[Cellular and tissue reactions of the mucous membrane of the maxillary sinus in the patients presenting with odontogenic aspergillous maxillary sinusitis].

The results of this morphological study of the mucous membrane of the maxillary sinuses in the patients presenting with the non-invasive fungal form of odontogenic sinusitis revealed the signs of granulematous inflammation. Epithelium underwent metaplasia into the single-row cubic or prismatic layer. The invasive form of fungal odontogenic sinusitis was characterized by allergic inflammation with intensive infiltration of maxillary sinus mucosa by antigen-representing and effector cells.

Effect of the anatomical patterns of maxillary sinus floor on the endodontic microsurgery plan of maxillary sinusitis of endodontic origin.

BACKGROUND: Due to the close relationship between maxillary posterior teeth and maxillary sinus floor (MSF), infection from teeth with endodontic disease may spread into maxillary sinus and cause the occurrence of maxillary sinusitis of endodontic origin (MSEO). Endodontic microsurgical intervention in infected teeth causing MSEO requires careful assessment and planning. This study evaluated the anatomical patterns of MSF using cone-beam computed tomography (CBCT), and further explored their influence on the endodontic microsurgery of MSEO. METHODS: A total of 333 CBCT scans were collected be…

Orbital apex syndrome associated with cranial nerve V neuritis complicating bacterial maxillary sinusitis.

Maxillary sinusitis is a common pathology of the paranasal sinuses, most frequently caused by bacterial infections. Although typically benign, it can occasionally lead to severe complications when the infectious process extends beyond the sinus cavity. One such rare but serious complication is orbital apex syndrome (OAS), also known as Jacod syndrome, characterized by dysfunction of multiple cranial nerves at the orbital apex. OAS may result from various etiologies, including infectious, traumatic, and neoplastic causes. Among infectious origins, bacterial maxillary sinusitis is an uncommon ye…

Chronic Maxillary Sinusitis Secondary to a Retained Wooden Foreign Body: A Case Report.

BACKGROUND: Retained wooden foreign bodies in the maxillary sinus, particularly with prolonged latency, pose significant diagnostic, and therapeutic challenges due to their low radiodensity and resemblance to fungal infections on imaging. This case highlights the role of electromagnetic navigation-guided endoscopic sinus surgery (ENG-ESS) in managing complex long-latency foreign bodies. CASE PRESENTATION: A 31-year-old man presented with recurrent right infraorbital abscesses and facial swelling for over a year and was initially misdiagnosed with fungal sinusitis. Conventional treatments, inc

Appropriate Implant Rehabilitation in Patients With Iatrogenic Oroantral Fistula and Odontogenic Maxillary Sinusitis.

Oroantral fistula (OAF) is the most common etiology for odontogenic maxillary sinusitis that can be caused by tooth extractions, failed maxillary sinus lifts, bone grafts, and poor positioning of dental implant fixtures. A 52-year-old man presented with an OAF and maxillary sinusitis after implant placement and bone grafting. The authors treated the patient with modified endoscopic sinus surgery to obtain OAF closure and provided dental implant placement procedures afterward. The authors also treated 8 other similar cases with favorable outcomes. In this study, the authors report the know-how …

Maxillary sinusitis after sinus lift due to Gemella morbillorum: antibiotic and surgical treatment.

Sinus lift is a predictable procedure for increasing alveolar bone height in the posterosuperior alveolar regions to allow oral prosthetic rehabilitation. Several complications have been documented in the literature and vary from sinus membrane perforation to maxillary rhinosinusitis. The authors present a case of Gemella morbillorum acute sinusitis after sinus lift surgery. The purpose of this report is to describe the surgical and pharmacological management of a patient allergic to penicillin.

Implementing guidelines on acute maxillary sinusitis in general practice--a randomized controlled trial.

BACKGROUND: Management of acute maxillary sinusitis (AMS) is not optimal; antibiotics are often prescribed for viral sinusitis, which leads to many problems including those with antimicrobial resistance. Guidelines have been proposed as a means to change the professional practices. OBJECTIVE: Our aim was to study whether a nationwide guidelines implementation programme has an effect on the management of AMS in primary care. METHODS: A multi-centre randomized controlled trial was conducted in 30 health centres (HCs) covering a population of 819 777 people from 1998 to 2002. The participating …

[Use of long-acting drainage (sino-ject) in treatment of chronic maxillary sinusitis].

The authors present the results of treatment of chronic maxillary sinusitis with punctures and with permanent drainage (sino-ject). The use of permanent drainage shortened the time of treatment and increased percentage of very good results of treatment. Presented method of permanent drainage of maxillary sinus avoid many punctures of sinus. The drainage of sinus allow for each day rinsing of sinus with physiological saline and with drugs (mucolitic, antibiotic) and also for bacteriological examination of sinus secretion. As in classical puncture of maxillary sinus the permanent drainage play a…

Transformation of mucosal secretory elements in chronic maxillary sinusitis after endoscopic sinus surgery.

To evaluate the secretory element transformation of maxillary sinus mucosa after endoscopic sinus surgery (ESS), I enrolled 20 patients. Five normal antral mucosae were used as controls, and 15 antral mucosae were investigated according to pathologic and secretory patterns. The post-ESS mucosa was evaluated at the 16th week, when gross recovery appeared. Specimens from the rear wall of the antrum were observed under a scanning electron microscope (x2,000). A significant decrease of goblet cells and increase of glands was illustrated in the sinusitis cases, especially the polypoid and purulent

Predictive value of nasal bacterial culture for etiological agents in acute maxillary sinusitis.

Nasal secretion, aspiration yield and lavage content from the sinus were studied for bacteria in 175 patients (247 sinuses) with acute maxillary sinusitis. The same pathogen was cultured from the nose and aspiration fluid in 91% of cases of acute purulent sinusitis. This indicated a significant predictive value of the nasal bacteriological culture for presence of pathogenic bacteria in the sinus in purulent cases. In cases with no growth of pathogens in the aspirate, the nasal culture showed pathogenic bacteria in about 50%. Examination of the aspiration fluid may occasionally give false negat…
