Mental status assessment in the elderly.

Mental status testing is an essential element of evaluating elderly patients. A structured examination often helps identify the etiology of symptoms that are responsible for changes in patient behavior, that lead to family stress, but that are otherwise not indicative of cognitive dysfunction. A structured mental status examination is also helpful in distinguishing cognitive changes caused by dementia, delirium, and depression. [The mental state of parkinsonian patients].

The systematic study of cognitive functions in parkinsonian patients has demonstrated specific disturbances of memory, visuo-spatial function and frontal lobe activity. Global impairment of intellectual functions is less common (it occurs in 15 to 20 percent of the cases) and is observed mainly in elderly patients. Dementia differs in its neuropsychological set up from that of Alzheimer's disease. The most frequent psychiatric disorders are depressive state, signs of anxiety and side-effects of drugs, including visual hallucinations and confusion. This means that some of these drugs (e.g. anti

Mentally retarded adults and their siblings.

The relationships of 35 community-based mildly mentally retarded adults and their siblings were examined through participant-observation techniques over an 18-month period. Five kinds of sibling relationships were identified based on the independent ranking of each sibling pair on a continuum that reflected three major relationship dimensions: warmth, frequency of contact, and degree of involvement. These relationships were described as were some general patterns that seem to have the greatest impact on relations between retarded adults and their siblings.

Risk factors for mental retardation.

Risk factors for mental retardation were studied prospectively in 12 000 children born in northern Finland in 1966 and followed to the age of 14 years. The number of untraced children was less than 2 per 1000. Altogether 326 children had an IQ less than 86, and the incidence of severe retardation (IQ less than 50) was especially high. An incidence figure for children with mental retardation, a separate figure for healthy children, and also the death rate were calculated for each disease. Only in the cases of Down's syndrome and some hereditary diseases were all the exposed children mentally re

Hydrocephalus and mental retardation in craniosynostosis.

We prospectively studied craniosynostosis, regardless of neurologic status, by cranial computed tomography or psychometric testing in 56 children. None of the 27 children with simple craniosynostosis (single or multiple suture involvement) had evidence of hydrocephalus on CT scan. Of the 24 patients with simple craniosynostosis who underwent psychometric testing, 17 were of average intelligence; six were in the low average range. The single mentally retarded child had a history of severe perinatal asphyxia. Hydrocephalus occurred more frequently (five of 23 cases) in children with complex cran

Altered mental status in the elderly.

Misdiagnosis of treatable dementia in the aged is costly to society and the family, and is unmeasurable in personal tragedy. The 4 Ds of the elderly--dementia, depression, delirium, and delusion--are discussed. Family physicians are encouraged to seek out treatable disease.

Employability of the mentally disabled in the 1970s.

Of 156 discharged patients, the forty who succeeded in open employment had shown before they left hospital a combination of significantly less clinical disability, less abnormal behaviour, better work performance and better relationships with other people. We specify the approximate levels of these variables that were required to get and hold a job in the 1970s.
