more challenging in the absence of pulmonary foci and a contact history of tuberculosis. Here is a case of a young adult male who presented with complaints of pain over his left leg for three months. A diagnosis was achieved with ma

A Rare Combination of Ipsilateral Partial Talocalcaneal and Talonavicular Coalition.

Tarsal coalitions refer to fibrous, cartilaginous or osseous fusion between two tarsal bones. Commonly seen are talocalcaneal coalitions and calcaneonavicular coalitions. Talonavicular, calcaneocuboid and cubonavicular coalition are very uncommonly seen. Talocalcaneal and calcaneonavicular coalitions are generally symptomatic whereas talonavicular coalitions are asymptomatic. Special view radiography, CT and MRI will be helpful in diagnosing coalitions depending on nature of coalitions. In this case report, we present 24-year-old male patient with rare combination of talocalcaneal and talonavi

Reconstruction of metatarsal bone defects with a free fibular osteomyocutaneous flap incorporating soleus muscle.

Severe traumatic bone and soft-tissue defects are often treated by lower leg amputation. The amputation level becomes a very important factor with respect to the patient's basic daily activities. We report the case of a 51-year-old man who was referred to us with severe traumatic metatarsal bone and dorsum pedis skin and soft-tissue defects. To avoid amputation, a free fibular osteomyocutaneous flap incorporating the soleus muscle was used to reconstruct the second and third metatarsal bones and the soft-tissue defect, respectively. Now, 2 years after the procedure, the patient is able to walk

[Küntscher intramedullary nailing of the tibial shaft fracture. A 19-year evaluation].

Marrow nailing developed by Gerhard Küntscher for long tubular bones is an excellent surgical procedure for fractures of the lower leg shaft. It yields outstanding results both with the typical good indications in respect of shape of fracture, localisation and soft tissue conditions and even with atypical borderline and emergency indications where the healing results can be said to be good and satisfactory. Of course long-standing experience is required especially when employing the Küntscher intramedullary nail in the atypical range of indications. This method produces good results even in cr

Quadriceps strength in women with a previous hip fracture: relationships to physical ability and bone mass.

Associations between physical ability, level of current physical activity and bone mass were examined in 47 elderly women (mean age 80 years) who had suffered from a hip fracture 3-36 months (mean 17 months) previously. Measures of physical ability included isokinetic quadriceps strength of both the non-fractured and fractured leg, and walking and stair climbing speed. An estimate of current physical activity was made using the Northwick Park activity index questionnaire specifically designed for hip fracture patients. Bone mineral density of the spine and hip (Ward's triangle, femoral neck an

Late changes in bone mineral density of the proximal tibia following total or partial medial meniscectomy. A randomized study.

The adaptive bone remodeling in the proximal tibia following medial meniscectomy was measured quantitatively by dual photon absorptiometry. Thirty-three patients who had undergone a meniscectomy (randomized to either total [n=19] or partial [n=14] meniscectomy) performed by open joint surgery approximately 12 years earlier were included in the study. Bone mineral density was measured in the previously injured legs and in the healthy contralateral legs in areas located medially and laterally in the cortical bone of the subchondral plates and below in the trabecular bone of the medial and latera

Transforaminal Endoscopic Lumbar Decompression for Isthmic Spondylolisthesis: Technique Description and Clinical Outcome.

OBJECTIVES: To describe a transforaminal endoscopic spinal decompression technique for treating adult patients with isthmic spondylolisthesis and report preliminary surgical and radiological results. BACKGROUND: Spondylolisthesis is prevalent in the general population. Surgical approaches for symptomatic spondylolisthesis that is refractory to conservative treatment vary. Direct repair of pars fractures and spinal nerve decompression with or without fusion have been reported with varied clinical results. The de facto gold standard, "fusion," is often associated with high complication rates an

Reduced bone formation markers, and altered trabecular and cortical bone mineral densities of non-paretic femurs observed in rats with ischemic stroke: A randomized controlled pilot study.

BACKGROUND: Immobility and neural damage likely contribute to accelerated bone loss after stroke, and subsequent heightened fracture risk in humans. OBJECTIVE: To investigate the skeletal effect of middle cerebral artery occlusion (MCAo) stroke in rats and examine its utility as a model of human post-stroke bone loss. METHODS: Twenty 15-week old spontaneously hypertensive male rats were randomized to MCAo or sham surgery controls. Primary outcome: group differences in trabecular bone volume fraction (BV/TV) measured by Micro-CT (10.5 micron istropic voxel size) at the ultra-distal femur of s

A randomized controlled trial comparing bone mineral density changes of three different ACL reconstruction techniques.

INTRODUCTION: This study aimed to compare the changes in bone mineral density (BMD) of three different ACL reconstruction (ACLR) techniques and its association with early clinical and functional outcomes. METHODS: Sixty-two male adult patients undergoing primary ACLR were prospectively parallel randomized to bone-patellar tendon-bone graft (BPTB), single-bundle (HT-SB) or double-bundle (HT-DB) hamstring graft. BMD (primary outcome) at the proximal tibia, distal femur, femoral neck and trochanteric region was measured blindly at day 1, 3 months, 5 months and 1 year after surgery. KT-1000, Lysh

Acute and long-term effects of gastrin on muscle and bone calcium uptake in rats.

It has previously been proposed that the hypocalcemic action of gastrin was mediated by a gastric peptide gastrocalcin which enhanced bone uptake of calcium (Ca). The present study, thus, aimed to reconfirm the effect of gastrin on muscle and bone Ca uptake and to evaluate a possible direct effect of gastrin on tissue Ca uptake. First, we showed that human and rat gastrin (100 microgram /100 g body weight (bw), s.c.) acutely increased uptake of intragastrically administered 45 Ca by the tibia and femur. Interestingly, human gastrin was more effective and also enhanced 45 Ca uptake by the gastr

Analysis of radiographic characteristics of anterolateral bowing of the leg before fracture in neurofibromatosis type 1.

BACKGROUND: Anterolateral leg bowing is associated with neurofibromatosis type 1 (NF1) frequently leading to fracture and nonunion of the tibia. The objective of the study was to characterize the radiographic findings of tibial dysplasia in NF1. METHODS: This study is a retrospective review of radiographs of tibial dysplasia obtained within 52 years, between 1950 and 2002, from the Shriners Hospitals for Children, Salt Lake City, and of peripheral quantitative computed tomographic images of 3 individuals with anterolateral bowing of the leg without fracture compared with age- and sex-matched

A morphological study of skeletal development in turkeys during the pre-hatching stage.

Skeletal chondrofication, ossification and growth of turkey embryos were investigated and analysed to enable assessment of the developmental status and evaluation of the experimental effects on skeletal development, skeletal mutations and development of cultured embryos. Ten embryos were prepared every 24 h from 8 to 28 days of incubation. The fixed embryos were cleared and stained in toto with Alcian blue & Alizarin red for cartilage and ossified components, respectively. Observation of the skeleton was performed under a stereoscopic microscopy, with special attention to the timing of chondro

Effects of mechanical loading on the tissues of the rabbit knee.

We studied changes in subchondral bone and articular cartilage in an animal model of osteoarthrosis. In this model we applied repetitive impulsive loads to rabbits' knees. Their legs were held in short leg splints so the rabbits were unable to dampen the peak applied load with ankle flexion. After sacrifice, at 1 day to 6 weeks, we studied proximal tibial load-bearing cartilage histologically, biochemically, and with radioactive sulfate uptake. We also studied the subchondral bone under that cartilage histologically, histomorphometrically, with bone scan (99mTc pyrophosphate), and by tetracycl
