We report a case of 33 year old man who consulted us in 1994 for recurrent bronchitis sometimes with haemoptysis. The physical examination was unremarkable. At consultation abnormal fascicular sounds were noted at the left base. A chest x-ray showed hyperlucency of the left lung associated with a very small left hilar shadow. We considered the diagnosis of Mac Leod's syndrome after eliminating a proximal obstructive pathology and pulmonary embolus. Mac Leod's syndrome or Swyer James' syndrome was described in 1954 and is suggested on the standard radiograph above all if the expiratory films ha…

Female lung cancer death rates increased by more than 550 percent between 1950 and 1991. In 1986 lung cancer surpassed breast cancer to become the leading cause of cancer death in women in the United States. The lung cancer epidemic is primarily attributable to cigarette smoking, which is responsible for at least 80% of the disease in women. There are gender differences in the distribution of lung cancer by histologic type, even controlling for smoking, and some data suggest greater female than male susceptibility to lung cancer at a given level of smoking. Exposure to environmental tobacco sm…

Aspiration lung injury is caused by a failure of the normal protective mechanisms that prevent aspiration of gastric contents. Prevention measures are focused on preserving these protective mechanisms. There are three components to the lung injury. The first component is mechanical obstruction to airways from particles in the aspirate. Treatment is aggressive pulmonary toilet to restore airway patency. The second component, seen beginning in the first several hours, is a chemical injury to the airways, leading initially to bronchorrhea, airway constriction, and edema. Later, there is a marked …

Bilateral operations on the lungs were carried out in 90 patients with hydatid disease, intrapulmonary metastases, carcinoma, and bronchiectasis. Various types of stepwise resections of the lung are the operation of choice. One-stage interventions are allowable in rare cases of circumscribed affection with a hydatid cyst or metastases. Sternotomy is the standard approach in such cases. The total postoperative mortality was 4.4%. Economical operations less than lobectomy in volume are safest and most effective. Neither complications nor fatal outcomes were encountered subsequently. More extensi…

The pneumoconioses, extrinsic allergic alveolitis, lung damage due to irritant gases, fumes, and smoke constitute the occupational lung diseases that affect the lung parenchyma. The pneumoconioses are diseases resulting from the accumulation of dust in the lungs. The ILO has established a standardized system for classification of these pneumoconioses that includes both descriptions of diffuse lung opacities and pleural disease. The most common of the fibrogenic pneumoconioses are silicosis, CWP, and asbestosis. The former two entities are characterized radiographically by the presence of small…

The interstitial lung diseases are comprised of a group of pulmonary disorders characterized clinically by diffuse infiltrates on the chest radiograph and histologically by distortion of the gas exchanging portion of the lung. The physiologic correlates are restriction of lung volumes and impaired oxygenation. The term "interstitial" when applied to these diseases is actually a misnomer because it implies that the inflammatory process is limited specifically to the area between the alveolar epithelial and capillary endothelial basement membranes. The diseases currently grouped as "interstitial…

We studied in vitro and in vivo epinephrine (E) synthesis by rat lung. Nine days after removal of the adrenal medullas, circulating E was reduced to 7% of levels found in sham-operated rats but 30% of lung E remained. Treatment of demedullated rats with 6 hydroxydopamine plus reserpine did not further reduce lung E. In the presence of S-[3H]adenosylmethionine lung homogenates readily N-methylated norepinephrine (NE) to form [3H]E. The rate of E synthesis by lung homogenates was progressively more rapid with increasing NE up to a concentration of 3 mM, above which it declined. The rate of E for…

To date, more than 200 human heart-lung transplantations (HLT) have been performed in USA and Europe. The main indications are still primary pulmonary hypertension and Eisenmenger's syndrome but the intervention has also been employed in cases of parenchymatous pulmonary disease. At operation, the thoracic organ is replaced. The recipient's heart can frequently be donated. Recent reviews suggest acceptable two-year survival following HLT of approximately 60%. The direct economical costs of transplantation have been stated to be approximately 17,000 pounds but to this must be added the expenses…

Since the successful clinical reintroduction of heart and lung transplantation in 1981, more than 350 of these procedures have been performed worldwide. Although survival following this operation is less than that reported for heart transplantation, the results are improving. It is clear that the increased technical difficulty of the procedure combined with the exquisite susceptibility of the transplanted lung to postoperative injury from infection, rejection or other causes account for these differences. In this report we provide an overview of the experience in heart-lung transplantation and…

Extravascular lung water (idQw1) is measured in vivo from the difference in mean transit times, computed by extrapolating the dilution curves, of two indicators, one freely diffusible, the other confined to the intravascular space. Using 3H2O it has been shown that idQw1 is smaller than the amount of extravascular water obtained from the difference between wet and dry lung weight (Qw1). Extrapolation allows one to use dilution curves for a short time, i.e., up to onset of obvious recirculation. Clearing the dilution curves or recirculation by deconvolution extends the observation time, which t…

The combined transplantation of heart and lungs, first done successfully by the Stanford Team (USA) in 1982, at present seems to be superseding lung transplantation alone, and has broadened the indications of heart transplantation to include terminal heart failure with fixed pulmonary arterial hypertension. After reviewing the causes for failure in lung transplants, the authors stress the superiority of heart-lung transplants compared to isolated lung transplantations: healing of the tracheal anastomosis, ease of detection of rejects by endomyocardial biopsy and the lack of inhomogeneity of th…

Traumatic lung pseudocyst is an uncommon lung injury due to closed chest trauma. Four cases are reported; all were male and one was a child. Three cases showed spontaneous resolution of the pseudocyst and in one case, where resolution was slow, lobectomy was carried out at the patient's insistence. Diagnosis poses no serious problems as there is inevitably a history of substantial chest trauma. The chest radiograph shows a characteristic cavitatory lesion. The pseudocysts may be multiple. Tomography may be helpful in diagnosis and computed tomography can be particularly useful in the demonstra…
