Effect of non-invasive tear stability assessment on tear meniscus height.

PURPOSE: To investigate the effect of non-invasive tear stability assessment with forced eye opening on the lower tear meniscus. METHODS: Twenty-three eyes of 23 patients with aqueous-deficient dry eye and 23 eyes of 23 normal subjects were enrolled. All subjects underwent imaging with a Keratograph 5M equipped with a modified tear film scanning function. Lower tear meniscus images were captured, and tear meniscus height (TMH) was measured with an integrated ruler before and after non-invasive Keratograph break-up time (NIKBUT) measurements in each subject. Subjects were instructed to keep th…

Common peroneal nerve neuropraxia after arthroscopic inside-out lateral meniscus repair.

Common peroneal nerve injury caused by suture entrapment is a complication of arthroscopic inside-out lateral meniscus repair. In the literature, the treatment of this complication has been described as immediate reexploration. We present a case of recovery after late neurolysis performed 7 months postoperatively for a common peroneal nerve neuropraxia. In addition to describing return of function after late exploration, we discuss the management and prevention of this complication.

Septic arthritis following arthroscopic meniscus repair: a cluster of three cases.

Three cases of Staphylococcus epidermidis septic arthritis following inside-out arthroscopic meniscus repair within a 4-day period at the same facility are described. All three patients responded to surgical debridement and 4 to 6 weeks of intravenous antibiotics. In each instance, the meniscus and repair sutures were left intact; 12- to 38-month follow-up revealed no evidence of infection or meniscal symptoms. Epidemiological investigation implicated the meniscus repair cannulas as one of the few factors common to all three cases. Molecular typing of bacterial DNA revealed that two of the thr…

Arthroscopic intercruciate trans-septal all-inside repair for lateral bucket-handle meniscus tear: A current technique: Intercruciate trans-septal all-inside lateral buckle handle meniscus tear repair.

Lateral bucket-handle meniscus tears are challenging to reduce and repair due to limited visualization of the posterior compartment and the risk of iatrogenic injury. We present an arthroscopic workflow that combines standard anterior portals with an intercruciate trans-septal route to enhance visualization and working space across the posterior compartments of the knee. Key steps include safe creation of posteromedial and posterolateral apertures, protected posterior portals, and guided passage of a Wissinger rod to connect compartments prior to reduction and fixation. Reduction is performed …

Modified Arthroscopic Technique for Repair of Medial Meniscus Posterior Root Tear and Centralization of the Extruded Medial Meniscus Through a Double Transtibial Tunnel (Without Use of Accessory Ports).

Medial meniscus posterior root tears (MMPRTs) are significant injuries that compromise the biomechanical function of the knee, leading to meniscus extrusion and rapid progression of osteoarthritis if left untreated. While various arthroscopic repair techniques exist, persistent meniscus extrusion after repair remains a challenge. Centralization of the extruded meniscus has shown promise in improving repair outcomes and reducing arthritic progression. This Technical Note describes a modified arthroscopic technique for the repair of MMPRT and concomitant centralization of the extruded medial men…

Secretome from human placenta-derived mesenchymal stem cells repairs mechanically induced meniscus injury in mice by activating the proliferation and suppressing the apoptosis of endogenous meniscus progenitor cells.

BACKGROUND: Meniscus diseases present certain therapeutic limitations. Although meniscectomy is the primary treatment option for meniscus injury (MI), this approach may accelerate the development of osteoarthritis and other degenerative joint diseases, and its therapeutic efficacy remains controversial. While human mesenchymal stem cells (MSCs) have emerged as a promising treatment option for MI, particularly in promoting cell proliferation and preventing apoptosis, their effect on activating endogenous meniscus progenitor cells (MPCs) to ameliorate MI and the underlying mechanisms remain uncl…

An Update on Meniscus Tears and Lower Limb Alignment in Pediatric Sports Injuries.

The incidence of pediatric meniscal tears has increased in recent years, with complex injuries such as bucket-handle and discoid meniscus tears presenting unique challenges. It is important to provide an updated overview of the diagnosis and nonsurgical and surgical management of pediatric meniscal tears, emphasizing MRI interpretation, repair techniques, and the risks associated with total meniscectomy. In addition, the orthopaedic surgeon should be knowledgeable about the critical role of lower limb alignment in pediatric knee pathology, developmental changes in coronal plane alignment, and …

Advances in repair of non-discoid meniscus injuries in children: a narrative review.

Non-Discoid meniscus injuries in children and adolescents are generally less common than in adults. However, with the increasing number of children participating in physical exercise and the intensification of exercise intensity, the frequency of meniscus injuries is gradually rising. Currently, research on the diagnosis and treatment of meniscus tears in adults is quite advanced, and some progress has also been made in the research on the repair of meniscus injuries in children. Nevertheless, there is a lack of consensus regarding the repair of meniscus injuries in this population. This study…

Managing discoid lateral meniscus in children and adolescents: a review of recent advances.

Discoid lateral meniscus (DLM) is a structural anomaly characterized by an increased volume and altered collagen fibers, primarily affecting the lateral meniscus. Although its incidence varies, DLM is often present in the pediatric population and may present with acute or chronic knee symptoms, such as joint line pain, mechanical symptoms, and the inability to achieve terminal extension. Diagnosis is typically confirmed through magnetic resonance imaging and clinical evaluation. Histological studies have revealed disorganization and decreased collagen density in DLM, predisposing it to tears a…

A Ring-Shaped Lateral Meniscus in an Anterior Cruciate Ligament Reconstruction Surgery.

The ring-shaped meniscus is a rare variant of meniscus anomalies. We report a case of a young man who experienced right knee instability following a sports-related injury. Clinical examination suggested an anterior cruciate ligament (ACL) tear, which was confirmed through imaging studies. Intra-operatively, an incidental finding revealed the presence of a ring-shaped lateral meniscus. We discuss the diagnostic features of this uncommon abnormality.

[Medial discoid meniscus in a pediatric patient. Case report and review of the literature].

Discoid meniscus is a congenital morphological variant of the meniscus, which tends to occur more frequently in its lateral form than in the medial form. This anomaly is characterized by central hypertrophy of the meniscus and a larger diameter than the normal meniscus, resulting in an abnormal shape and greater coverage of the tibial plateau. The clinical presentation of this condition varies depending on the stability of the meniscus. In pediatric patients, in particular, it is common to experience progressive and atraumatic symptoms, such as pain and limited mobility. Diagnosis is based on …
