Correction of the long face syndrome by Le Fort I osteotomy. A report on some new technical modifications and treatment results.

The combined efforts of multiple specialists are needed for the successful treatment of patients with the long face syndrome. The plan of therapy is designed to correct the individual's dentofacial deformity and achieve the best possible occlusion and facial esthetics. Reduction of facial height by surgical means and proper alignment of the teeth by orthodontic and restorative procedures are the common denominators of successful treatment. By properly planned and executed Le Fort I maxillary osteotomies, the vertical dimension of the face can be shortened to improve the esthetic balance betwee

[Hybrid osteosynthesis in orthognathic surgery: 28 cases of Le Fort I osteotomy].

INTRODUCTION: The use of resorbable materials in orthognathic surgery is presently the object of numerous clinical and experimental studies. Their main drawbacks are their lack of rigidity and induced chronic inflammatory reactions. The purpose of this study was to propose and to test an original system of hybrid osteosynthesis associating a single titanium plate with three resorbable plates in Le Fort I osteotomies in the absence of maxillary expansion, or two titanium plates with two resorbable plates in case of maxillary expansion. METHODS: Our retrospective study concerned 28 patients ope

Surgically assisted rapid orthodontic lengthening of the maxilla in primates--a pilot study.

Orthodontic and surgical treatment of patients with maxillary retrusion and/or midface hypoplasia is challenging. This study was designed to show that the maxilla can be successfully lengthened by surgical assisted rapid orthopedic movement, using the principles of distraction osteogenesis. Three experimental and three control adolescent cynomolgus primates were used in this study. Metallic markers were placed in the cranial base and the maxilla and cephalometric x-ray films were taken. An orthodontic appliance was constructed with a Glen-Ross screw (Dentaurum, Newtown, Pa.) oriented anteropos

Temporomandibular joint disorders and maxillomandibular malformations: role of condylar "repositionin" plate.

Even if the relationship between condylar position and/or temporomandibular disorders (TMDs) and dentofacial deformity is controversial in literature, several patients presenting malocclusion refer to pain and TMDs as the main trouble. There are also various opinions concerning the alterations or improvements of temporomandibular joint symptoms after orthognathic surgery. In agreement with the experience of Universität Würzburg, the purpose of this study was to evaluate the validity of splint technique to reproduce centric condyle positioning in bimaxillary osteotomy surgeries for the patients…

Do Patients With Cleft Lip and Palate Have an Increased Risk of Short-Term Complications After Le Fort I Osteotomy?

BACKGROUND: Patients with cleft lip and palate (CL/P) often require Le Fort I maxillary advancement for correction of maxillary hypoplasia. Because of scar tissue formation, aberrant anatomy, and diminished vascularity, these patients may be at higher risk for complications, compared with patients without CL/P. The objective of this study was to evaluate 90-day postoperative complications of Le Fort I osteotomy in patients with CL/P compared with patients without CL/P. The secondary objective was to evaluate differences in reoperation and readmission rates between cohorts. MATERIALS AND METHO

The surgical correction of maxillary hypoplasia and midfacial retrusion.

On the basis of 35 maxillary osteotomies of the Le Fort I and III types, the authors discuss the main problems connected with treatment of maxillary hypoplasia and midfacial retrusion. Special attention is paid to cases that demand coordinated treatment including skeletal as well as soft tissue repair and reconstruction. Thanks to combined treatment in all cases, including 7 patients with partial backward displacement of the osteotomized fragments whose occlusion could not be entirely corrected, striking or highly satisfactory improvement of appearance was achieved.

Transmaxillary Sinus Approach for Le Fort II Osteotomy.

The Le Fort II osteotomy is a relatively rare operation. The posterior wall osteotomy of the maxillary sinus (MS) is often difficult for Le Fort III. We developed the transmaxillary sinus approach (TSA) for the Le Fort II osteotomy that cuts the posterior wall of the MS directly. This report illustrates this easy-to-use procedure for the Le Fort II osteotomy in syndromic craniosynostosis. This procedure was performed in an 18-year-old patient with Apert syndrome and a 15-year-old patient with Pfeiffer syndrome. The thin anterior walls of the MS were removed through an intraoral approach to loo

Segmental distraction osteogenesis with modified LeFort II osteotomy for a patient with craniosynostosis.

In this article, we report successful orthodontic treatment combined with segmental distraction osteogenesis after a modified LeFort II osteotomy in a patient with craniosynostosis. An 8-year-old boy diagnosed with craniosynostosis had a dished-in face, an anterior crossbite, and a skeletal Class III jaw relationship because of midfacial hypoplasia. At the age of 13 years 6 months, the maxillary second and mandibular first premolars were extracted, and leveling and alignment of both arches was started with preadjusted edgewise appliances. At age 14 years 11 months, the patient had a modified L

Piezosurgery-Assisted Endoscopic Transpterygoid Approach for Maxillary Ameloblastoma.

The use of piezoelectric instrumentation has been investigated to perform an endoscopic-assisted transpterygoid maxillectomy for a maxillary ameloblastoma. This technology allowed safe disinsertion of the pterygoid root from the cranial base through a precise and clean osteotomy, while minimizing the risk of inadvertent soft-tissue injury. Piezoelectric saws can represent an alternative to high-speed endoscopic drills in selected cases. For posterior maxillary neoplasms, we recommend the employment of endoscopic-assisted maxillectomy, as it offers improved anatomical exposure and a more accura

Sinus pathology and anatomy in relation to complications in lateral window sinus augmentation.

Antral pathoses and anatomical variations increase the risk of surgical complications during a lateral window sinus augmentation procedure. Therefore, an understanding of maxillary sinus diseases and anatomies is imperative. In the first part of this article, common sinus diseases will be reviewed, which include acute/chronic rhinosinusitis, mucoceles, pseudocysts, retention cysts, and odontogenic diseases of the maxillary sinus. In addition, a treatment strategy will be proposed toward the management of these antral diseases. In the second part, anatomical variations of the maxillary sinus, f

[Factors associated with the outcome of maxillary distraction osteogenesis in cleft lip and palate patients].

Since distraction osteogenesis had been used in maxillofacial surgery in the 1990s,it has become an important option for treating maxillary hypoplasia in cleft lip and palate (CLP) patients. Numerous domestic and international studies reported that the prognosis of distraction osteogenesis might be affected by various factors, including the design of osteotomy line, type of distractor, advancement of maxillary bone and the age of patients, etc. Therefore, the present article reviewed possible impact factors that affect the effectiveness and outcomes of maxillary distraction osteogenesis of CLP
