Trends in mental health outcomes and combat exposure among US marines returning from Iraq, Afghanistan or other deployments, 2004-13.

BACKGROUND: As of 2015, more than 2.7 million US military Veterans have served in support of the Global War on Terror. The negative sequelae associated with deployment stressors and related traumas are well-documented. Although data on mental health issues are routinely collected from service members returning from deployment, these data have not been made publicly available, leaving researchers and clinicians to rely on convenience samples, outdated studies and small sample sizes. METHODS: Population-based data of US Marines returning from deployment between 2004 and 2013 were analyzed, usin

Deployment and PTSD in the female combat veteran: a systematic review.

BACKGROUND: The number of females serving in the Gulf War has risen and continues to be a rapidly growing group. Females occupy a wide range of roles and face a myriad of challenges as they serve alongside their male counterparts in almost every role. Females are also facing redeployment, multiple deployments, and/or extended deployments. The stressors of war and deployments may be putting them at risk for posttraumatic stress disorder (PTSD). If left untreated, PTSD can have devastating and debilitating consequences for veteran's functioning and relationships, their families, and society. OB

Risk Factors for Postpartum Depression in Active Duty Women.

INTRODUCTION: Postpartum depression (PPD) is a common perinatal complication. Risk factors previously found to correlate with PPD in civilians include prenatal depression, childcare stress, limited social support, difficult infant temperament, and maternity blues. Previously identified risk factors in military spouses include spouse deployment/redeployment cycles. It is unclear if these previously identified risk factors are also a risk factor for AD women or if the additional stressors associated with being on active duty (AD) are risk factors for PPD. The purpose of this review is to determi

Association of Child Abuse Exposure With Suicidal Ideation, Suicide Plans, and Suicide Attempts in Military Personnel and the General Population in Canada.

IMPORTANCE: Recent evidence indicates a high prevalence of child abuse exposure in modern US veterans, which may explain in part their higher likelihood of suicide relative to civilians. However, the relationship between child abuse exposure and suicide-related outcomes in military personnel relative to civilians is unknown. Furthermore, the associations among deployment-related trauma, child abuse exposure, and suicide-related outcomes in military personnel have not been examined. OBJECTIVES: To determine whether child abuse exposure is more prevalent in Canadian Armed Forces (CAF) personnel

Post deployment care for returning combat veterans.

Since September 11, 2001, 2.4 million military personnel have deployed to Iraq and Afghanistan. To date, roughly 1.44 million have separated from the military and approximately 772,000 of these veterans have used VA health care. Combat deployments impact the physical, psychological, and social health of veterans. Given that many veterans are receiving care from non-VA providers, it is important that all community health care workers be familiar with the unique health care needs of this patient population, which include injuries associated with blast exposures (including mild traumatic brain in

New Onset of Fibromyalgia After Exposure to a Combat Environment: A Longitudinal Cohort Study.

OBJECTIVE: Traumatic life events are hypothesized to be triggers for the onset of fibromyalgia. Posttraumatic stress disorder (PTSD) is a common comorbidity of fibromyalgia. However, limited prospective data are available on the development of fibromyalgia after exposure to high-magnitude stress. METHODS: This longitudinal cohort study of US military service members (N = 1,761) assessed fibromyalgia and PTSD before and upon return from combat deployment. Fibromyalgia was assessed with the 2011 questionnaire modification of the 2010 American College of Rheumatology preliminary diagnostic crite

Typhoid fever cases in the U.S. military.

BACKGROUND: Salmonella enterica, serovar Typhi (S. Typhi), a causative agent of enteric fever (typhoid fever), predominately affects populations in developing regions with poor access to clean food and water. In addition, travelers to these regions are at risk of exposure. METHODS: We report the epidemiological characteristics of S. Typhi cases among active duty United States military personnel from 1998 to 2011 using data obtained from the Defense Medical Surveillance System. Cases were identified based on International Classification for Disease Ninth Edition - Clinical Modification codes.

[Prevention of the development and importation of infections in members of the Army of the Czech Republic participating in foreign missions].

Over the last years, Czech soldiers became involved in several foreign missions. In 2001-2003 more than 4000 persons participated in such missions. Since their activities are typically associated with high health risks including that of acquiring infectious diseases, the significance of prevention is underlined. Preventive antiepidemic measures taken in soldiers of the Army of the Czech Republic before their deployment and upon return home are described. Prior to being dispatched on a foreign mission any soldier is screened at the department for occupational diseases at the military hospital.

Anaphylaxis Secondary to Aeroallergen Immunotherapy in a Military Member.

We present the case of a 43-year-old active-duty US Air Force service member who experienced an anaphylactic reaction during routine aeroallergen immunotherapy. The patient had a long-standing history of perennial and seasonal allergic rhinoconjunctivitis, with confirmed sensitization to multiple environmental allergens through skin prick and serum-specific IgE testing. He tolerated build-up immunotherapy until reaching the 0.4 mL injections from the maintenance 1:1 volume/volume (1:100 weight/volume) vials, at which point he developed an anaphylactic reaction characterized by vomiting, hypote

Menstrual suppression for combat operations: advantages of oral contraceptive pills.

BACKGROUND: increasing numbers of women are deployed to austere settings in which menstruation may impose logistical challenges. Minimal data exists about the use of oral contraceptive pills (OCPs) for menstrual suppression in this population. Post-deployment survey was undertaken to establish prevalence of continuous OCP use, perceived barriers, and associations with menstrual burden in a military population within the austere environment. METHODS: voluntary and anonymous 44-item questionnaire. RESULTS: of 500 women, 78% (n = 390) had personal experience using OCPs and 66% (n = 330) desired

Reproductive and Female Health in the Australian Defence Force 2002-2023: A Systematic Review and Synthesis.

INTRODUCTION: Military service involves potential exposure to a range of physical, chemical, and biological hazards that could impact reproductive health. Females represent 20% of Australian Defence Force (ADF) members and, like many military services, the ADF is prioritizing their recruitment and retention. However, given that females have different physiology and anthropometry, specific female occupational research is essential. This systematic review aimed to retrieve and summarize the findings of research involving currently serving ADF personnel with outcomes relevant to male or female re

Pre-Existing Audiometric Hearing Loss is a Predictor of Significant Threshold Shift Following Injury During Combat Deployment.

OBJECTIVES: Military personnel are exposed to multiple risk factors for hearing loss, particularly on the battlefield. The objective of this study was to determine whether pre-existing hearing loss predicted hearing threshold shift in male U.S. military personnel following injury during combat deployment. DESIGN: This was a retrospective cohort study with 1573 male military personnel physically injured in Operations Enduring and Iraqi Freedom between 2004 and 2012. Audiograms before and after injury were analyzed and used to calculate significant threshold shift (STS), defined as a 30 dB or g

Development of a multiplex serological assay to detect immunoreactivity against a novel Bartonella species in military working dogs.

Bartonella pose a significant health risk to military working dogs (MWDs), and these zoonotic organisms may also cause disease in humans. According to the U.S. Army zoonotic disease surveillance program, there was a 47.4% seroprevalence for Bartonella spp. among feral dogs in Iraq. This surveillance program identified infection with a novel species Candidatus Bartonella merieuxii (CBm) in 37% of the dogs by whole blood Bartonella DNA amplification and sequencing for the gltA and rpoB genes and intergenic spacer region. This bacterium has not been successfully cultured to date. The objectives o

Associations of contextual risk and protective factors with fathers' parenting practices in the postdeployment environment.

Deployment separation and reunifications are salient contexts that directly impact effective family functioning and parenting for military fathers. Yet, we know very little about determinants of postdeployed father involvement and effective parenting. The present study examined hypothesized risk and protective factors of observed parenting for 282 postdeployed fathers who served in the National Guard/Reserves. Preintervention data were employed from fathers participating in the After Deployment, Adaptive Parenting Tools randomized control trial. Parenting practices were obtained from direct ob

Effects from using a race-neutral prediction equation for interpreting spirometry in an active-duty military population.

INTRODUCTION: The purpose of our analysis was to study the clinical and occupational consequences from switching to a "race-neutral" spirometry reference equation for active-duty service members (ADSMs). METHODS: We hypothesized that switching to a "race-neutral" equation would affect eligibility for deployment and improve clinical accuracy. We tested this hypothesis using data from a prospective cohort study (STAMPEDE II) of ADSMs screened using spirometry pre-deployment. Prevalence of abnormality and z-scores were compared using the "race-neutral" GLI-Global (GLI-G) versus the original GLI
