Safety and efficacy of topical mitomycin C in myringotomy patency.

OBJECTIVE: To develop an alternative method for prolonged middle ear ventilation using topical mitomycin C. STUDY DESIGN AND SETTING: Twenty guinea pigs with normal ears had bilateral myringotomies performed using the argon laser. After myringotomy, either mitomycin C (0.4 mg/mL) or saline pledgets were applied topically. Monitoring consisted of otomicroscopy and distortion-product otoacoustic emissions. RESULTS: Before myringotomy, all tympanic membranes were intact, and distortion-product otoacoustic emissions were measurable. After myringotomy, none (0%) of the saline-treated myringotomie…

Grommets (ventilation tubes) for recurrent acute otitis media in children.

BACKGROUND: Acute suppurative otitis media is one of the most common infectious diseases in childhood. Recurrent acute otitis media is defined for the purposes of this review as either three or more acute infections of the middle ear cleft in a six-month period, or at least four episodes in a year. Strategies for managing the condition include the assessment and modification of risk factors where possible, repeated courses of antibiotics for each new infection, antibiotic prophylaxis and the insertion of ventilation tubes (grommets). OBJECTIVES: To establish whether ventilation tube insertion

Do middle ear muscles trigger attacks of Menière's disease?

OBJECTIVE: Current surgical methods for treating Ménière's disease (MD), e.g. endolymphatic sac surgery, mastoidectomy or insertion of middle ear ventilation tubes, only improve vertigo and fail to influence the hearing threshold. In this retrospective study we examined the long-term effects of tenotomy on the symptoms of MD. MATERIAL AND METHODS: A total of 45 patients suffering from definite MD underwent middle ear muscle tenotomy, a treatment that is very rarely considered nowadays. Of these 45 patients, 20 were observed postoperatively for a period of > 2 years. The disease was scored pre…

[The errors of the operative technique as a cause of unsuccessful tympanoplasty. Part II].

This study is based on the author's personal experience in a middle ear reconstructions. Errors can occur at each surgical stage--tympanic membrane and chain reconstructions, mobilisation of the footplate. At any moment the surgeon can gain or loose the dB. The operating technique is the main factor. A proper operative technic includes a good operating approach and excellent hemostasis. The tympanic membrane, should be as thin as possible, exactly and carefully dissected. Care should be taken to avoid any damage to the fascia graft fibers. Bad or unsure material should be discarded. The graft

Outcome of Eustachian Tube Balloon Dilation in Children: A Systematic Review.

OBJECTIVE: Eustachian tube dysfunction (ETD) is a chronic entity that has been historically managed with adenoidectomy and ventilation tube insertion. Recently, balloon dilation of the eustachian tube has shown promising results in recalcitrant eustachian tube dysfunction. We reviewed the literature to determine the outcome of eustachian tube balloon dilation in children. METHODS: A literature search was conducted for the period from 1990 to 2020 by searching several databases over a 1-month period (January 2021) according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses

Middle ear disease requiring myringotomy in the first two years after tracheotomy in the pediatric population.

OBJECTIVES: Risk factors for middle ear disease necessitating myringotomy with tympanostomy tube placement after a tracheostomy have not been thoroughly explored. This study investigates the incidence and risk factors for ear tube placement in pediatric patients with a tracheostomy. METHODS: Pediatric patients under age 18 who underwent tracheotomy between 2002 and 2010 at two institutions were identified. Patients were excluded if they had undergone myringotomy prior to, or at the same time as, the tracheotomy, or did not have at least two years of follow-up clinic visits. The presence of ot

Middle ear secretions following spontaneous CSF leak repair may represent effusion rather than CSF leak.

OBJECTIVE: To characterize middle ear (ME) effusion still present 2 months after repair surgery for spontaneous cerebrospinal fluid (CSF) leak via the temporal bone (TB). STUDY DESIGN: A retrospective chart review (2011-2022). SETTING: Tertiary referral academic center. SUBJECTS AND METHODS: All patients with persistent ME effusion at 2 months after surgery were included in this study. The indication for surgery for spontaneous TB CSF leak was an active CSF leak with tegmen dehiscence. The presence of effusion was established by findings on microscopic otoscopy aided by tympanocentesis.ME w

Effectiveness and Safety of Advanced Audiology-Led Triage in Pediatric Otolaryngology Services.

OBJECTIVES: Expansion of the scopes of practice of allied health practitioners has the potential to improve the efficiency and cost-effectiveness of healthcare, given the identified shortages in medical personnel. Despite numerous examples in other allied health disciplines, this has yet to be applied to pediatric Audiology. This study aimed to investigate the effectiveness and safety of using audiologists with advanced training to independently triage children referred to otolaryngology (ORL) services, and compare the subsequent use of specialist resources, and postoperative grommet care to a

Surgical treatments for otitis media with effusion: a systematic review.

BACKGROUND AND OBJECTIVE: The near universality of otitis media with effusion (OME) in children makes a comparative review of treatment modalities important. This study's objective was to compare the effectiveness of surgical strategies currently used for managing OME. METHODS: We identified 3 recent systematic reviews and searched 4 major electronic databases. Eligible studies included randomized controlled trials, nonrandomized trials, and cohort studies that compared myringotomy, adenoidectomy, tympanostomy tubes (tubes), and watchful waiting. Using established criteria, pairs of reviewers
