Perihematomal Edema After Intracerebral Hemorrhage: An Update on Pathogenesis, Risk Factors, and Therapeutic Advances.

Intracerebral hemorrhage (ICH) has one of the worst prognoses among patients with stroke. Surgical measures have been adopted to relieve the mass effect of the hematoma, and developing targeted therapy against secondary brain injury (SBI) after ICH is equally essential. Numerous preclinical and clinical studies have demonstrated that perihematomal edema (PHE) is a quantifiable marker of SBI after ICH and is associated with a poor prognosis. Thus, PHE has been considered a promising therapeutic target for ICH. However, the findings derived from existing studies on PHE are disparate and unclear.

Essential Neuroscience in Immunology.

The field of immunology is principally focused on the molecular mechanisms by which hematopoietic cells initiate and maintain innate and adaptive immunity. That cornerstone of attention has been expanded by recent discoveries that neuronal signals occupy a critical regulatory niche in immunity. The discovery is that neuronal circuits operating reflexively regulate innate and adaptive immunity. One particularly well-characterized circuit regulating innate immunity, the inflammatory reflex, is dependent upon action potentials transmitted to the reticuloendothelial system via the vagus and spleni

Blood-induced joint disease: the pathophysiology of hemophilic arthropathy.

Arthropathy is a frequent and serious complication of repeated joint bleeding in patients with hemophilia, resulting in pain, deformity, and disability. Although the pathogenesis of hemophilic arthropathy has not been fully elucidated, it appears to have similarities with the degenerative joint damage that occurs in osteoarthritis and the inflammatory processes associated with rheumatoid arthritis. This article reviews the potential actions of various blood constituents on joint components that culminate in the development of hemophilic arthropathy.

Neurogenic mechanisms in rhinosinusitis.

Nasal sensory nerve stimulation leads to sensations of pain and congestion and nociceptive nerve axon response-mediated release of substance P that stimulates glandular secretion as an immediate-acting protective mucosal defense. Recruited parasympathetic reflexes cause submucosal gland secretion via muscarinic M3 receptors. Parasympathetic reflexes, sneezing, and other avoidance behaviors rapidly clear the upper airway of offending agents while protecting the lower airways. Dysfunction contributes to allergic, infectious, and other nonallergic rhinitides and possibly sinusitis. Sympathetic ar

Platelets and polymorphonuclear leukocytes in experimental ocular inflammation in the rabbit eye.

Accumulation of 111indium-oxine (111In)-labelled platelets and the kinetics of 111In-labelled polymorphonuclear leukocytes (PMNLs) were studied in the anterior eye during neurogenic inflammation (induced by topical neutral formaldehyde) or after paracentesis in the rabbit, after formaldehyde irritation, 40 times more platelets were found in the aqueous humor 60 min later than in the control eyes and 400 times more after paracentesis. Platelets were found to be increased in the ciliary body, but not in the iris or choroid. Under light microscopy, some of the blood vessels in the ciliary process…

Engineered Chitosan Nanoparticle Platform Enables Controlled Ibuprofen Delivery and Coordinated Neuroimmune Regulation in a Human Microphysiological Model.

Controlled delivery of anti-inflammatory agents using bioactive nanomaterials offers a strategy to achieve localized and temporally regulated modulation of inflammatory and neurogenic signaling. Here, we engineered an adsorption-based chitosan nanoparticle platform for controlled Ibuprofen delivery to coordinate inflammatory and neurogenic responses during the acute postoperative phase. The formulation was designed to generate an initial burst release followed by sustained drug delivery while leveraging the intrinsic immunomodulatory properties of chitosan. Therapeutic performance was evaluate

Urine resazurin reduction ratio as a biomarker of urinary tract infection in people with neurogenic bladder: A first in human study.

Among people with neurogenic lower urinary tract dysfunction (NLUTD), urinary tract infection (UTI) is the most common infection and a leading cause of morbidity. UTI diagnosis is based on urinary symptoms, bladder inflammation (urinalysis (UA)), and bacterial growth (standard urine culture (SUC)), the latter two assessments taking approximately 24h and 48-72h, respectively. Here, urine resazurin reduction ratio (uRRR), which is a measure of nicotinamide adenine dinucleotide hydrogen conversion, a bioproduct of bacterial metabolism, is assessed as a point-of-care biomarker of UTI with time to

Serum calcitonin gene-related peptide and neuropeptide Y levels in seborrheic dermatitis: comparison with healthy controls and correlation with disease severity.

BACKGROUND: Seborrheic dermatitis (SD) is a chronic inflammatory skin disorder influenced by factors such as Malassezia colonization, sebaceous gland activity, immune dysregulation, and neurogenic inflammation. Neuropeptides like calcitonin gene-related peptide (CGRP) and neuropeptide Y (NPY) can modulate disease activity, while psychological stress may play a role in disease onset and flare-ups. OBJECTIVES: To evaluate serum CGRP and NPY levels in patients with SD, investigate their relationship with disease severity, and explore potential links with perceived stress. METHODS: In this cross…

Plasma extravasation in the skin and pelvic organs evoked by antidromic stimulation of the lumbosacral dorsal roots of the rat.

Electrical stimulation of the distal stump of cut peripheral nerves is a commonly accepted way to evoke neurogenic inflammation. Nevertheless, the modulatory effect of biogenic amines and vasoactive peptides released from efferent fibres can be excluded only if the dorsal roots are stimulated. The present study was focussed to investigate plasma extravasation in the appropriate skin and mucosal areas as well as in the genito-urinary organs in response to antidromic stimulation of the lumbar and sacral dorsal roots of the rat. Plasma extravasation was detected by quantitative measurement of the…

Clinical effects of nedocromil sodium on challenges invoking neuronal mechanisms and on virally induced symptoms.

Although sensory nerve activity may be important to the human airway in numerous possible ways, the relevance of "neurogenic inflammation" to the onset and development of asthma is unknown. However, several of the symptoms of asthma (bronchoconstriction, cough, and dyspnea) have a neuronal component that can be modeled in the laboratory by various stimuli that are thought to invoke sensory nerve activation. Nedocromil sodium is highly effective against bronchoconstriction induced by bradykinin, the tachykinins substance P and neurokinin A, and sulfur dioxide and metabisulfite. The results for …

Neuroimmune pharmacotherapy across the skin-brain axis: mechanisms, therapeutic targets, and AI-enabled precision approaches.

The skin-brain axis is a bidirectional neuroimmune network linking cutaneous inflammation with central neuroinflammation, stress signaling, and behavioral disturbances. This review summarizes the mechanisms underlying skin-brain communication and evaluates current and emerging pharmacological strategies targeting this axis in inflammatory skin disorders. We synthesized contemporary experimental, translational, and clinical evidence on neuroimmune signaling across the skin-brain axis, with emphasis on sensory neuron-immune cell interactions, neurogenic inflammation, glial activation, pharmacolo…

Primary mucinous adenocarcinoma in a defunctionalized urinary bladder: a case report.

INTRODUCTION: Malignancies are rare in defunctionalized bladders and are thought to arise from metaplasia secondary to chronic inflammation. Transitional cell and squamous cell carcinomas are the most common but there are three reported cases of mucinous adenocarcinoma. CASE PRESENTATION: We report a 57-year-old Caucasian man presenting with penile discharge for 30 years following ileal conduit surgery for neurogenic bladder, and who was found to have primary mucinous adenocarcinoma of his defunctionalized bladder. CONCLUSION: Although urinary diversion without cystectomy is less common in c

The connection between endometriosis and secondary dysmenorrhea.

Endometriosis (EMS) is a prevalent gynecological condition characterized by the presence of endometrial tissue outside the uterus, often leading to secondary dysmenorrhea (SD), chronic pelvic pain and infertility. This review explores the intricate connection between EMS- associated pain and SD, focusing on the pathophysiological mechanisms underlying dysmenorrhea in EMS. Key contributors to pain include inflammation, aberrant immune responses, neurogenic inflammation, peritoneal irritation, peripheral sensitization, central sensitization and cross-organ sensitization. Understanding the pain p…

Attenuation of acid induced oesophagitis in VR-1 deficient mice.

BACKGROUND AND AIMS: Activation of the vanilloid receptor subtype 1 (VR-1) results in release of proinflammatory peptides which initiate an inflammatory cascade known as neurogenic inflammation. We investigated its role in an acute model of surgically induced oesophagitis. METHODS: Oesophagitis was induced by pyloric ligation in wild-type and VR-1 deficient mice. A subset of animals were administered the VR-1 antagonist capsazepine, famotidine, or omeprazole one hour before surgery. Five hours after surgery, myeloperoxidase activity (MPO), histological damage scores, intragastric pH, and immu…

Neutral endopeptidase (EC 3.4.24.11) downregulates the onset of intestinal inflammation in the nematode infected mouse.

BACKGROUND AND AIMS: Substance P (SP) release from sensory nerves induces neurogenic inflammation. Neutral endopeptidase (NEP) degrades SP, thereby limiting its proinflammatory effects. Intestinal inflammation following Trichinella spiralis infection markedly downregulates NEP, resulting in diminished SP degradation, with unknown functional consequences. We hypothesised that diminished expression of NEP would exacerbate T spiralis induced enteritis. METHODS: NEP knockout (NEP-/-) and wild-type (NEP+/+) mice were infected with T spiralis and studied at 6, 12, 24, and 48 hours post infection (P…

[Neurogenic claudication and claudication by arteritis of the lower limbs].

Intermittent claudication of neurogenic origin can be traced to three basic causes: --either a chronic circulatory deficiency in the arteries leading to the spinal cord, whether these arteries be the site of an atheroma of the ostia, an inflammation or a loco-regional compression. In such cases, the claudication is painless which differentiates it from the arteritis claudication of the lower limbs. --or to a compression of the roots of the cauda equina and to a lesser degree of the spinal cord, through a narrow rachidian canal that is hereditary or acquired, and relative or absolute. --or fina
