Amyotrophic lateral sclerosis (ALS) is the most common adult-onset neuromuscular disease for which there is currently no effective treatment. The progression of ALS includes loss of motor neurons controlling the voluntary muscles, with much of this loss occurring at the neuromuscular junction. Sjögren-Larsson syndrome (SLS) is an autosomal recessive disease characterized by a triad of congenital ichthyosis, spastic quadriplegia and mental retardation. PURPOSE: Myasthenia gravis (MG) pathophysiology and drug-drug interactions with cholinesterase inhibitors can make patients both relatively sensitive and resistant to neuromuscular blocking agents (NMBAs). NMBA dose adjustments in MG patients undergoing rapid sequence intubation (RSI) are described in anesthesia literature but not outside of the operating room (OR). This study sought to characterize NMBA use in MG patients undergoing RSI outside of the OR.
METHODS: Retrospective, observational, multicenter study of MG patients undergoing RSI outside of the OR from January 2011-September 2023.

A flexible fibreoptic laryngoscope was used to facilitate the passage of a long polyvinyl chloride tracheal tube into the left main bronchus during repair of a tracheo-oesophageal fistula situated near the carina in a patient in whom intubation with a double-lumen endobronchial tube was difficult. Close monitoring of neuromuscular transmission using a peripheral nerve stimulator also contributed to the successful anaesthetic management of this patient.
