Experimental study of temperature rise during bone drilling process.

An excessive temperature rise during bone drilling processes can result in osteonecrosis or impairment of the osteogenic potential. However, the effect of geometric parameters of the surgical drill bit, drilling process parameters, and the bone type on the temperature rise have not been fully investigated. In this study, thermocouples are introduced to measure the temperature rise, and three experimental designs are utilized separately to investigate the temperature rise with respect to each parameter, identify the effect of important drill geometric parameters and their interaction on the tem

Avascular Necrosis of the Sesamoids.

Avascular necrosis (AVN) of the sesamoid is pathology of the medial or lateral hallucal sesamoid resulting in pain under the first metatarsophalangeal joint often presenting in young female athletes. There is overlap of stress fracture, nonunion, and AVN that makes defining the diagnosis difficult but the treatment and outcomes are similar. The most reliable operative treatment used for AVN of the sesamoid is an accumulation of anatomic and mechanical factors with repetitive microtrauma. Nonoperative modalities are designed to offload the sesamoid. The only operative treatment used for AVN of

On-Top-Plasty for Atypical Thumb Duplication: A Case Report With 10 Year Follow-up.

BACKGROUND: A very uncommon pattern of thumb duplication consists of an ulnar-side floating thumb and a radial-side distally hypoplastic thumb. METHODS: We report the case of a 15-month-old child with this type of thumb duplication on his right hand treated with an on-top-plasty technique. The ulnar-side segment was pedicled and transferred to the lateral thumb, which was distally resected. RESULTS: A well-aligned and widely mobile thumb with a wide first web and an excellent cosmetic appearance was exhibited 10 years after surgery. CONCLUSIONS: On-top-plasty technique might be amenable to

The mucopolysaccharidoses: advances in medical care lead to challenges in orthopaedic surgical care.

The mucopolysaccharidoses (MPS) are a group of inherited lysosomal storage disorders with clinical manifestations relevant to the orthopaedic surgeon. Our aim was to review the recent advances in their management and the implications for surgical practice. The current literature about MPSs is summarised, emphasising orthopaedic complications and their management. Recent advances in the diagnosis and management of MPSs include the recognition of slowly progressive, late presenting subtypes, developments in life-prolonging systemic treatment and potentially new indications for surgical treatment

Round and angular kyphosis in paediatric patients.

Structural kyphosis is a posterior convex deformity of the spine that may appear in childhood then worsen with growth, most notably during the pubertal growth spurt. The abnormal curvature may be smooth, defining round kyphosis, or may display a sharp angular pattern. Angular kyphosis is the more severe of the two forms. The main causes of round kyphosis are postural kyphosis and Scheuermann's disease. The spontaneous outcome is favourable, and round kyphosis is well tolerated in adulthood. The treatment relies on orthopaedic methods in the overwhelming majority of cases. Surgery is reserved f

Management of severe spinal deformity: scoliosis and kyphosis.

STUDY DESIGN: Review of the literature and author';s experience with the treatment of severe spinal deformity. OBJECTIVE: To define the anatomic and physiologic challenges in treating severe spinal deformity and to describe the preoperative, intraoperative, and postoperative strategies to achieve the optimal safe result. SUMMARY OF BACKGROUND DATA: Severe pediatric spinal deformity is a relatively uncommon condition that often arises following treatment of early onset scoliosis. Patients most often present with severe clinical and radiographic deformity with poor pulmonary function. In contr

Contemporary management of isthmic spondylolisthesis: pediatric and adult.

BACKGROUND: Isthmic spondylolisthesis is common in pediatric and adult patients. Most cases are asymptomatic. When symptomatic, nonsurgical treatment is an appropriate first step. Surgical treatment of this condition varies depending on patient age, degree of slip, presence of neurologic findings, and degree of deformity. PURPOSE: To review the literature on the management of isthmic spondylolisthesis in pediatric and adult patients. STUDY DESIGN: Review article. METHODS: Literature review. RESULTS AND CONCLUSIONS: Achieving a solid fusion leads to improved functional outcomes and reductio

Implications for treatment of variations in length of the first metacarpal in different types of triphalangeal thumbs.

BACKGROUND/AIM: Cytochalasin B is a mycogenic toxin that preferentially damages malignant cells through multiple mechanisms. The microfilament-disrupting agent inhibits cytokinesis, producing enlarged and multinucleated neoplastic cells without enlarging or producing multinucleated normal cells. In addition, cytochalasin B has been shown to induce apoptosis and to increase the mitochondrial activity of malignant cells. In spite of these pharmacological properties potentially exploitable in cancer chemotherapy, no cytochalasin congener or derivative and indeed no microfilament-directed agent ha

[Surgery of the wrist and hand in rheumatoid arthritis].

Wrist and hand surgery in rheumatoid arthritis has continued to evolve since first being tried fifty years ago. Improvements have flowed from a better understanding of pathophysiological mechanisms, from technical progress and from more effective medical treatment. The use of methotrexate and the subsequent development of biological therapies have transformed the course of the disease, increasing the chances of positive and durable surgical results. Priority must be given to re-alignment of the wrist in order to protect the fingers. It is equally important to perform synovectomies of the extri

Perioperative medical comorbidities in the orthopaedic patient.

Evaluation and management of medical comorbidities in the perioperative period can help improve surgical morbidity and mortality. Perioperative evaluation essentially is risk assessment and minimization. Patients undergoing orthopaedic treatment may benefit from temporizing measures to reduce systemic complications associated with some procedures. Patients at increased risk of cardiac ischemia should undergo risk stratification to determine possible perioperative interventions. Use of perioperative medications and/or consultation with specialists can help to address heart murmurs, bacterial en

Surgical repair of cartilage defects of the patella.

The structure and biomechanical forces on the patellar joint challenges researchers to define an ideal method for resurfacing the patellar cartilage. The articular surface of the patella presents variability between individuals, and has various minor articulations that bear partial or total compressive, shear, and combined forces during movement. Surgical techniques for the repair of patellar cartilage defects have evolved from cumulative advances in basic science and technology. Such surgeries include the techniques that promote either fibrocartilage formation or hyalinelike cartilage formati

One-stage elongation of the third or fourth brachymetacarpia through the palmar approach.

The technique of 1-stage elongation of a short third or fourth metacarpal through the palmar approach was devised to avoid the drawbacks of the conventional procedures through the dorsal approach with or without using an external fixator. This technique was used in the treatment of 3 cases and the results and the technique are described.

[Principles of choice of primary operative techniques for congenital clubfoot].

Review article. A definition of the congenital clubfoot as the rotational and torsional disorder in the sagittal, coronal and horizontal plane was described. The method of choice of the operative technique on the basis of clinical and radiographic parameters was presented. It was found that the primary technique used for surgical clubfoot correction is chosen from a set of different procedures, ranging from posterior release to complete subtalar release. The technique can be extended and modified during surgery.
