INTRODUCTION: Otogenic cerebral sinus thrombosis (OCST) represents a rare but serious pathology. The medical and surgical treatment to achie … criteria with a total of 149 patients. 88.5% of patients underwent surgery, most commonly cortical mastoidectomy. 54.8% of patients received anticoagulation. Sixty-five percent of patients with reimaging had complete recannalisation of the sinus and 9.4% of patients experienced long term complications.
CONCLUSIONS: The combination of antibiotics, anticoagulat …

A prospective case review of 3 patients treated at a tertiary referral center from 1997 to 2002 are presented. Three men, age 30 to 68 (mean, 45) years, were treated for acquired cholesteatoma with skull base invasion. Two patients had no prior otological surgery, and 1 was detected after 4 prior mastoid procedures. One presented with progressive unilateral facial palsy, 1 with otorrhea, and 1 with temporal mass above a modified radical mastoidectomy cavity. All 3 patients were treated with single-staged canal wall-down mastoidectomy. Patients were followed up with otoscopy and neuroimaging fr…

Thirty children with acute mastoiditis were identified over a 12-year-period and their hospital records were reviewed retrospectively. All h … s of acute mastoiditis on physical examination. Overall, 19 (63%) of the children recovered without mastoidectomy. We conclude that children without meningitis or subperiosteal abscess may be treated initially with antimicrobial therapy plus myringotomy. The need for mastoidectomy should be reassessed in children who fail to respond in 24 to 48 hours.

BACKGROUND: Mastoid obliteration is a useful surgical step in the treatment of chronic ear operation. The technique of mastoid obliteration … d epitympanum obliteration were performed.
RESULTS: Fifteen patients who underwent canal wall down mastoidectomy for cholesteatoma were included in the study. In all patients, the targeted external ear canal and hearing results were obtained after obliteration.
CONCLUSION: It is an effective surgical technique for creating a natural external ear path after ma …

Cochlear implantation is anatomically more difficult in postoperative ears than in normal ears due to chronic otitis media. In cochlear implantation after canal wall down mastoidectomy, the electrodes must be protected. We report a case of cochlear implantation with partial mastoid obliteration using a retrofacial approach in an 81-year-old woman. Bilateral tympanoplasty was performed for bilateral chronic otitis media, but no improvement in hearing was observed. Hearing aids were used, but their effect was insufficient and cochlear implantation was needed. Since the patient was an older adult…

The study includes 54 cholesteatomatous ears in 50 children aged 16 years or less. The mean follow-up period after surgery was 7.1 years. In 26% of the ears the cholesteatoma was 'huge' involving the middle ear and the attic and filling the entire mastoid air cell system. A patient had lateral sinus thrombophlebitis. Patients with large cholesteatomas underwent canal wall down mastoidectomy with simultaneous tympanoplasty and, in most cases, cavity obliteration. Limited cholesteatomas were removed using either intact canal wall mastoidectomy or tympanotomy approach. Recurrence rate (including …

From the results of 689 cases of cholesteatoma operated and followed over a period of 9 years, are discussed the respective indications of opened and closed techniques. The intact canal wall tympanoplasty with mastoidectomy is the preferred technique. It is undertaken on the understanding that it will be done in 2 stages over a 1-year interval. From the revised 460 cases, the presence of residual cholesteatoma was found in one out of 2 cases. Cause of high incidence are analyzed and commented. Retraction pockets or true recurrence of cholesteatoma whose incidence was 11% at the beginning of th…

PURPOSE: To present the first pediatric study on the safety and efficacy of mastoid obliteration using S53P4 bioactive glass (BAG) for cholesteatoma surgery.
METHODS: A single-center retrospective cohort study was conducted. Inclusion criteria were pediatric cases (≤ 18 years) and at least at least one year of follow-up including non-echo planar diffusion-weighted MRI to assess cholesteatoma recidivism. Both canal wall up (CWU) and canal wall down (CWD) procedures were evaluated.
RESULTS: A total of 61 cases (56 patients) were included. Most cases had an otologic history before the developme

UNLABELLED: Cholesteatomas in children have a more aggressive growth pattern compared to adults, which leads to a higher incidence of both residual and recurrent disease. A staged canal wall-up or a canal wall-down tympanomastoidectomy (CWUT and CWDT, respectively) is selected depending on the extent of the disease and condition of the middle ear (ME) cleft and mastoid. Endoscopic ear surgery (EES) has been recently introduced as an adjuvant tool for the treatment of this pathology even in the pediatric population.
OBJECTIVES: To analyze long term outcomes of CWUT and CWDT in the pediatric po…

OBJECTIVES: The aim of this scoping review was to evaluate existing classifications of surgical procedures of the middle ear and mastoid and find a suitable classification that could serve as an international standard.
MATERIALS AND METHODS: Scoping review with a systematic literature search using reference tracking and a syntax including all surgical procedures in mastoid and middle ear surgery and their synonyms. Studies were selected based on inclusion and exclusion criteria.
RESULTS: Eleven reported classifications were included; six of which focused on middle ear surgery, two on mastoid

Cholesteatoma is a non-neoplastic cystic lesion arising in the middle ear cleft with the propensity to spread and recur after surgery, but it is unusual to find cholesteatoma invading sternocleidomastoid muscle after 15 years of modified radical mastoidectomy and presenting as Bezold abscess. In this report, the authors highlight the fact that cholesteatoma recurrence if neglected can present as a Bezold abscess with the invasion of cholesteatoma from the mastoid tip into the sternocleidomastoid muscle. This requires complete excision of the cholesteatoma sac along with the surrounding soft ti…

In 1984-1987 the authors followed up a selected group of 15 children with a long-standing history of secretory otitis media and latent mastoiditis. In there of them the condition was complicated by labyrinthitis. Mastoidectomy which was performed in 12 children contributed to the healing of chronic inflammatory changes in the middle ear.

To our knowledge this is the first report of a patient with infectious mononucleosis associated with a mononuclear granulomatous mastoiditis and facial paralysis. Pathologic examination of tissue from this patient, removed at the time of radical mastoidectomy, provides a basis for further speculation about the relationship between facial paralysis and infectious mononucleosis.
