For the period from 2004 till 2009 142 patients were observed in our clinic. We were able to follow up 53 of the 74 patients in the main group and 49 of the 68 ones in the control group for 12 months. The majority of injuries were located in the front area of foot; most of them were dislocation fractures. It was shown that the frequency of isolated stable and unstable traumas was roughly equal. The series involves 42 consecutive acute fractures of the dens, of which 41 were treated nonoperatively. There were 26 Type II and 16 Type III fractures, as classified by Anderson and D'Alonzo. All Type III fractures and undisplaced or anteriorly displaced Type II fractures united. Existing internal fixation systems for the injured or deformed spine present problems with overdistraction and control of the contoured rod necessary for transverse forces. A locking hook spinal rod avoids these problems by using a locking cover to secure the lamina in the hook and meshing radial grooves to lock the contoured rod to both the upper and lower hooks in 6 degrees intervals of rotation. This is a report of an irreducible distal radioulnar joint associated with a Galeazzi fracture. The irreducible dislocation of the radioulnar joint was not appreciated until 2 weeks following compression plating of the radius fracture. The extensor carpi ulnaris was found to be wrapped anteriorly about the ulnar border of the distal ulna and was trapped in this position by the displaced ulnar styloid. The patient was successfully treated with open reduction of the styloid fracture and temporary wire fixation.

The Ilizarov device was used for extrafocal osteosynthesis in complex fractures of the distal end of radius. With the aid of the device the bone fragments can be joined and kept in a correct position till a callus is formed. The mean time of fixation in the apparatus is equal to 21 days. In 114 patients 117 operations were performed. Good remote results were obtained in 73 of 99 patients (73,7%), satisfactory results in 25 (25,3%).

OBJECTIVES: Various comparative studies and techniques have been described for median sternotomy closure in the literature, previously. However, some patients are still under risk of sternal dehiscence, malunion or nonunion due to intrinsic or extrinsic factors after median sternotomy closure. Sternal nonunion described as sternal pain, with clicking, instability, or both for more than 3 months in the absence of infection, is an uncommon complication of midline sternotomy incision. To date, only a few studies have addressed the entity of sternal nonunion and its treatment.

A number of approaches have been attempted to address the treatment of acute odontoid fractures, spanning from conservative treatment with an external orthotic device to posterior C1-C2 arthrodesis or anterior screw fixation. The current report describes stabilisation of such fractures with a C1 supralaminar hook and a C2 pars interarticularis screw construct. Two patients with odontoid fracture were treated by this technique. Two-year follow-up of both patients revealed complete union without complications. The authors stress that this technique avoids the hazards of C1 lateral mass screws.

PURPOSE OF REVIEW: We review recent advances in the prevention, diagnosis and treatment of infections associated with joint prostheses and internal fixation devices.
RECENT FINDINGS: The perioperative antimicrobial prophylaxis should be administered 60-30 min before incision or before inflation of the tourniquet. New diagnostic approaches include sonication of removed implants to dislodge adherent microorganisms growing in biofilms and the use of molecular techniques to improve diagnostic yield. OBJECTIVE AND IMPORTANCE: Early sacral fracture is an extremely rare complication of instrumented lumbosacral fusion seen in older, osteopenic women. Previous reports have attributed the problem to the use of multisegmental (three or more levels) fixation, with the transfer of stress forces from rigid spinal implants to the sacrum. We report the only case, to the best of our knowledge, of early sacral fracture after a two-level lumbosacral fusion and the only case of early sacral fracture after reduction of spondylolisthesis.
