Second chances.

An 81-year-old woman presented with light perception vision in her left eye and had pallid swelling of the left optic disk. Temporal artery biopsy was normal. Magnetic resonance imaging revealed enlargement and enhancement of the left orbital optic nerve. There were vitreous cells, but a vitrectomy specimen showed only a benign lymphocytic population. Her vision deteriorated to no light perception in the left eye, and an optic nerve biopsy revealed a diffuse astrocytoma of World Health Organization grade II.

[Iritis and optic nerve meningitis--initial symptoms of latent syphilis].

A 19-year-old woman presented in the latent phase of secondary syphilis with chronic bilateral anterior uveitis and a slightly elevated optic disk with indistinct margins in the left eye; this was interpreted as a meningitis of the optic nerve. Serologic examination revealed the syphilitic infection, which had not previously been apparent. The symptoms resolved completely after specific therapy.

Selective plasmonic trapping of nano-particles by Archimedes metalens.

Optical tweezer is a non-invasive method for optical force tool applied in various fields like biology, physics, and lab on chip manipulation. The Archimedean helix shape is ideal for creating chiral nanostructures, and being able to generate plasmonic focused hotspot field for optical trapping. Here we design a metal disk with the Archimedean shape to own the ability of selective trapping nanoparticles based on the spin-orbit interactions with circularly polarized light. The plasmonic near field on the metalens can be designed by adjusting the geometric parameter flexibly. We numerically anal

Documentation of congenital hypertrophy of the retinal pigment epithelium with wide-field funduscopy.

BACKGROUND: Congenital hypertrophy of the retinal pigment epithelium (CHRPE) are rare hyperpigmented fundus lesions of the peripheral retina. METHODS: We evaluated CHRPE with the Optomap wide-field fundus camera. RESULTS: The pattern, location, and extension of the pigmented lesion were compared between the wide-field and composed digital fundus camera. The wide-field image had the same angle of the fundus image, magnification as well homogenious illumination of the entire fundus. Superimposed pattern of the lesions extended from the margin of the optic disk and radiated in sectors to the fu…

Glaucoma. A primer for primary care physicians.

Glaucoma is best detected by examination of the optic disk, since intraocular pressure is not always elevated in patients with the condition. A large, vertically oval cup within the optic disk is strong evidence for glaucoma. Open-angle glaucoma, the most common form of the disorder, often is not detected until the disease is advanced. It can usually be treated successfully with topical medications, but systemic absorption of these can result in serious side effects. If medical treatment fails, laser therapy or filtering surgery may be helpful. Acute angle-closure glaucoma has a sudden onset m…

Progressive visual loss in syphilitic optic atrophy.

A 62-year-old man had progressive visual loss from neurosyphilis while his optic disks appeared atrophic. At no time was there evidence of inflammation of the globe or of either optic disk. The presence of an extremely high IgG index and five oligoclonal bands in the cerebrospinal fluid indicated that the central nervous system was synthesizing antibody specific to Treponema pallidum. Recent investigations indicate that much larger doses of penicillin are necessary for adequate treatment of neurosyphilis than have been recommended previously.

Quasi-periodic X-ray eruptions years after a nearby tidal disruption event.

Quasi-periodic eruptions (QPEs) are luminous bursts of soft X-rays from the nuclei of galaxies, repeating on timescales of hours to weeks1-5. The mechanism behind these rare systems is uncertain, but most theories involve accretion disks around supermassive black holes (SMBHs) undergoing instabilities6-8 or interacting with a stellar object in a close orbit9-11. It has been suggested that this disk could be created when the SMBH disrupts a passing star8,11, implying that many QPEs should be preceded by observable tidal disruption events (TDEs). Two known QPE sources show long-term decays in qu

Ischemic optic neuropathy.

Ischemic optic neuropathy is one of the major causes of blindness or seriously impaired vision, yet there is disagreement as to its pathogenesis, clinical features and especially its management. This is because ischemic optic neuropathy is not one disease but a spectrum of several different types, each with its own etiology, pathogenesis, clinical features and management. They cannot be lumped together. Ischemic optic neuropathy is primarily of two types: anterior (AION) and posterior (PION), involving the optic nerve head (ONH) and the rest of the optic nerve respectively. Furthermore, both A

[Ocular syphilis].

BACKGROUND: Syphilis has distinctly increased in the last decades. It is necessary to know the different eye changes in syphilis. PATIENTS: In four patients, between 34 and 71 years of age, typical as well as unusual syphilitic eye changes were found. In three patients diagnosis was made due to the inflammatory eye changes. RESULTS: An iritis was found in two patients (62 and 34 years old). In addition, the 62-year-old patient showed the unusual fundus picture of both vasculitis and neuroretinitis, with macular exudates and swelling of the optic disk. A venous occlusion, a rare sign in syphi…

Management of proliferative diabetic retinopathy.

Proliferative diabetic retinopathy is characterized by neovascularization originating from the retina and/or optic disk in patients with diabetes mellitus. The role of vascular endothelial growth factor appears to be central in the pathogenesis of proliferative diabetic retinopathy. Advanced glycation end products are important in the development of vitreous abnormalities in proliferative diabetic retinopathy. The majority of the neovascular membranes are adherent to the posterior vitreous cortex. When the posterior hyaloid exerts traction, the edges of the neovascular complex are pulled forwa…

[Research advances in the risk factors of nonarteritic anterior ischemic optic neuropathy].

As the most common acute optic neuropathy among individuals who are over 50-year old,non arteritic anterior ischemic optic neuropathy ( NAION) is presumed result from a circulatory insufficiency,or infarct, within the lamina or retrolaminar portion of the optic nerve head that is supplied by the short posterior ciliary arteries. It usually presents with sudden painless unilateral visual loss, characterized visual field defects, relative afferent papillary defect and pallid optic disk edema. The exact etiology of NAION remains elusive. Although numerous risk factors have been proposed, most of …

Fractal-based automatic localization and segmentation of optic disc in retinal images.

In this paper, we proposed a novel algorithm to detect optic disc location in retinal images. Optic disc is a bright disk area and all major blood vessels and nerves originate from it. With its high fractal dimension of blood vessel, optic disc can be easily differentiated from other bright regions such as hard exudates and artifacts. Compared with existing algorithms, ours has much lower computational cost and is more robust. With its location known, segmentation of optic disc can be done with simple local histogram analysis. The algorithm can be valuable for automated processing for early st
