Association of maternal birth weight and maternal preterm birth with subsequent risk for adverse reproductive outcomes: The Women's Health Initiative.

BACKGROUND: Advancements in medical technology and pharmacologic interventions have drastically improved survival of infants born preterm and low birth weight, but knowledge regarding the long-term health impacts of these individuals is limited and inconsistent. AIM: To investigate whether an individual's birthweight or history of being born preterm increases the risk of an adverse reproductive outcome. STUDY DESIGN: Nested case-control study within the Women's Health Initiative. SUBJECTS: 79,934 individuals who self-reported their personal birthweight category and/or preterm birth status.

Aggressive behavior of children exposed to intimate partner violence: an examination of maternal mental health, maternal warmth and child maltreatment.

OBJECTIVES: Over 4.5 million children each year are exposed to intimate partner violence (IPV). Furthermore, IPV rarely occurs without other forms of violence and aggression in the home. IPV is associated with mental health and parenting problems in mothers, and children experience a wide variety of short-term social adjustment and emotional difficulties, including behavioral problems. The current study investigated the influence of IPV exposure on children's aggressive behavior, and tested if this relation was mediated by poor maternal mental health, and, in turn, by maternal warmth and child

Conflict-Related Disruption of Maternal Health Services: A Case Report of Maternal Death in Transit Due to Obstructed Labour in Somalia.

Maternal mortality remains a major public health challenge in fragile and conflict-affected settings, where insecurity can disrupt referral pathways, skilled birth attendance, and timely emergency obstetric care. This case report describes a maternal death in transit from a remote nomadic community in Mahas District, Hirshabelle State, Somalia, and uses the Three Delays Model to frame the health-system failures contributing to the outcome. A 22-year-old primigravida woman developed prolonged labour at home under the care of a traditional birth attendant. The nearest district health facility ha

Maternal mental health and its intergenerational transmission in-utero: a scoping review.

Maternal mental health during pregnancy is important for maternal wellbeing and offspring development. Prenatal depression, anxiety, stress, and psychological distress have been associated with offspring emotional, behavioural, cognitive, and mental health outcomes, but evidence remains dispersed across conditions, outcomes, and proposed mechanisms. This scoping review synthesises empirical evidence on maternal mental health during pregnancy as a potential intergenerational exposure, including potential in-utero pathways.Five databases (MEDLINE, Scopus, PsycInfo, CINAHL, and Embase) were searc

Impact1: Supporting Innovation in Pediatric and Maternal Health-5 Years of Outcomes.

PURPOSE: Impact1 at the Stanford Mussallem Center for Biodesign was established in 2019 through its core partnership with UCSF as an FDA-funded Pediatric Device Consortium. We hypothesized that providing innovators in pediatric and maternal healthcare with mentorship, connections to experts, and grant funding would promote development, validation, approval, and commercialization of safe and effective health technologies for pediatric and maternal patients, and lead to positive broader impacts, measured as patients treated, capital raised, and jobs created. METHODS: Data were collected from 18…

Linking maternal childhood maltreatment to infant socioemotional development through perinatal mental health.

BACKGROUND: Childhood maltreatment (CM) is associated with an elevated risk of experiencing psychological problems, particularly during the perinatal period. Maternal history of CM is also associated with disruptions to offspring socioemotional development. One potential pathway through which maternal CM contributes to offspring socioemotional development is through maternal mental health. OBJECTIVE: The present study explored how maternal CM is associated with infant socioemotional development through maternal mental health, both during pregnancy and postpartum. PARTICIPANTS AND SETTING: Pa

Changes in Maternal Care and Infant Health After Noneconomic Damage Cap Repeals.

IMPORTANCE: Noneconomic damage caps, a form of medical malpractice law, remain controversial, as several states have enacted such laws since 2010, whereas others have repealed them. The clinical consequences of repealing these caps are poorly understood, and understanding these associations can inform the ongoing debate about medical malpractice reform. OBJECTIVE: To examine whether repealing noneconomic damage caps is associated with changes in maternal care and infant health outcomes. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study adopted a difference-in-differences design,

Placenta Biology, Future Cardiovascular Health, and Impact on the Maternal-Fetal Heart.

The placenta is a highly specialized organ that ensures the developing fetus gets adequate oxygen and nutrients from its mother. The placenta shares common development pathways with the fetal heart and releases factors that can influence the way the mother's heart functions. Thus, pregnancy complications, where the placenta fails to develop properly, can lead to the development of fetal heart defects and induce long-term maternal cardiac dysfunction. In this review, we focus on how the placenta shapes cardiac events during development and the subsequent long-term impact on the offspring's hear

Placental microRNAs as molecular links between maternal metabolic health and neonatal outcomes.

BACKGROUND AND AIMS: Gestational diabetes (GDM) predisposes women and their offspring to future cardiometabolic disease. Dysregulation of microRNAs (miRNAs) has been linked to environmental influences and complex diseases. MiRNAs (namely miR-27a-3p, -222-3p, -423-3p and -16-5p) and lipoprotein lipase (LPL) are involved in insulin-signaling, glucose and lipid metabolism. Nevertheless, the role of the placental miRNAs in metabolic adaptation in pregnancy remains poorly understood. This cross-sectional study aimed to evaluate the association between placental selected miRNAs expression and clinic

Mitochondrial Dysfunction: A Critical Link Between Maternal Diet and Offspring Metabolic Health.

Background/Objectives: The developmental origins of health and disease (DOHaD) theory suggests that intrauterine and early postnatal life represents a critical window for programming lifelong health trajectories and disease susceptibility in offspring. Maternal nutritional imbalance during this period is closely associated with obstetric complications and an elevated risk of metabolic disorders in children. As central metabolic hubs, mitochondria constitute a critical axis linking adverse in utero exposure to metabolic defects in offspring across generations. Methods: In this narrative review,

Epigenetic signatures of maternal-fetal health: insights from cord blood and placenta.

The placenta is vital for fetal growth, and its methylation patterns reflect placental function, affecting the fetus and providing insights into disease origins. While cord blood methylation is convenient for assessing the fetal environment, methylation profiles vary by tissue due to variance in cell populations, function, and life stage. As tissue differences extensively contribute to the DNA methylation patterns, using surrogate samples such as cord blood may result in inconsistent findings. In this study, we aim to quantify the correlation of cytosine-phosphate-guanine dinucleotides (CpGs)

Assessing Perinatal Psychiatric Morbidity: Implications for Maternal Mental Health Care in Italy.

BACKGROUND: Traumatic births impact women's long-term health, family dynamics, and healthcare systems, underscoring the need for prevention and effective interventions. Despite Italy's universal healthcare, perinatal mental health services and guidelines, especially for childbirth-related PTSD (CB-PTSD), remain underdeveloped. This study aims to investigate the prevalence of CB-PTSD, postpartum depression (PPD), and anxiety in Italian women 6-12 weeks postpartum, and assess the impact of comorbidities on mother-child bonding. METHOD: The study was part of a broader longitudinal research that

Expert Review: Confronting Climate-Driven Heat Risks to Maternal and Fetal Health.

IMPORTANCE: In the current era of climate change, extreme heat exposure poses escalating threats to maternal-fetal health. Despite the expansive scale of extreme heat exposure worldwide, dramatized by the record-breaking global ocean and atmospheric temperatures throughout 2023 and into 2024, the extent of the threat posed by heat is underestimated and underappreciated. Unlike the physical destruction wrought by climate-driven events like hurricanes and wildfires, heat exposure does not cause severe damage to the built environment. Yet, in most years, heat has been the deadliest hazard in the

From exposure to outcomes: How air pollutants impact maternal and foetal health.

Air pollution is a leading environmental risk factor, contributing significantly to respiratory illnesses, cardiovascular diseases, and other chronic conditions; ultimately increasing the global burden of ailments. While the harmful effects of air pollutants on the respiratory and cardiovascular systems are well-known, concerns over their impact on pregnancy outcomes are rising. Pregnant women and their developing fetuses are particularly more susceptible to the harmful effects of air pollutants, which can penetrate the placental barrier and lead to a wide range of negative consequences, such

Improving maternal health literacy among low-income pregnant women: A systematic review.

Previous studies on maternal health have highlighted the need to improve health literacy, particularly among women from lower socioeconomic backgrounds. Some crucial factors for improving maternal health literacy are midwife capacity and systems support that can help ensure women's ability and motivation to access timely health services. However, the extent of roles midwives need and the system that must be developed require further elaboration. The aim of this systematic review was to investigate approaches for enhancing maternal health literacy in low-income pregnant women. Using Preferred R…

Awareness of the Maternal Health Benefits of Lactation Among U.S. Pregnant Individuals.

INTRODUCTION: We assessed awareness of the maternal health benefits of lactation among a sample of nulliparous pregnant individuals in the United States, identified variables associated with awareness of these benefits, and examined whether awareness of these benefits impacts breastfeeding attitudes or intentions. METHODS: We administered a web-based survey to nulliparous U.S.-born individuals carrying a singleton gestation of at least 28 weeks. We assessed awareness of the maternal health benefits of lactation using 10 items to create a summative score. We examined variation in awareness of …

Crafting the unsayable: Making meaning out of racialised maternal health-care encounters.

There are persistent and profound racialised inequalities in maternal and reproductive health in the UK. Yet in multiple settings, these disparities have been blamed on class or ethnicity, individuals and communities rather than the structures within which they live. In this study, we draw on narratives told within a 'slow-stitch' craft workshop, organised in southern England for racialised women with reproductive trauma, to show how processes of racialisation and racism shape experiences of maternal and reproductive healthcare. Experiences of reproductive trauma were multiple and cumulative.
