[Perioperative psychological status of patients undergoing radical mastectomy and total hysterectomy].

Using four different psychological tests, namely Spielberger's rating scale [The State-Trait Anxiety Inventory (STAI)], Self-Rating Depression Scale (SDS), Maudsley Personality Inventory (MPI) and Baum test, we investigated the perioperative psychological status of patients undergoing radical mastectomy for breast cancer and compared data with those of patients undergoing total hysterectomy for uterine myoma. Parameters reflecting the psychological status of hysterectomy patients disclosed lower postoperative anxiety, and slight nervousness and persistent depression perioperatively. In contras…

[Breast reconstruction using a skin expander prostheses following modified radical mastectomy].

In the period from September 1983 to May 1986, 116 breast reconstructions were performed at Heidelberg University Gynecological Clinic with the aid of skin expander prostheses, following modified radical mastectomy (89 primary and 27 secondary reconstructions). In 18% of all cases, complications occurred due to the expanders, the most common being loss of prosthesis material. A total of 82 women have meanwhile had the expander prosthesis replaced by a definitive gel prosthesis in a "second reconstruction phase". Reconstruction of the nipple and areola has been performed in six cases in a third…

Locoregional Recurrence After Mastectomy: Risk Factors and Implications for Postmastectomy Radiation.

INTRODUCTION: Locoregional recurrence (LRR) after mastectomy is a critical concern in breast cancer management, although contemporary recurrence patterns in the era of modern systemic therapy and the expanded use of postmastectomy radiation therapy (PMRT) are not well defined. METHODS: We retrospectively reviewed 595 women who underwent mastectomy for invasive breast cancer at a single academic center between 2014 and 2020. Patient demographics, clinicopathologic features, treatments, and outcomes were abstracted from the electronic medical record. Univariable and multivariable Cox proportion…

Unexpected parasternal musculature during skin-sparing mastectomy and microvascular breast reconstruction.

The sternalis muscle is an uncommon anatomical variant of the anterior thoracic wall. Although regarded as functionally insignificant, its presence may influence diagnostic interpretation, surgical dissection and reconstructive planning in breast surgery. We report bilateral sternalis muscles discovered during bilateral skin-sparing mastectomy and immediate autologous reconstruction in a woman in her late 40s. The sternalis muscles were identified intraoperatively, following which retrospective review of preoperative imaging confirmed their presence. Recognition of this variant aided orientati…

William Halsted's Radical Mastectomy Patients: A Historical Analysis Through Patient Narratives.

The radical mastectomy-a surgery pioneered by William Stewart Halsted at The Johns Hopkins Hospital-was the gold standard surgical treatment for breast cancer until the late 20th century. The procedure included the removal of the breast and lymph nodes, as well as the pectoral muscles. The medical debate surrounding the radical mastectomy is well documented in medical journals and secondary historical sources. Also well documented are Halsted's life, surgical innovations, and his long-standing struggle with addiction. Yet, the experiences of Halsted's patients who underwent this procedure rema…

Breast-Conserving Surgery Versus Mastectomy After Neoadjuvant Therapy: A Survival Analysis.

BACKGROUND: Breast cancer management has increasingly incorporated neoadjuvant therapy (NAT), particularly to improve rates of pathologic complete response (pCR) and expand eligibility for breast-conserving surgery (BCS) instead of mastectomy. Using SEER database data, we evaluated whether BCS versus mastectomy after NAT is associated with different survival outcomes in patients with pCR. PATIENTS AND METHODS: We performed a population-based study including women with nonmetastatic invasive breast cancer diagnosed between 2010 and 2015 who received NAT followed by BCS or mastectomy and achiev…

Late onset nipple recurrence of borderline phyllodes after nipple-sparing mastectomy.

Phyllodes tumours (PTs) are infrequent fibroepithelial neoplasms that account for <1% of all primary breast tumours. Local recurrence (LR) rates and distant metastasis vary depending on tumour histology. In this case report, the patient developed a painful growing tumour in the nipple after nipple-sparing mastectomy 17 years before. Local excision was performed with a final diagnosis of borderline PT after 205 months disease-free.The management of surgical margins of PT remains under discussion due to the high risk of LR. Breast-conserving surgery, when feasible, is considered the surgical app…

Implant Size Versus Mastectomy Weight in Breast Reconstruction: Does Matching Matter?

INTRODUCTION: During nipple-sparing mastectomy with direct-to-implant based breast reconstruction, if the size of the chosen implant is smaller than the mastectomy specimen, a volumetric mismatch occurs creating dead space. This study evaluates the effect of volumetric mismatch between chosen implant size and mastectomy weight on postoperative complications after breast reconstruction. METHODS: A multicenter retrospective cohort study was conducted on patients undergoing nipple-sparing mastectomy with direct-to-implant based breast reconstruction. The effect of volumetric mismatch [mastectomy…

Targeted Nipple Areola Complex Reinnervation in Gynecomastia Mastectomy: A Case Report.

Targeted nipple areola complex (NAC) reinnervation (TNR) to restore nipple and chest sensation has been previously described in patients undergoing breast reconstruction and gender-affirming mastectomy. A healthy 32-year-old male, with grade II gynecomastia with severe skin laxity and a BMI of 25 kg/m2, underwent bilateral mastectomy with free nipple grafting (FNG) for persistent gynecomastia and chest wall soft tissue laxity. The mastectomy weight was 242.5 (range: 242-243) grams. Three lateral intercostal nerves (3rd, 4th and 5th) were preserved and directly coaptated to the new NAC on each …

Infections after mastectomy and tissue expander placement: A multivariate regression analysis.

BACKGROUND: While breast surgery is considered a clean case, tissue expander-based breast reconstruction (TE-BR) has infection rates quoted up to 31%, decidedly higher than the typical 1% to 2% rate of surgical site infections. Through multivariate regression, we sought to analyze risk factors that contribute to infections following TE placement. METHODS: A retrospective study reviewed all patients undergoing mastectomy with immediate or delayed TE placement over a 22-month period. Infections were defined as clinically documented cellulitis or infection, return to the operating room (RTOR) fo…

EBCC-13 manifesto: Balancing pros and cons for contralateral prophylactic mastectomy.

After a diagnosis of unilateral breast cancer, increasing numbers of patients are requesting contralateral prophylactic mastectomy (CPM), the surgical removal of the healthy breast after diagnosis of unilateral breast cancer. It is important for the community of breast cancer specialists to provide meaningful guidance to women considering CPM. This manifesto discusses the issues and challenges of CPM and provides recommendations to improve oncological, surgical, physical and psychological outcomes for women presenting with unilateral breast cancer: (1) Communicate best available risks in manag…

Impact of Patient Primary Language upon Immediate Breast Reconstruction After Mastectomy.

BACKGROUND: Preoperative decision-making in patients who speak a primary language other than English is understudied. We investigated whether patient primary language is associated with differences in immediate breast reconstruction (IBR) after mastectomy. PATIENTS AND METHODS: This retrospective observational study analyzed female patients undergoing mastectomy in the New Jersey State Inpatient Database (2009-2014). The primary outcome was the odds of IBR with a prespecified subanalysis of autologous tissue-based IBR. We used multivariable logistic regression and hierarchical generalized lin…

Comparison between Gasless and Gas-Inflated Robot-Assisted Nipple-Sparing Mastectomy.

PURPOSE: Nipple-sparing mastectomy (NSM) includes various techniques, including conventional or endoscopic mastectomies. Since the introduction of robot-assisted NSM (RANSM) in 2015, 2 main methods have been used: gasless and gas-inflated techniques. The aim of this study was to compare clinicopathologic characteristics, surgical outcomes, and postoperative complications between patients treated with gasless RANSM and those treated with gas-inflated RANSM. METHODS: We conducted a retrospective study of women who underwent gasless or gas-inflated RANSM with immediate breast reconstruction betw…

Is staging breast magnetic resonance imaging prompted upgrade to mastectomy appropriate?

BACKGROUND: Breast magnetic resonance imaging (MRI) use for surgical staging is increasing, though remains controversial. We aimed to evaluate the accuracy of MRI in surgical decision-making to determine if mastectomy prompted by MRI was appropriate. METHODS: A single-centre observational study in Perth, Western Australia, with the inclusion of all preoperative and postoperative studies (e.g. involved margins after breast-conserving surgery) undergoing staging breast MRI from 1 January 2015 to 26 August 2019. A standard protocol using gadolinium contrast was used. The reference standard was p…
