Symmetrical Midfacial Growth After Pediatric Mandibular Reconstruction With Free Fibula Flap.

BACKGROUND: Free fibula is the workhorse flap for mandibular reconstruction and is increasingly being used in pediatric patients. However, craniomaxillofacial growth and development involve interdependent processes, and it remains unknown whether mandibular reconstruction with free fibula allows symmetric growth of the midface. PURPOSE: The study evaluated midfacial symmetry after pediatric mandibular defect reconstruction. STUDY DESIGN, SETTING, SAMPLE: This retrospective cohort study included pediatric patients aged ≤14 years who underwent mandibular reconstruction with free fibula flap. P

Analysis of the effects of mandibular reconstruction based on microvascular free flaps after oncological resections in 21 patients, using 3D planning, surgical templates and individual implants.

INTRODUCTION: Post- resection defects in the area of the head and neck frequently result in functional impairment of the masticatory system and unaesthetic outcome in the facial appearance. In pediatric population they exert a devastating effect on speech development, breathing and facial growth leading to secondary deformations which are extremely challenging in reconstruction by means of conventional surgery protocols. MATERIAL AND METHODS: 21 patients (14 males/ 7 females) aged 4-17 years old, treated between 2015 and 2019 due to malignant/benign tumors or congenital deformities requiring

Biomechanical Analysis of Various Reconstructive Methods for the Mandibular Body and Ramus Defect Using a Free Vascularized Fibula Flap.

Several different methods exist for reconstructing the mandibular body and ramus defect with the use of a free vascularized fibula flap, but none have adequately addressed the long-term mechanical stability and osseointegration. The aim of this study is to compare the biomechanics of different surgical methods and to investigate the best approach for reconstructing the mandibular body and ramus defect. Five finite element models based on different reconstructive methods were simulated. Stress, strain, and displacement of connective bone sections were calculated for five models and compared. Th

Extensive Mandibular Ameloblastic Fibro-Odontoma.

Ameloblastic fibro-odontoma (AFO) is a mixed odontogenic tumor that presents epithelial and mesenchymal components. Ameloblastic fibro-odontoma is generally diagnosed between the first and second decades of life and normally shows a slow clinical growth in the posterior portion of the maxilla or mandible, being mostly associated with 1 or more impacted teeth. Radiographic features of AFO show a radiolucent well-defined, uni, or multilocular defect due to containing variable amounts of calcified material. The enucleation of the tumor is the usual conduct and should be followed up for a long per

Mandibular nerve fascicular cross-face for sensitive recovery after mandibulectomy: a new technique.

Trigeminal nerve damage after mandibulectomy is a condition that may occur not infrequently when oncologic resections of the maxillo-facial district are performed. In the last decades microsurgery has made possible effective osteomuscular reconstructions using vascularised free flaps. Nevertheless, this procedure, if assuring satisfactory results, involved the sacrifice of the mandibular nerve. Nowadays, supramicrosurgical innovation has shifted the gold-standard to a higher level, allowing the complete sensitive recovery after mandibulectomy. The cross-face nerve transfer technique is an inno

[Conservative surgery for a mandibular condyle osteochondroma].

INTRODUCTION: Osteochondroma is a rare benign bone tumor of the mandibular condyle (90 cases reported). The symptoms are similar to those of temporomandibular joint disorders: painful temporomandibular joint, joint sounds, and modification of dental occlusion. CASE REPORT: A 37-year-old female patient consulted for a wide left laterodeviation of the mandible. CT scan revealed a 27 mm long bony growth on the condyle. The condyle neck was osteotomized, then the tumor and condyle head were split, the condyle was then remodeled and osteosynthesized on the condyle neck. The functional and esthetic

Osteochondroma of the temporomandibular joint: a case report.

Osteochondroma of the mandibular condyle has been found in the oral and maxillofacial region rarely. This paper describes a case of osteochondroma of the mandibular condyle in a 20-year-old woman, who was referred to our service with facial asymmetry, prognathic deviation of chin, cross-bite to the contralateral side, changes in condylar morphology, limited mouth opening, and malocclusion. Computed tomography (CT) was performed for better evaluation to the pathological conditions on the temporomandibular joint. Based on the clinical examination, patient history, and complementary exams, the hy

Unicystic ameloblastoma: a possible pitfall in periapical diagnosis.

AIM: To report a clinical case of unicystic ameloblastoma previously misdiagnosed as radicular cyst. SUMMARY: A 49-year-old white male was referred to a private practitioner complaining of an asymptomatic bony hard swelling of the left posterior mandible. The patient's dental history indicated that his left mandibular first molar had been extracted approximately 10 years previously. At that time, preoperative radiographic examination demonstrated a radiolucent area of 1.5 cm diameter with well-defined margins involving the distal root of tooth 36. The lesion was diagnosed as cystic and surger

Haemangioma of the body of the mandible: a case report.

We present a case of haemangioma of the mandible in a 46-year-old woman. The radiological appearance was of a unilocular radiolucency that looked similar to other more common lesions of the mandible. This case shows that a potentially life-threatening haemangioma can present as an apparently benign cyst-like unilocular radiolucency, and highlights the need to include haemangioma in many differential diagnoses, and the importance of taking all radiological features into account.

Peripheral osteoma of the mandible: a study of 10 new cases and analysis of the literature.

Ten new cases of peripheral osteoma of the mandible are described. These lesions are uncommon and only 15 other cases have been reported in the English language literature over the past 30 years. There is evidence to suggest that the peripheral osteoma of the mandible is a traumatically induced reactive lesion and that muscle traction plays a role in its initiation. In view of this possibility, the term "parosteal osseous hyperplasia" may be more appropriate for those lesions in which a positive history of trauma preceded the onset.

[Temporomandibular joint dysfunction and neurofibromatosis. Apropos of a case].

The authors report one case of Von Recklinghausen's disease localized to the temporomandibular joint capsule and unilaterally associated with other cephalic manifestations. Although no other study, notably a CAT-scan, showed this tumor localization, surgical exploration was imperative and led to exeresis of the lesion which proved to be the treatment of choice.

When reconstruction isn't the end: Late-occurring ameloblastoma in a fibula-grafted mandible.

BACKGROUND: Ameloblastomas are rare odontogenic tumors with a high tendency for local recurrence, particularly in previously operated or reconstructed jaws. Late manifestations decades after initial therapy are extremely uncommon and pose diagnostic as well as surgical challenges. CASE PRESENTATION: We present the case of a 90-year-old woman with a history of squamous cell carcinoma of the mandible, treated decades earlier by segmental resection and microvascular fibula graft reconstruction. After nearly three decades of clinical stability, the patient developed bilateral mandibular masses wit

Influence of microvascular mandibular bony reconstruction on the posterior airway space: A retrospective cohort study.

INTRODUCTION: The posterior airway space (PAS) is a common site of passive obstructions with high morbidity. Surgical changes to the craniomandibular system may affect the PAS. Data regarding the effects of mandibular reconstruction using vascularized bone flaps on PAS are insufficient. This retrospective cohort study aimed to investigate changes in PAS after mandibular reconstruction. MATERIALS AND METHODS: Pre- and post-reconstructive computed tomography scans of 40 patients undergoing segmental mandibulectomy and mandibular reconstruction with deep circumflex iliac artery or fibula flaps w

Customed 3D-printed Polyetheretherketone (PEEK) Implant for Secondary Salvage Reconstruction of Mandibular Defects: Case Report and Literature Review.

Given the insufficient height of single-barrel fibula and inadequate bone volume of double-barrel vascularized fibula in mandibular reconstruction, it is a better choice to combine the upper full-thickness vascularized fibula with the lower half-thickness nonvascularized fibula. However, the nonvascularized fibula may fail due to complications, affecting the facial shape and occlusal function. Polyetheretherketone is a thermoplastic polymer used for bone defect reconstruction due to its good mechanical properties and biocompatibility. This case report mainly presents a secondary salvage recons

Risk factors associated with fixation-related complications in microsurgical free flap reconstruction of the mandible.

BACKGROUND: Hardware complications (loosening of screws, infection, or exposure of the plate) in mandibular reconstruction with vascularized osseous free flaps impose significant morbidity, and frequently require revision surgery. Purpose of this study was to identify possible contributing factors for hardware complications. METHODS: This is a retrospective cohort study involving case series of patients who underwent microvascular mandible reconstructions between 2000 and 2020. Patient demographics, pathological, clinical, and treatment-related factors were analyzed in univariate and multivar
