Sir Hugh Cairns: a pioneering collaborator.

In April 1988, Peter Schurr delivered the twelfth Sir Hugh Cairns Memorial Lecture to the Society of British Neurological Surgeons. In his lecture, The Cairns Tradition, Schurr extolled the personal virtues of Cairns. He encouraged his colleagues to draw inspiration from Cairns' renowned determination, organisation, drive for perfection, compassion, and commitment to the training of those around him. Indeed, Cairns' own personality has come to define the specialty which he established in Britain. Today's neurosurgeons are, whether knowingly or not, formed in his image. But there is a side to H…

Treatment by specialist surgical neurooncologists improves survival times for patients with malignant glioma.

OBJECT: Surgeries for CNS tumors are frequently performed by general neurosurgeons and by those who specialize in surgical neurooncology. Subspecialization in neurosurgical practice has become common and may improve patient morbidity and mortality rates. However, the potential benefits for patients of having their surgeries performed by surgical neurooncologists remain unclear. Recently, a shift in patient care to those who practice predominantly surgical neurooncology has been promoted. Evidence for this practice is lacking and therefore requires fundamental investigation. METHODS: The autho…

Stroke team workforce maldistribution in indonesia: a burden-based policy analysis.

BACKGROUND: Stroke is Indonesia's leading cause of disability, yet workforce planning relies on population-based norms, risking territorial inequities in access to essential services. OBJECTIVE: To examine the ecological alignment between provincial stroke burden and key stroke team workforce availability, and to test the robustness of a proposed burden-based planning indicator. METHODS: Cross-sectional ecological policy analysis across 34 provinces combining Global Burden of Disease Study 2023 DALYs with 2025 administrative data on neurologists, neurosurgeons, and physiotherapists. Burden-t…

BACKGROUND: Nerve transfers have been heralded as a "quantum leap" in peripheral nerve surgery, fundamentally altering the reconstructive pa … eons performed 60% of nerve transfers despite accounting for only 33.5% of all BPI reconstructions. Neurosurgeons had the lowest overall nerve transfer utilization but increased to a rate matching orthopedic surgeons in the final 4 years. A case of carotid blowout syndrome following endoscopic endonasal surgery and proton beam therapy for chondrosarcoma: Considering the role of nasoseptal mucosal flap for minimizing bleeding.

BACKGROUND: Following advancements in endoscopic endonasal surgery (EES), neurosurgeons have become increasingly proficient in exposing the internal carotid artery (ICA) through EES. Consequently, they are also more frequently encountering carotid blowout syndrome (CBS), a potentially fatal complication. Therefore, it is essential for neurosurgeons to be well-prepared to manage the emergency effectively. CASE DESCRIPTION: A 64-year-old man underwent EES for a left clival chondrosarcoma. Following tumor resection, the left ICA was covered with a nasoseptal mucosal flap to mitigate the risk of …

Augmented-reality integrated robotics in neurosurgery: are we there yet?

Surgical robots have captured the interest-if not the widespread acceptance-of spinal neurosurgeons. But successful innovation, scientific or commercial, requires the majority to adopt a new practice. "Faster, better, cheaper" products should in theory conquer the market, but often fail. The psychology of change is complex, and the "follow the leader" mentality, common in the field today, lends little trust to the process of disseminating new technology. Beyond product quality, timing has proven to be a key factor in the inception, design, and execution of new technologies. Although the first …

Determination of a facial nerve safety zone for navigated temporal bone surgery.

BACKGROUND: Transtemporal approaches require surgeons to drill the temporal bone to expose target lesions while avoiding the critical struct … erveClick's accuracy in an experimental setting of neuro-otologic and neurosurgical patients. Three neurosurgeons used it to segment 126 facial nerves, which were compared with the gold standard: manually segmented facial nerve centerlines. The centerlines are used as a central axis around which a tubular safety zone is built. The zone's thickness incorporates …

A lesson in history: the evolution of endoscopic third ventriculostomy.

The history of endoscopic third ventriculostomy (ETV) demonstrates the importance of studying neurosurgery's history. A story that began with numerous technological advancements started to fizzle as neurosurgeons were stymied by problems encountered during the infancy of the technology they were still developing. The new technique, although sound in theory, failed to deliver a realistic solution for managing hydrocephalus; it lost the battle to the valved shunt. Over the last 15-20 years, a clearer understanding of pathophysiological mechanisms underlying various forms of hydrocephalus, along …

Innovation in neurosurgery: Walter Dandy in his day.

In 1925, Walter Dandy published a preliminary report of an innovative operative procedure for patients with tic douloureux. Dandy reported treating tic by selectively sectioning the trigeminal nerve at the brain stem. His operative field was the cerebellopontine angle, which he exposed using a cerebellar approach. It is commonly acknowledged among neurosurgeons that Dandy's technique was overlooked in favor of the Spiller - Frazier procedure during Dandy's lifetime and for at least 15 years after his death. This article examines historically Doctor Dandy's ideas regarding the treatment of tic …

[Original Ventriculoperitoneal Shunt for Pediatric Hydrocephalus].

Although the use of newly developed procedures such as endoscopic third ventriculostomy or coagulation of the choroid plexus has gradually expanded, ventriculoperitoneal(VP)shunts are still not obsolete. They are the most commonly performed surgery for treating pediatric hydrocephalus. Young neurosurgeons may frequently encounter this CSF diversion procedure in clinical practice. However, the VP shunt remains a failure-prone procedure, as exemplified by obstruction, infection, mechanical shunt failure, over drainage, and distal catheter site-specific failures. Therefore, surgery requires exqui…

CSF rhinorrhoea: the place of endoscopic sinus surgery.

Cerebrospinal fluid (CSF) rhinorrhoea has been managed by both neurosurgeons and otorhinolaryngologists, with neurosurgeons often choosing an intracranial approach and otorhinolaryngologists an extracranial approach. Recently, transnasal endoscopic techniques have been introduced that significantly reduce the morbidity of surgical repair when compared with previous techniques. The sense of smell was preserved in all patients who underwent an endoscopic repair of their CSF leak where it was present preoperatively. The results of transnasal endoscopic repair now make it the treatment of choice f…
