Triplopia: thirteen patients from a neurology inpatient service.

BACKGROUND: Seeing triple is a rare complaint, so anatomically unlikely that it is often considered a diagnostic symptom of hysteria. OBJECTIVE: To evaluate the complaint of triple vision among a large group of neurological inpatients. DESIGN: Personal case series during a 34-year period. SETTING: Neurology and neurosurgery wards of the University of Southern California-Los Angeles County Medical Center. Patients Thirteen patients who reported seeing objects in triplicate. RESULTS: Eleven of 13 patients had ocular motor findings, including third nerve palsy in 5 patients, internuclear opht…

[Neurology in the court of the Learned King].

During the reign of Alfonso X of Castile one of the most flourishing cultural periods of European history was reached. The so-called Learned King contributed to this development by promoting the use of Castilian as the language of scientific discovery instead of Latin, and favoring the recovery of Arabic origin manuscripts in the Toledo Translators' School. Where the wise men from the "three cultures" (Christian, Islamic and Jewish) worked together in harmony to perpetuate classical knowledge. The aim of this paper is to analyze neurological pathology in the period concerned. The Learned King

[Quality control of Doppler diagnoses in vascular neurology].

INTRODUCTION: The use of ultrasonography in the study of extracranial pathologies is becoming widespread among the different neurology services in our community. Its main limitation is that it is a technique that depends on the explorer. AIMS: The purpose of this study is to determine the reliability of our Neurosonology laboratory by validating its results. PATIENTS AND METHODS: We conducted a retrospective study by selecting the first 100 consecutive patients (with ischemic stroke) who had been examined in the year 2001 in the Neurosonology laboratory in our hospital. These subjects had to…

The polysynaptic spine-stem reflex in clinical neurology.

Data from 25 years of studies of the spinobulbospinal (SBS) reflex in normal conditions and in a variety of nervous system diseases with lesions of the afferent and efferent systems, subcortical brain formations, and musculature (contracture of the expressive muscles, myofascial pain syndrome) are presented. The instability of many characteristics of the reflex (latency, amplitude, duration of response, presence) is shown to characterize this reflex in normal conditions. These measures become more stable in central nervous system pathology, which is evidence for impairment of programs which co

[Interface neurology/psychiatry: a case report of neurosyphilis].

Organic diseases can occur with different psychiatric symptoms. Neurosyphilis was considered to be a landmark in the history of organic mental syndromes. The complexity of its clinical picture decreases the boundaries between Neurology and Psychiatry and requires a multidisciplinary approach. We report a case of neurosyphilis that began with psychiatric symptoms in a twenty five years old male.

Armand Trousseau--some of his contributions to neurology.

Trousseau made a remarkably large number of original clinical contributions to medicine and neurology. Best known are Trousseau's syndrome, the combination of venous thrombosis with visceral carcinoma; tache cérébrale, the red streak seen on scratching the skin in acute meningitis; and Trousseau's sign, the cardinal physical sign in tetany. His pioneering work in tracheostomy in diphtheria, haemochromatosis, Parkinson's disease, aphasia and chorea are but a few of his outstanding clinical studies. Based on his famously comprehensive text, Clinique Médicale de l'Hôtel Dieu, this paper highlight…

The impact of molecular biology on clinical neurology.

Advances in molecular biology have increased our understanding of both inherited and sporadic forms of neurological disease. In this review, the impact of these advances is discussed in relation to specific neurological conditions. These include the hereditary neuropathies and ataxias, Huntington's disease, and the muscular dystrophies, as well as Alzheimer's disease, Parkinson's disease, and motor neuron disease. Genetic channelopathies, such as familial hemiplegic migraine, are also described. Although knowledge in this area overall is still relatively scant, current advances in molecular bi

Acute neurology and neurophysiology of haemolytic-uraemic syndrome.

Involvement of the central nervous system (CNS) is found in around 30% of children with haemolytic-uraemic syndrome (HUS). This complication is the single most common cause of mortality and also a major contributor to the morbidity associated with HUS. We reviewed 22 children with HUS and acute CNS involvement. Both global and focal derangements occurred, and in survivors, early regional EEG abnormalities-especially those in the occipital and temporal areas-were prognostically useful.

[Recent progress in electrodiagnostic tests in clinical neurology].

Electrodiagnostic testings including electromyography have become increasingly important tools in clinical practice of neurology. These tests should not merely be referred to a technician, but be viewed as a clinical tool used by a neurologist. Recent advances in clinical electromyography, nerve conduction tests, and tests for axonal function including the newly developed threshold electrotonus are reviewed. These not only serve as a diagnostic tool, but also are useful in clinical trials by providing an objective parameter of nerve function. Computerized threshold tracking techniques such as …

[Therapeutic use of botulinum A toxin in neurology].

The highly potent neurotoxins produced by Clostridium botulinum lead to botulism when ingested in appreciable amounts. However, botulinum toxin injections delivered intramuscularly in very small quantities can produce a therapeutically intended focal paresis while producing only negligible local or systemic side effects. Over the past several years, various neurological disorders, especially those involving increased muscle tone and/or abnormal movements, have been successfully treated with local botulinum A toxin injections. The success of this method has led to a general change in the manage

Neurology of AIDS virus infection: a clinical classification.

Infection with the AIDS virus itself (HIV, HTLV-III, LAV, ARV) is associated with a full spectrum of neurological disorders. The application of diagnostic studies for HTLV-III infection has demonstrated that these neurologic disorders can be the first manifestation of AIDS or occur in the absence of AIDS. The most common conditions associated with HTLV-III infection alone are a subacute encephalopathy (AIDS dementia) and peripheral neuropathy; however, vacuolar myelopathy and both acute and chronic aseptic meningitis are also common. Congenital (or neonatal) transmission of the virus can resul

Child neurology: Amino acid concentrations in cerebrospinal fluid.

Cerebrospinal fluid amino acid concentrations were measured in 16 pediatric patients with febrile convulsions, in 9 with bacterial meningitis, and in 8 noninfected newborn infants. Most CSF amino acids are present in greater concentration in patients with bacterial meningitis than in those without CNS infection. Newborn infants have elevated CNS amino acid concentrations similar to those found in older patients with bacterial meningitis. The reason for increased amino acid concentration in the meningitis group may be due to alterations in brain metabolism, changes in the kinetics of CSF format

Impact of new technologies on neurology in Spain. Review by the New Technologies Ad-Hoc Committee of the Spanish Society of Neurology.

INTRODUCTION: New technologies are increasingly widespread in biomedicine. Using the consensus definition of new technologies established by the New Technologies Ad-Hoc Committee of the Spanish Society of Neurology (SEN), we evaluated the impact of these technologies on Spanish neurology, based on communications presented at Annual Meetings of the SEN. MATERIAL AND METHODS: We defined the concept of new technology in neurology as a novel technology or novel application of an existing technology, characterised by a certain degree of coherence persisting over time, with the potential to have an…
