[Jehovah's witness patients in obstetrics : challenges and strategies].

The refusal of blood transfusions by Jehovah's Witness patients is a major challenge in obstetrics. Although transfusions are rare (1.4 % in obstetrics), the lack of blood transfusions during severe hemorrhage significantly increases the risk of maternal death. This article analyzes the shift in the ethical paradigm in Switzerland, marked by the transition from paternalism to respect for the self-determination of the competent patient. It details blood management strategies (PBM), ranging from antenatal hemoglobin optimization to early surgical hemostasis. Multidisciplinary planning and the fo…

Racial, Ethnic, and Socioeconomic Disparities in Cardio-Obstetrics.

Racial, ethnic, and socioeconomic disparities persist in American medicine. Black pregnant people die from pregnancy-related causes at 2 to 3 times the rate of their White counterparts, with cardiovascular disease as the leading contributor. These disparities in cardio-obstetrics arise from intersecting structural forces-economic inequity, residential segregation, limited health care access, implicit bias, and the cumulative physiological toll of structural racism. Effective solutions require a multifaceted action: community health worker programs, group prenatal care, hospital quality improve…

Pseudo-uterine Contractions: A New Entity in Obstetrics.

BACKGROUND: Listening to patients and obtaining a thorough history are fundamental in clinical practice. Some pregnant women, particularly in late or preterm gestation, report regular, painful uterine contractions that may not correspond to true labor. In some cases, patients admitted to feigning contractions using the Valsalva maneuver to gain secondary benefits. Misdiagnosing such cases may lead to unnecessary interventions, including iatrogenic prematurity. OBJECTIVES: To explore the phenomenon of pseudo-uterine contractions and assess whether maternal heart rate (HR) can help differentiat

[Cell salvage in obstetrics-Background and practical implementation].

Postpartum hemorrhage (PPH) remains a leading cause of maternal mortality worldwide, with an increasing incidence in western countries over the past decades. During this period the numbers of PPH-related allogeneic red blood cell (RBC) transfusions have also significantly increased. Given the increasing scarcity and risks of allogeneic RBC transfusions, which are also associated with adverse maternal outcomes, optimized blood management strategies are urgently needed in obstetrics. In recent years, patient blood management (PBM) has been increasingly integrated into medical care, resulting in …

Procedures by Physician Associates in Obstetrics and Gynecology.

BACKGROUND AND OBJECTIVES: The number of obstetricians and gynecologists in the United States is decreasing and providers backfilling this service have not been well described. The intent of the study was to identify the skills that physician associates (PAs) in obstetrics and gynecology (OBGyn) contribute to this aspect of medicine and surgery. METHODS: A survey of PAs specializing in OBGyn was conducted in 2022. The intent was to list office-based procedures that were part of their skill set. A vetted questionnaire was sent to the 1,630 American Academy of Physician Associates members who i…

Current concepts in preoperative anemia management in obstetrics.

PURPOSE OF REVIEW: The purpose of this article is to provide an overview of currently recommended treatment approaches for anemia during pregnancy, with a special focus on iron deficiency and iron deficiency anemia (IDA). RECENT FINDINGS: As consistent patient blood management (PBM) guidelines in obstetrics are still lacking, recommendations regarding the timing of anemia screening and the treatment recommendations for iron deficiency and IDA during pregnancy are still controversial. Based on increasing evidence, early screening for anemia and iron deficiency should be recommended at the begi…

The role of labor induction in modern obstetrics.

A primary goal of obstetrical practice is the optimization of maternal and perinatal health. This goal translates into a seemingly simple assessment with regard to considerations of the timing of delivery: delivery should occur when the benefits are greater than those of continued pregnancy. In the absence of an indication for cesarean delivery, planned delivery is initiated with induction of labor. When medical or obstetrical complications exist, they may guide recommendations regarding the timing of delivery. In the absence of these complications, gestational age also has been used to guide

Collaborative Efforts Between Interventional Radiology and Obstetrics/Gynecology.

Since the introduction of uterine artery embolization as a treatment option for symptomatic leiomyomas, there has been a growing interest in expanding the role of interventional radiology (IR) in the evaluation and treatment of obstetrics and gynecology (OBGYN) patients. This review provides an overview of opportunities for collaboration between IR and OBGYN. This can include medically and/or surgically complex patients, efforts to reduce quantitative blood loss, obstetrical emergencies, and consideration of fertility or uterine preservation. Increased collaborative efforts between IR and OBGY…

Role of Interventional Procedures in Obstetrics and Gynecology.

Radiological guided intervention techniques are discussed in obstetric and gynecologic patients. Fallopian tube recanalization, postpartum hemorrhage control, techniques of treating uterine leiomyomas, pelvic congestion treatment, and the use of percutaneous and transvaginal ultrasonography-guided aspirations and biopsy are covered. These techniques use basic radiological interventional skills and show how they are adapted for use in the female pelvis.

A case series of scrub typhus in obstetrics.

Scrub typhus is endemic and re-emerging in eastern and southern Asia. Illness varies from mild and self-limiting to fatal. Only few studies were published about its effect in maternal and neonatal outcome. A retrospective analysis was done in six prenatal and two postnatal women with scrub typhus. Details about clinical presentation, investigations, treatment given, response to treatment and pregnancy outcome were collected. The common symptoms were fever with chills, vomiting, myalgia, headache and abdominal pain. Typical features of eschar and lymphadenopathy were noted in only two cases. Tw
