[From reconstruction to function : Hands-on training in tympanoplasty using real-time feedback].

BACKGROUND: Surgery of the middle ear requires a very high level of fine motor skills. Due to the increased potential for complications in middle ear operations, it is appropriate to acquire these skills beforehand by operating on a model. OBJECTIVE: How satisfactory is the training on suitable models? Are the skills acquired from working on a model transferable to intraoperative situations? Will the type of model and its use for training influence ear surgery in the future? MATERIAL AND METHODS: Available publications and own experiences with the Dresden tympanoplasty model (DTM) are analyz

Training model for salvage procedures in endoscopic stapes surgery.

PURPOSE: The learning curve in endoscopic stapes surgery, is flat and long. There is limited space during training for advanced and salvage procedures, which could be necessary in unexpected intraoperative scenarios. The aim of the present study is to develop an ex-vivo animal model for training in salvage and advanced procedures in endoscopic stapes surgery METHODS: After defining the difficult intraoperative scenarios requiring a salvage procedure during stapes surgery, we used the ovine model to create those intraoperative situations. After assessing the suitability of the model for that pu

Feasibility of ossicular chain reconstruction with resin cement.

INTRODUCTION: Disjunction of ossicular chain is a common finding in middle ear chronic disease. In addition to ossicular interposition, various materials have been used for reconstruction, such as ceramic prostheses, polyethylene, and titanium. OBJECTIVE: Because of the high cost of the available options, the authors propose to reconstruct the ossicular chain with resin cement, a material typically used in dental reconstruction and fixation. METHODS: Two anatomical parts of the temporal bones were used, creating a disjunction of the ossicular chain between the incus and staples and then reco

[Implantable Hearing Devices].

Combined hearing loss is an essential indication for implantable hearing systems. Depending on the bone conduction threshold, various options are available: Patients with mild sensorineural deafness usually benefit from transcutaneous BCI, while percutaneous BCI systems are recommended also for moderate hearing loss. For combined hearing loss with moderate and high-grade cochlear hearing loss, active middle ear implants are recommended. For patients with incompatibilities or middle ear surgery, implants are a valuable and proven addition to the therapeutic options.

The Vibrant Soundbridge® middle ear implant: A historical overview.

OBJECTIVE: To present a historical overview of the Vibrant Soundbridge® (VSB) middle ear implant (MEI), since its beginning in the 1990s to date and to describe its course and contemplate what it might become in the future. HISTORY: MEIs started to take form in researchers' mind in the 1930s with the first experiment of Wilska. In the 1970s, several devices, such as the Goode and Perkins', the Maniglia's, or the Hough and Dormer's were created but remained prototypes. It is only in the 1990s the devices that emerged remained on the market. In 1994, Symphonix, Inc. was created and aimed to man

Design of a resilient ring for middle ear's chamber stapes prosthesis.

This paper presents the process of designing a new elastic element replacing a membrane in the chamber stapes prosthesis (ChSP). The results of the study are volume displacement characteristics obtained for the prosthesis and physiological stapes. Simulation tests on a 3D CAD model have confirmed that a properly designed ring can stimulate perilymph with the same or greater efficacy as the physiological stapes footplate placed on the elastic annular ligament. The ChSP with a new elastic element creates a good chance of improving hearing in patients suffering from otosclerosis.

[Minimally-invasive incision using the implantable Hearing device Carina].

OBJECTIVE: The skin incision for cochlear implants and active middle ear implants like Vibrant Soundbridge and Bonebridge has evolved toward a minimally invasive access in recent years. For the active middle ear implant CARINA a significantly larger incision is recommended, which corresponds to that of Cochlear Implant care nearly two decades ago.As a result, increased complication rates due to extended incision can be expected. METHODS: In order to minimize the complication rate for the active middle ear implant CARINA the possibility for minimally invasive incisions was reconsidered and deve
