Medical and Non-Surgical Strategies for Mild Traumatic Brain Injury.

Historically, mild traumatic brain injury (mTBI) was often overlooked. However, growing clinical consensus emphasizes that active intervention is essential to manage physiological impairments and facilitate functional recovery. As secondary impact syndrome poses fatal risks during the acute phase, transitioning from passive rest to multimodal, active management is critical. This paper reviews current medical and non-surgical strategies for mTBI, shifting the paradigm from simple observation to proactive, evidence-based interventions. The review categorizes non-surgical interventions into 4 pri

Overview of Treatment Options for Mild Traumatic Brain Injury: A Literature Review.

The incidence and prevalence of concussion, a type of mild traumatic brain injury (mTBI), have steadily increased among athletes, both students and professionals, across a wide variety of sports, including, but not limited to, swimming, tennis, football, and boxing. Recent data have demonstrated that sports are one of the leading causes of concussions among student athletes. While the exact mechanism of concussion onset has yet to be fully elucidated, data suggest that the pathophysiology involves rotational acceleration and deceleration of the brain, leading to axon tearing and disturbance in

The Neurophysiological Effects of Craniosacral Treatment on Heart Rate Variability: A Systematic Review of Literature and Meta-Analysis.

Craniosacral treatment (CST) is an osteopathic technique grounded in the assumption that there is an intrinsic, fine movement of the cerebrospinal fluid. This rhythmic movement can be utilized for diagnostic and therapeutic purposes by palpation and manipulation of the skull, spine, and associated connective tissues. Therapeutic benefit is likely due to action on the autonomic nervous system (ANS), specifically through the vagus nerve. Current literature on the neurophysiological effects of CST is limited, which has contributed to controversy regarding its effectiveness. Heart rate variability

Short-term effects on heart rate variability of occipito-mastoid suture normalization in healthy subjects.

Occipito-mastoid structure normalization (OMSN) is an osteopathic manipulative treatment aimed at reducing tension around the jugular foramen, where cranial nerves IX, X, and XI exit the skull. The purpose of this study was to observe how heart rate variability (HRV), a marker of autonomic cardiac regulation, was modulated after an OMSN vs. a sham technique (SHAM). Pre- and post-intervention HRV was analyzed in two randomly chosen groups of 15 participants (OMSN vs. SHAM group). HRV was collected in the supine position 5 min before and 5 min after a 10-min application of either OMSN or SHAM. T

Tongue stretching: technique and clinical proposal.

OBJECTIVES: The tongue is an organ with multiple functions, from sucking to phonation, from swallowing to postural control and equilibrium. An incorrect position or mechanics of the tongue can causes sucking problems in the newborn or atypical swallowing in the adult, with repercussions on the position of the head and neck, up to influencing upright posture and other problems. Tongue dysfunctions are quite frequent (10-15%) in the population. For the manual therapist, this frequency indicates one to two subjects every 30 patients. Exercises have been proposed to improve the tone and strength o

Pilot study MOVENDOP protocol - impact on quality of life following postoperative osteopathic abdominal mobilizations in patients operated for endometriosis.

BACKGROUND: Surgery remains the gold standard for management of endometriosis, offering significant improvement in patient pelvic pain and quality of life (QoL). Postoperative tissue adhesions can however diminish these benefits, limiting the long-term effectiveness of the intervention. Despite the development of strategies and devices to reduce adhesion formation, their efficacy remains inconclusive. OBJECTIVE: This study aims to propose and evaluate a novel approach involving early visceral mobilization and training of patients in abdominal self-mobilization as a means to improve QoL follow

[Results of manual therapy in degenerative spine diseases with and without radiculopathy. (Literature review)].

The prevalence of degenerative diseases of the spine, including neck and low back pain, which reduce the ability to work and lead to disability, is increasing. We have studied the materials of clinical studies, meta-analyses and systematic reviews devoted to the evaluation of the results of manual therapy in patients with degenerative spine diseases, including those who underwent surgical treatment. The analysis (38 sources were analyzed, including 15 meta-analyses and 21 systematic reviews) revealed the ability of manual therapy to influence the level of acute and chronic pain (local and proj

Soft tissue manipulation enhances recovery of muscle mass in a disuse model of sarcopenia.

CONTEXT: Sarcopenia is a disease characterized by low muscle mass and function that places individuals at greater risk of disability, loss of independence, and death. Current therapies include addressing underlying performance issues, resistance training, and/or nutritional strategies. However, these approaches have significant limitations, and chronic inflammation associated with sarcopenia may blunt the anabolic response to exercise and nutrition. This presents an unmet need for treatment strategies that promote gains in muscle function. One such possibility is soft tissue manipulation (STM)

Evaluating median nerve stiffness in patients with mild to moderate-severe idiopathic carpal tunnel syndrome receiving OMT and conservative therapy: a pilot study.

CONTEXT: Osteopathic manipulative treatment (OMT) has been recognized as a conservative management option for patients with carpal tunnel syndrome (CTS), although limited research exists to validate its ability to effect posttreatment changes in the median nerve or the surrounding soft tissues. OBJECTIVES: The objectives of this study are to evaluate and quantify changes in the elasticity of the median nerve, transverse carpal ligament (TCL), and intracarpal tunnel soft tissues in patients treated for CTS with traditional conservative therapy (e.g., steroid injection and splinting), OMT, or O

Effect of osteopathy in the cranial field on visual function--a pilot study.

CONTEXT: The effects of osteopathy in the cranial field on visual function-particularly on changes in the visual field and on the binocular alignment of the eyes-have been poorly characterized in the literature. The authors examined whether osteopathy in the cranial field resulted in an immediate, measurable change in visual function among a sample of adults with cranial asymmetry. STUDY DESIGN: Randomized controlled double-blinded pilot clinical trial. SUBJECTS: Adult volunteers between ages 18 and 35 years who were free of strabismus or active ocular or systemic disease were recruited. Inc

HEMOLYTIC TRANSFUSION REACTIONS.

After receiving apparently compatible blood three patients suffered hemolytic reactions. The compatibility tests were by saline and indirect Coombs technique including a screening tube of group 0 cells. The antibodies responsible for these reactions were not clearly demonstrable for several days following the transfusion. In two instances the antibody was anti-E.These case reports point up the following. (a) Currently used cross-matching procedures will occasionally fail to demonstrate an incompatibility. (b) In two of the cases the direct Coombs test was negative on an immediate post-transfus

Osteopathy modulates brain-heart interaction in chronic pain patients: an ASL study.

In this study we used a combination of measures including regional cerebral blood flow (rCBF) and heart rate variability (HRV) to investigate brain-heart correlates of longitudinal baseline changes of chronic low back pain (cLBP) after osteopathic manipulative treatment (OMT). Thirty-two right-handed patients were randomised and divided into 4 weekly session of OMT (N = 16) or Sham (N = 16). Participants aged 42.3 ± 7.3 (M/F: 20/12) with cLBP (duration: 14.6 ± 8.0 m). At the end of the study, patients receiving OMT showed decreased baseline rCBF within several regions belonging to the pain mat

The immediate effects of sigmoid colon manipulation on pressure pain thresholds in the lumbar spine.

Visceral manual therapy is increasingly used by UK osteopaths and manual therapists, but there is a paucity of research investigating its underlying mechanisms, and in particular in relation to hypoalgesia. The aim of this study was to investigate the immediate effects of osteopathic visceral mobilisation on pressure pain thresholds. A single-blinded, randomised, within subjects, repeated measures design was conducted on 15 asymptomatic subjects. Pressure pain thresholds were measured at the L1 paraspinal musculature and 1st dorsal interossei before and after osteopathic visceral mobilisation

Manual therapy modalities and depression: a systematic review.

CONTEXT: Depression and other mood disorders have a significant impact on patients, affecting their daily functioning, physical health, and emotional well-being. While psychopharmacological medications and psychotherapy are the most common evidence-based treatments (supported by a significant amount of scientific research), there is growing research that supports the use of complementary therapies (nonmainstream approaches utilized together with conventional medicine), including manipulative or manual therapies. OBJECTIVES: This systematic review contributes to the existing literature by eval

Complementary and Alternative Medicine Therapies in relation to Self-Reported Better Sleep across Racial and Ethnic Groups in the United States.

OBJECTIVES: Complementary and Alternative Medicine (CAM) approaches vary across race and ethnicity, reflecting different cultural practices. However, its relationship with sleep is understudied. We investigated cross-sectional CAM-sleep associations across racial and ethnic groups. METHODS: In the nationally-representative 2012 National Health Interview Survey, participants identified their most important CAM therapies and whether the therapy yielded 'better sleep' (yes/no). We used Poisson regression with robust variance to estimate adjusted prevalence ratios (aPRs) and 95% confidence interv

Application of 3D printed customized external fixator in fracture reduction.

INTRODUCTION: Long bone fracture is common in traumatic osteopathic patients. Good reduction is beneficial for bone healing, preventing the complications such as delayed union, nonunion, malunion, but is hard to achieve. Repeated attempts during the surgery would increase the operation time, cause new damage to the fracture site and excessive exposure to radiation. Robotic and navigation techniques can help improve the reduction accuracy, however, the high cost and complexity of operation have limited their clinical application. MATERIALS AND METHODS: We combined 3D printing with computer-ass
