Neurological complications of ankylosing spondylitis: neurophysiological assessment.

Studies examined the neurological involvement of ankylosing spondylitis (AS) are limited. This study aimed to assess the frequency of myelopathy, radiculopathy and myopathy in AS correlating them to the clinical, radiological and laboratory parameters. Included were 24 patients with AS. Axial status was assessed using bath ankylosing spondylitis metrology index (BASMI). Patients underwent (a) standard cervical and lumbar spine and sacroiliac joint radiography, (b) somatosensory (SSEP) and magnetic motor (MEP) evoked potentials of upper and lower limbs, (c) electromyography (EMG) of trapezius a

Clinical and neurophysiologic testing of the penilo-cavernosus reflex.

AIMS: Both clinical and neurophysiologic testing of the penilo-cavernosus reflex is used in clinical practice. The aim of the present study was to determine the as yet unestablished potential contribution of sacral reflex testing to the diagnostic evaluation of patients with suspected neuropathic sacral lesions. METHODS: Fifty-three men with clinical, electrodiagnostic, and radiologic signs of chronic cauda equina or conus medullaris lesions were studied. Clinical examination, including assessment of anal sphincter tone and perianal sensation, and both clinical and neurophysiologic testing of

Psychiatric and neurophysiological predictors of obesity in HIV/AIDS.

The objective of the study was to identify predictors of obesity. One hundred eleven nonobese and 48 obese HIV-1 seropositive patients provided information on medical history and other characteristics. They were then asked to detect the passage of 2-s time intervals while the contingent negative variation (CNV) was recorded. Obese patients were healthier, more likely to be receiving Highly Active Antiretroviral Therapy, and less likely to be substance dependent. Obese patients also exhibited a greater CNV slope and responded prematurely. A path model suggested that CD4+count and protease inhib

Neurophysiological and neurochemical basis of modern pruritus treatment.

Chronic pruritus of any origin is a frequent discomfort in daily medical practice, and its therapy is challenging. Frequently, the underlying origin may not be identified and symptomatic therapy is necessary. Conventional treatment modalities such as antihistamines often lack efficacy, and hence new therapeutic strategies are necessary. The neuronal mechanisms underlying chronic pruritus have been partly identified during the past years and offer new therapeutic strategies. For example, mast cell degranulation, activation of neuroreceptors on sensory nerve fibres and neurogenic inflammation ha

Intraoperative neurophysiologic monitoring during spinal surgery.

Intraoperative neurophysiologic monitoring (IONM) is a battery of neurophysiologic tests used to assess the functional integrity of the spinal cord, nerve roots, and other peripheral nervous system structures (eg, brachial plexus) during spinal surgery. The underlying principle of IONM is to identify emerging insult to nervous system structures, pathways, and/or related vascular supply and to provide feedback regarding correlative changes in neural function before development of irreversible neural injury. IONM data provide an opportunity for intervention to prevent or minimize postoperative n

Krabbe disease: neurophysiologic studies and MRI correlations.

BACKGROUND: Krabbe disease (KD) is a rare hereditary leukodystrophy affecting children mostly in the first 6 months of life; later onset has been reported as well. OBJECTIVE: To review abnormalities in neurophysiologic studies in children with KD and determine if there is a correlation between these studies and disease severity as measured by MRI scans. METHODS: KD patients with at least one neurophysiologic study and one MRI scan at the authors' institution were reviewed. Relationships between KD type, neurophysiologic studies, and severity of disease as measured by MRI were explored. RESU

Agyria-pachygyria: clinical, neuroimaging, and neurophysiologic correlations.

Agyria-pachygyria complex is a disorder of neuronal migration and organization. Patients suffer either motor or intellectual retardation. We report our experiences of 10 patients with agyria-pachygyria complex and evaluate their clinical features, electroencephalography, and evoked potentials. Of nine electroencephalography examinations, five patients demonstrated characteristically high-amplitude fast activity. One of nine patients had an abnormal brainstem auditory-evoked potential. Three of seven patients had abnormal goggled visual-evoked potential. Six patients received somatosensory-evok

[Neurophysiological investigations in patients with sleep apnoea syndrome].

UNLABELLED: The untreated sleep apnoea syndrome (SAS) is associated with cardiovascular complications (arterial hypertension, coronary heart disease, cardiac arrhythmia) and increased morbidity and mortality. Disorders of the autonomic nervous system function are probably responsible for cardiac arrhythmia. Most researchers evaluated autonomic function using cardiovascular tests. The aim of our study was to assess whether neurophysiological method--sympathetic skin response (SSR)--could be useful in testing function of the autonomic nervous system in patients with SAS. Neurophysiological investigations in patients with primary writing tremor.

The pathophysiology of primary writing tremor (PWT) is still unknown: it has been classified as a focal form of essential tremor and as a tremulous form of writer's cramp. We studied cortical and spinal excitability in patients with PWT and compared the results with published data of patients with essential tremor, and writer's cramp. We used electrical stimulation of median and radial nerve to study reciprocal inhibition of forearm antagonist muscles and paired transcranial magnetic stimulation at short and long interstimulus intervals (ISIs) to assess intracortical excitability. Both studies

Neurophysiologic parameters and symptoms in chronic renal failure.

We studied: (1) the sensitivity of various neurophysiologic parameters in the diagnosis of uremic polyneuropathy, (2) the relationship between subjective symptoms and neurophysiologic parameters, and (3) the effect of a single hemodialysis on the neurophysiologic parameters in 21 patients undergoing hemodialysis. The following parameters were studied: sensory and motor nerve conduction, including F-wave parameters; vibration detection thresholds; and thermal thresholds. The clinical findings and subjective symptoms were studied using a standardized questionnaire. The most sensitive parameters

Protocol for clinical neurophysiologic examination of the pelvic floor.

Clinical neurophysiologic examination of the pelvic floor is performed worldwide, but there is no consensus on the choice of tests, nor on technical details of individual methods. Standardized methods are, however, necessary to obtain their valid application in different laboratories for the purpose of collection of normative data, comparison of patient data and organization of multi-center studies. It is proposed that in patients with suspected "lower motor neuron" type lesions concentric needle electromyography (CNEMG) is the most informative test to detect pelvic floor denervation/reinnerva

Neurophysiological study of patients with perceived 'electrical hypersensitivity'.

The aim of the present study was to investigate baseline neurophysiological characteristics of the central and autonomous regulation and their reactivity to different tests in a group of persons with so-called 'electrical hypersensitivity', which is often considered as a form of psychosomatic disorders. Twenty patients with combinations of neuroasthenic symptoms (general fatigue, weakness, dizziness, headache) and facial skin (itching, tingling, redness) have been investigated. An equal number of symptom-free persons served as a control group. The examination comprised self-reported measures,

Neurophysiological classification of ulnar entrapment across the elbow.

Ulnar nerve entrapment across the elbow (UAE), a common entrapment, requires neurophysiological evaluation for a diagnosis, but a standardised neurophysiological classification is not available. The aim of our study was to evaluate the validity of a neurophysiological classification of UAE, developed by us. To this end, we examined whether sensorimotor deficits, as observed by the physician and as referred by the patients, increased with the neurophysiological severity according to the classification. We performed a multiperspective assessment of 63 consecutive arms from 52 patients with a cli

Neurophysiological correlates of face gender processing in humans.

Event-related potentials (ERPs) were recorded while subjects were involved in three gender-processing tasks based on human faces and on human hands. In one condition all stimuli were only of one gender, preventing any gender discrimination. In a second condition, faces (or hands) of men and women were intermixed but the gender was irrelevant for the subject's task; hence gender discrimination was assumed to be incidental. In the third condition, the task required explicit gender discrimination; gender processing was therefore assumed to be intentional. Gender processing had no effect on the oc

Neurophysiological effects of pediatric balloon dilatation procedures.

Balloon dilatation of valvar and vascular stenoses has become routine therapy in pediatric cardiology. Repeated balloon inflations cause many episodes of low cerebral oxygen delivery. This study is a prospective study to assess the effects of balloon dilatation on cerebral perfusion and oxygenation. The study included 11 patients scheduled for elective catheterization and balloon dilatation at a university pediatric hospital. Blood flow velocity in the middle cerebral artery (Vmca) and regional cerebral oxygen saturation (rSO2) were monitored by means of transcranial Doppler sonography and nea

Neurophysiological evaluation of acute facial paralysis in children.

Objective evaluation of facial nerve paralysis represents a unique challenge to the clinician. Electroneurography (ENoG) and the acoustic reflex (AR) have been widely used as neurophysiological tests in an assessment of facial nerve function. However, ENoG or AR alone does not suffice diagnostic and prognostic purposes of facial function evaluation in children. To further investigate the diagnostic aspects of facial nerve paralysis, the prognostic value of AR and ENoG, and the time course of the disease in pediatric population, a series of 30 children with acute facial paralysis were investiga
