Multidrug-resistant Trichosporon: an unusual fungal sepsis in preterm neonates.

We report a cluster of three extremely-low birth weight (ELBW), preterm neonates who developed late-onset sepsis (LOS) by Trichosporon asahii within a span of 1 week period. Two of these cases had the initial diagnosis of respiratory distress syndrome and the third one was admitted for low birth weight and prematurity. Initial sepsis screen was negative and blood culture was sterile in all. Late-onset sepsis was developed after the first week of life and the presenting features were lethargy, feeding intolerance, bleeding manifestations, positive sepsis screen and severe thrombocytopaenia. The

Neonatal meningitis and sepsis by Chryseobacterium indologenes: a rare and resistant bacterium.

Early neonatal meningitis with non-fermenting Gram negative bacilli (NFGNB) is rare, and whenever it occurrs, inanimate environment is usually implicated as the source. The authors report a case of neonatal meningitis and sepsis with Chryseobacterium indologenes, a rare non fermenting Gram negative bacterium with unusual antimicrobial susceptibility. Despite resistance to all the beta lactams, carbapenems and aminoglycosides, therapy with ciprofloxacin led to a favorable outcome.

Management of neonates with suspected or proven early-onset bacterial sepsis.

With improved obstetrical management and evidence-based use of intrapartum antimicrobial therapy, early-onset neonatal sepsis is becoming less frequent. However, early-onset sepsis remains one of the most common causes of neonatal morbidity and mortality in the preterm population. The identification of neonates at risk for early-onset sepsis is frequently based on a constellation of perinatal risk factors that are neither sensitive nor specific. Furthermore, diagnostic tests for neonatal sepsis have a poor positive predictive accuracy. As a result, clinicians often treat well-appearing infants…

The impact of environmental and genetic factors on neonatal late-onset sepsis.

OBJECTIVE: To assess the genetic contribution to late-onset sepsis in twins in the newborn intensive care unit. STUDY DESIGN: A retrospective cohort analysis of twins born from 1994 to 2009 was performed on data collected from the newborn intensive care units at Yale University and the University of Connecticut. Sepsis concordance rates were compared between monozygotic and dizygotic twins. Mixed-effects logistic regression analysis was performed to determine the impact of selected nongenetic factors on late-onset sepsis. The influence of additive genetic and common and residual environmental

Using proteomics in perinatal and neonatal sepsis: hopes and challenges for the future.

PURPOSE OF REVIEW: Particularities of the fetal immune response to infection cause a heightened inflammatory state that acts synergistically … onitis and funisitis. Presence of S100A12 and S100A8 in amniotic fluid is predictive of early-onset neonatal sepsis and poor neurodevelopmental outcome. SUMMARY: Presence of amniotic fluid proteomic biomarkers of inflammation is associated with increased inflammatory status of the fetus at birth. Future challenges are to find biomarkers that provide insight into …

Effect of neonatal sepsis on the development of allergies and asthma in later childhood.

BACKGROUND: Exposure to large amounts of endotoxins and other bacterial products in early childhood may protect against the development of allergic diseases later in childhood. The aim of this study was to investigate the effects of neonatal sepsis on subsequent development of asthma, allergic rhinitis, and atopic dermatitis in children. METHODS: We recruited 85 children (mean age 48.67 +/- 12.88 months) who had been hospitalized for sepsis in their neonatal period and their siblings (n = 85) as controls (mean age 61.81 +/- 21.34 months) to investigate the prevalences of asthma, atopic dermat…

Strategies for prevention of nosocomial sepsis in the neonatal intensive care unit.

PURPOSE OF REVIEW: Infants hospitalized in the neonatal intensive care unit, particularly preterm infants, have very high rates of nosocomial sepsis (also referred to as late onset sepsis or healthcare-associated sepsis). Today's preventive strategies for nosocomial sepsis focus on augmenting the immunologic and functional immaturities of premature infants and ameliorating the risks of extrinsic factors by the use of prophylactic antibiotics and best clinical practices. RECENT FINDINGS: Topical emollients improved neonatal skin condition, but were associated with an increased risk of nosocomi

[Evaluation of alternative hemoculture tests for the diagnosis of early neonatal sepsis].

In order to evaluate the diagnostic usefulness of coadjuvant tests such as external auditory canal swab culture and cultures from nasopharyngeal and gastric aspirates, and to determine the incidence and etiology of early neonatal sepsis (ENS) at our Unit, 90 newborn cases whose mothers experienced premature rupture of the membranes (PRM) were studied prospectively. Although a firm diagnosis requires positive blood cultures, the difficulty in recovering microorganisms and the trauma induced by sample collection in the baby justify the search for alternative diagnostic tests. Out of 2293 childbi…

[Streptococcus bovis meningitis and sepsis in neonates: report of two cases].

Streptococcus bovis is a veterinary organism rarely previously reported in the neonatal infection. Two cases are presented of neonatal infection by this organism; one case manifested with meningitis and the other, with sepsis. Seizure and respiratory distress were the symptoms found in these two babies, respectively. Following adequate treatment, recovery was complete, and the infants enjoyed healthy lives post-discharge. S. bovis is always sensitive to penicillin, but resistant strain has been reported recently. It is reasonable to treat infant patients infected by this organism with a combin

Immunoglobulin therapy for neonatal sepsis: an overview of animal and clinical studies.

Bacteria such as Escherichia coli and group B streptococci are common neonatal pathogens. Neonates infected perinatally often develop overwhelming sepsis which may rapidly progress to shock and death in just a few hours. This fulminant course suggests that immunity to these bacteria is deficient. Studies from several laboratories have shown that antibody is important in immunity to group B streptococcus. In vitro studies have demonstrated that efficient phagocytosis and killing of these bacteria by neutrophils or monocytes requires opsonic antibody. Many premature babies have decreased amounts

Prophylactic hydrocortisone and the risk of sepsis in neonates born extremely preterm.

UNLABELLED: Bronchopulmonary dysplasia (BPD) is a serious complication of extreme prematurity and has few treatment options. The postnatal use of steroids to prevent BPD remains controversial, but prophylactic low-dose hydrocortisone (HC) has been shown to improve survival without BPD. However, an increased risk of late-onset sepsis (LOS) was also reported in extremely preterm neonates exposed to prophylactic HC treatment. Because its causal link remains unclear, our objective was to assess the effect of prophylactic HC exposure on LOS risk, adjusted for perinatal risk factors of LOS. We re-an
