A 5 year old girl with an echinococcuscyst in the right orbit is reported. The final diagnosis was made by removal of the cyst. A second cyst was found in the liver. The epidemiology, clinical and diagnostic problems of echinococcosis are reviewed. Radical surgery is still the only reliable treatment. For inoperable cases chemotherapy with Mebendazol seems promising. Many problems of chemotherapy remain to be solved and Mebendazol therapy is still in an experimental stage.

Compatible and efficient drugs for the therapy, caused by protozoal- or helminthic-infections, are available today. Many of them are not registered in Austria. Metronidazole, Mebendazole and Praziquantel are particularly effective. Most protozoal-infections can be treated with metronidazole. Mebendazole can be used against numerous nematode-infections (Ascariasis, Trichuriasis, Oxyuriasis, Ancylostomiasis), Niclosamide against cestode-infections (Taeniasis, Hymenolepiasis, Diphyllobothriasis), whereas, Praziquantel is applied against trematode-infections.

The chemotherapeutic responses of three test nematodes, Nippostrongylus brasiliensis, Nematospiroides dubius and Ancylostoma ceylanicum to standard antihookworm drugs were assessed in order to select a suitable host-parasite system for the primary screening of potential antihookworm compounds. After reviewing the main causes of elephantiasis in tropical countries, the results of combined medical and surgical treatment of a case of lymphatic filariasis are reported. Medical treatment consists of a cycle of high-dose mebendazole (40 mg/kg/die) and subsequent cycles of diethylcarbamazine at a habitual dose (6 mg/kg/die). In the interval, the elephantiasic right lip was removed surgically. The overall clinical results was good. Noteworthy was an unwanted, leukopenising side-effect of high-dose mebendazole.

We describe a patient with human immunodeficiency virus type-1 (HIV) infection and alveolar echinococcosis (AE) with a focus on two messages. Despite being severely immunocompromised over years the patient exhibited a long-term asymptomatic course of AE. This is in clear contrast to reports describing accelerated courses of AE in immunocompromised patients. The patient had therapeutic mebendazole drug levels with only 1/10 of the normal drug dose. Studies relating to the treatment of human taeniasis in the Philippines using Atabrine, bithionol, and mebendazole are reviewed. Cure rates of 93.7% for Atabrine, 85%-100% for bithionol, and 84.62% for mebendazole are reported. Aside from these studies, recent experience in the treatment of Taenia saginata infections at the Department of Parasitology, College of Public Health, is also reported. Twenty patients referred to the department were treated, 3 with bithionol, 17 with praziquantel. Of the 17 patients treated with praziquantel, the study reported cure rates of 47-88.24%.

Several drugs are available for mass campaigns against enteric helminths. Mebendazole and pyrantel pamoate are quite effective against ascariasis and reasonably effective against the hookworms.
