Utilizing transmission electron microscopy we have found multiple cilia with a 9 + 2 microtubular pattern (the same type found in the bronchial tree) in the renal tubules of four adults with the nephrotic syndrome. Each patient had a different pathologic diagnosis: amyloidosis, lipoid nephrosis, membranoproliferative glomerulonephritis, and focal segmental glomerulosclerosis. Previously, multiple renal tubular cilia with 9 + 2 architecture had been reported in patients with Burnett's syndrome, systemic lupus erythematosus, and congenital nephrotic syndrome. Additionally, we found multiple cili

A retrospective analysis of all renal biopsies (961) performed in two regional hospitals in Hong Kong during 1977-1985 revealed that IgA nephropathy was the most frequently encountered glomerulopathy. Lipoid nephrosis (minimal change nephrotic syndrome) remained the commonest cause of nephrotic syndrome in children. The frequencies of mesangiocapillary glomerulonephritis, focal glomerulosclerosis, and idiopathic membranous nephropathy were lower than in other populations. Membranous nephropathy was frequently associated with hepatitis B virus antigenemia, especially in children. Other chronic

In minimal-change steroid-sensitive nephrotic syndrome with selective proteinuria, mesangial IgA deposition at immunofluorescence is a very rare finding which has been previously considered a pure coincidence. Two patients, aged 6 and 14 years, respectively, with a steroid-sensitive but frequently relapsing nephrotic syndrome and highly selective proteinuria, exhibited minor glomerular alterations at light microscopy and an immunofluorescence deposition of predominant and diffuse mesangial IgA, confirmed by electron microscopy as dense deposits. The observed syndrome, that is surprisingly iden

In 2 patients with the nephrotic syndrome, unsuspected solid tumors were found. One was a small cell lung carcinoma, accompanied with the syndrome of inappropriate ADH secretion. The other was a cancer of the breast with lymph node and bone metastases. In both, renal biopsy showed minimal change disease without immune complex deposits. There are only 14 other reported cases of paraneoplastic lipoid nephrosis complicating solid tumors. Such cases lead to the discussion on the respective roles of tumor cell gene product(s) inducing proteinuria and of lymphokine secretion by lymphocytes directed

Basing on the general pathologic definition "glomerulonephritis" the authors analyse its modern morphologic classifications and arrive at a conclusion that minimal changes (lipoid nephrosis), membranous glomerulopathy and focal segmental glomerular hyalinosis should be excluded from the group of glomerulonephritis. These diseases form the group of primary nephrotic syndrome. Glomerulonephritis group is represented by the following morphologic forms: proliferative (endo- and extracapillary, mesangioproliferative and mesangiocapillary) and sclerosing (fibroplastic) glomerulonephritis. Each of th

Minimal change nephropathy (MCN) has been associated with a wide variety of abnormalities in the immune response of affected individuals. Many of these may be a consequence of the renal lesion, rather than a cause thereof. This article reviews some of these immunological phenomena and concentrates in particular on the imbalance between type 1 and type 2 cytokine responses, suggesting that there are plausible mechanisms whereby a type 2 cytokine bias could directly or indirectly cause MCN.

The history of nephrotic syndrome can go back to beginning of the era. In old Alexandria, urine was conceived to be produced in kidneys. In 1827, Bright classified diseases with proteinuria into three categories, and lipoid nephrosis seemed to be included in group 1. Mechanisms of urinary protein leakage have been in great controvesies, and nowadays defects of slit-membrane protein are supposed to play important roles. In the middle of the 20th centuries, electron microscopy and renal biopsy technique were adopted in practical use. Since then, the treatment and research of nephrotic syndrome h

Management of idiopathic nephrotic syndrome in children is based on a series of clinical trials. The trial by Sinha and colleagues in this issue is 1 of many needed to improve the evidence base for induction and maintenance therapies in this population. While key questions remain about identifying the appropriate therapy for each patient, clinical trials provide an opportunity to extend evidence-based practice that minimizes toxicity and optimizes patient health.
