Modern orthotics for spinal deformities.

Four basic types of orthoses are now being used at the Twin Cities Scoliosis Center for the treatment of spinal curvatures. The Milwaukee Brace, either custom-made or using a prefabricated pelvic section is the orthosis of choice for thoracic scoliosis and kyphosis in the ambulatory child. For lumbar and thoracolumbar curves, the one-piece "TLSO" has proven the most effective design. For the collapsing spine of myelomeningocele, childhood paraplegia, and spinal muscular atrophy, the 2-piece bivalved polypropylene body jacket is excellent. For the severely involved cerebral palsy or Duchenne Dy

Treatment of Peroneal Nerve Injuries in the Multiligament Injured/Dislocated Knee.

Tibiofemoral knee dislocations are typically a consequence of high-energy mechanisms, causing significant damage to the soft tissue and osse … e been dismal. A period of observation and nonoperative treatment is initially performed, utilizing orthotic devices to assist with lower extremity deficits. Surgical treatment options include neurolysis, nerve grafting, tendon transfer, arthrodesis, and direct motor nerve transfers. Motor nerve transfers continue to be explored with initial reports showing promis …

Post-poliomyelitis sequelae: A pathophysiologic basis for management.

Management of the late sequelae of poliomyelitis is based on their underlying pathophysiology and on a limited number of research reports. The key elements of management including the history, assessment and possible interventions are described in this article. The indications for intervention and its parameters are determined on an individual basis to maximise the benefit-to-risk ratio. Heavy resistive or exhaustive exercise, for example, cannot be supported physiologically as a means of preserving function in most patients. However rest, pacing of activities, lifestyle modification, orthotic

Design and control of a bio-inspired soft wearable robotic device for ankle-foot rehabilitation.

We describe the design and control of a wearable robotic device powered by pneumatic artificial muscle actuators for use in ankle-foot rehabilitation. The design is inspired by the biological musculoskeletal system of the human foot and lower leg, mimicking the morphology and the functionality of the biological muscle-tendon-ligament structure. A key feature of the device is its soft structure that provides active assistance without restricting natural degrees of freedom at the ankle joint. Four pneumatic artificial muscles assist dorsiflexion and plantarflexion as well as inversion and eversi

Limb Salvage in Secondary Chondrosarcoma of the Metatarsus.

Malignant transformation of metatarsal osteochondroma into chondrosarcoma is a rare entity. The most optimal line of treatment, salvage proc … ctional, and aesthetic results. At 1 year postoperatively, the patient was able to walk without any orthotic devices and had returned to his routine work. No evidence of local recurrence or metastasis was observed.

[Hallux rigidus].

Osteoarthritis of the first metatarsophalangeal joint is termed hallux rigidus. Clinical symptoms are expressed by a painful limitation of range of motion, finally leading to an eponymous total stiffness of the joint. Conservative treatment includes drug therapy, physical therapy and orthotic devices like insoles and footwear modification. After making use of the whole range of conservative treatment options, several surgical techniques are available, which should be considered depending on stage, patient's activity level and possible comorbid diseases. Open and arthroscopic techniques address…

A Wearable Open-Source Neuroprosthesis/Neuro-Orthosis for Restoring Hand Function.

This paper presents a wearable, open-source system that combines electromyography (EMG) and functional electrical stimulation (FES) to restore hand function in individuals with disabilities caused by cervical spinal cord injuries or stroke. The device captures electrical signals produced during volitional muscle contractions and analyzes them to interpret the user's intent to move. This information is then used to stimulate impaired muscles, promoting improved hand function and rehabilitation. We detail the design, prototyping, and testing of the system, emphasizing its modularity, affordabili

Adaptive Exoskeleton Device for Stress Reduction in the Ankle Joint Orthosis.

Treating ankle fractures in athletes, commonly resulting from training injuries, remains a significant challenge. Current approaches to managing both non-surgical and postoperative foot and ankle disorders have focused on integrating sensory systems into orthotic devices. Recent analyses have identified several gaps in rehabilitation strategies, especially regarding gait pattern reformation during recovery. This work aims to enhance rehabilitation effectiveness for patients with ankle injuries by controlling load distribution and monitoring joint flexion/extension angles, as well as the reacti…

Long-Term Results After Head Orthosis Therapy for Early Childhood Position-Related Cranial Deformities: Stability of Treatment Outcome and Parental Satisfaction.

The purpose of this study was to determine whether different types of position-related cranial deformities show changes after completion of head orthosis therapy. We investigated how children's age at the begin of molding helmet therapy affects the duration and long-term stability of treatment. In addition, parental satisfaction with helmet therapy has been investigated.Between 2012 and 2019, 47 patients have been analyzed after undergoing helmet therapy. At the beginning of treatment different disease pattern were classified. Posterior deformational Plagiocephalus defined by a cranial vault i

Applied ergonomics: adaptive equipment and instrument modification for musicians.

Failure or difficulty in treating musicians' overuse injuries is often due to overlooking aggravating roles of activities of daily living. Numerous devices are available for people with limited or painful hand function to protect joints and minimize stress of daily activities. The occupational therapist can evaluate the patient and determine which adaptations are appropriate. Instruments may be modified to render them more ergonomic and less likely to cause injuries; this is especially true for wind instruments. Biomechanically and ergonomically correct seating can minimize back and neck strai

Management of the diabetic foot.

The increasing prevalence of diabatic foot ulcers (DFUs) is not only costly, but carries a large mortality burden. In this article, we discuss important traditional concepts in the management of DFUs and elaborate on how new technologies have expanded our ability to treat DFUs effectively. New supplies and wound care products have been developed to target the following traditional areas of focus: tissue, infection/inflammation, moisture, and edge. Offloading strategies have grown from standard orthotics or insoles to total contact casting and three-dimensional-printed orthotics to produce the

Orthotics and taping in the management of vertebral fractures in people with osteoporosis: a systematic review.

OBJECTIVE: To establish the current evidence base for the use of orthotics and taping for people with osteoporotic vertebral fracture (OVF). … ndings were mixed. The quality of studies was limited. CONCLUSIONS: The current evidence for using orthotic devices or taping for people with OVF is inconsistent and of limited quality and therefore careful consideration should be taken by clinicians before prescribing them in practice. SYSTEMATIC REVIEW REGISTRATION NUMBER: CRD42015020893.

Quality and user satisfaction scores for prosthetic limbs provided in a fitting camp, Cambodia.

OBJECTIVE: To evaluate prosthetic devices delivered during an intensive fitting camp in Cambodia, considering device quality and client satisfaction. METHODS: We conducted an observational cohort study in which we assessed prosthetic devices produced and delivered by an international nongovernmental organization at an intensive fitting camp. We conducted our assessment in two stages: at prosthetic device provision using a checklist to assess device quality and client satisfaction at discharge, and at 3-month follow-up using a telephone interview to assess the client's device usage and prefere
