The skull may mirror many systemic diseases or conditions. In fact, when gross and microscopic pathologic findings are confusing, proper analysis of the skull radiograph may lead to a more accurate and complete diagnosis. The article discusses some of the most common systemic diseases with skull or calvarial manifestations (fibrous dysplasia, hyperparathyroidism, histiocytosis X, Paget's disease, and anemias), with special attention to pathophysiology.

PURPOSE OF THE STUDY: Bifocal proximal and distal avulsion of the patellar tendon is a rare event in adults with, to our knowledge, no previ … in adults. It is usually associated with an underlying bone disease (Paget's disease, osteomalacia, osteitis deformans) or systemic disease (lupus erythematosus, rheumatoid arthritis, hyperparathyroidism, Wilson's disease), but bilateral spontaneous disruption may occur. There have been two reports of patellar full-thickness tendon tears in diabetic patients but no …

Non-neoplastic bone lesions represent a heterogeneous group of conditions that stem from reactive, genetic, or metabolic processes. These lesions can sometimes closely mimic malignant tumors, posing diagnostic challenges. This article focuses on a few non-neoplastic bone lesions and their differential diagnoses, including florid reactive periostitis, Paget disease of bone, fractures (including compression/stress fractures), and melorheostosis, each of which exhibits distinct clinical, radiological, and pathologic features. Unlike neoplastic processes of bone, no specific immunohistochemical fe

Hereditary spastic paraplegia (HSP) is a neurodegenerative disorder clinically characterized by slowly progressing spastic paraparesis. We herein report a 50-year-old Japanese woman who presented with slowly progressing spastic paraplegia and a history of Paget's disease of bone (PDB). Genetic testing revealed a mutation of the Valosin-containing protein (VCP) gene (p.Arg155Cys; c.436C>T). This mutation has not been reported to cause HSP with PDB.

Various forms of bony deformations and dysplasias are often present in the facial skeleton. Bone defects can be either localized or general. Quite often they are not only present in the skull but also can be found in other parts of the skeleton. In many cases the presence and levels of specific bone markers should be measured in order to fully describe their activity and presence in the skeleton. Fibrous dysplasia (FD) is the most common one in the facial skeleton; however, other bone deformations regarding bone growth and activity can also be present. Every clinician should be aware of all co

Isolated involvement of calcaneus with Paget's disease of bone is highly unusual, with very few cases reported in the world literature. We present herein a unique 69-year-old male patient with monostotic Paget's disease of the left calcaneus discovered incidentally during a whole body bone scintigraphy. Since most bone scans are performed for evaluating osseous metastases, differentiating Paget's disease from overlapping metastases may be of significant clinical value. We discuss further the importance of whole body bone scan and other imaging modalities in establishing the correct diagnoses i

Lumbar spinal stenosis is a common condition in elderly patients and also one of the most common reasons to perform spinal surgery at an advanced age. Disc degeneration, facet degeneration and hypertrophy, and ligamentum flavum hypertrophy and calcification usually participate in the genesis of a stenotic condition in the elderly. These changes can lead to symptoms by themselves or decompensate a preexisting narrow canal. Although some lesions are more central or more lateral, this classic dichotomy is less present in the elderly patient, in whom the degenerative process usually encroaches bot

Ipriflavone, an isoflavone synthesized from the soy isoflavone daidzein, holds great promise in the prevention and treatment of osteoporosis and other metabolic bone diseases. It has been widely studied in humans and found effective for inhibiting bone resorption and enhancing bone formation, the net result being an increase in bone density and a decrease in fracture rates in osteoporotic women. While ipriflavone appears to enhance estrogen's effect, it does not possess intrinsic estrogenic activity, making it an attractive adjunct or alternative to conventional hormone replacement therapy. Paget's disease is a common entity but osteosarcoma arising in Paget's disease is an infrequent complication. Primary osteosarcomas are typically long bone tumours and the skull is rarely affected. A case with malignant transformation of Paget's disease of the skull and an unusual clinical presentation is reported. Computed tomography scans of the tumour area show interesting results. Main features are: an intensive sclerotic reaction of the host bone, an inner and outer spiculation of the calvarium, radial ossifications in the soft extracranial mass, and spotty calcifications and ossification

The authors illustrate a case report of Paget's disease in an exclusively cranio-facial localisation. Paget's disease is a dystrophy of an osteo-condensing type. Excess bone remodelling activity, due primarily to hyperclastic hyperactivity, leads to accelerated resorption and excess tissue formation. This marked alteration of the metabolism causes the hypertrophy of the bones involved, with zones of greater density and increased vascularisation. Its clinical manifestations include morphological deformations and painful symptoms in the areas affected. Complications may include pathological frac

Osteoma is the commonest benign tumour of the paranasal sinuses (Handousa, 1952). A case of a large osteoma removed from the frontal and ethmoid sinuses is presented. Histopathological examination of this tumour revealed evidence of bony remodelling similar to that seen in Paget's disease which has not been previously reported. The pathological significance of this finding is discussed.
