Appropriateness of clinical severity classification of new WHO childhood pneumonia guidance: a multi-hospital, retrospective, cohort study.

BACKGROUND: Management of pneumonia in many low-income and middle-income countries is based on WHO guidelines that classify children accordi … associated with death of children retrospectively classified as having non-severe pneumonia: severe pallor (adjusted risk ratio 5·9, 95% CI 5·1-6·8), mild to moderate pallor (3·4, 3·0-3·8), and weight-for-age Z score (WAZ) less than -3 SD (3·8, 3·4-4·3). Additional factors that were independently associated with death were: WAZ less than -2 to -3 SD, age …

Evolution of optic nerve and retina alterations in a child with indirect traumatic neuropathy as assessed by optical coherence tomography.

Herein, we describe the case of a 4-year-old child with indirect traumatic optic neuropathy and serial changes of the optic nerve head and retinal nerve fiber layer (RNFL) documented using optical coherence tomography (OCT). Visual acuity improved despite progressive RNFL thinning and optic disc pallor. We concluded that OCT may be useful for monitoring axonal loss but may not predict the final visual outcome.

Optic neuropathy due to brain abscess.

A 26-year-old man went blind as part of a multifocal central nervous system disease. Bilateral optic nerve head pallor developed four weeks later. There had been no papilledema. In this setting, the appearance of optic atrophy without preceding papilledema in part led to the clinical diagnosis of severe disseminated encephalomyelitis. At autopsy multiple brain abscesses were found, including an area of inflammation within the chiasm.

Abdominal tuberculosis with autoimmune hemolytic anemia.

An eight-year-old male child presented with progressive distension of abdomen, fever, pallor and jaundice with a history of tubercular contact. Investigations were suggestive of abdominal tuberculosis with autoimmune hemolytic anemia. The child responded well to a course of oral steroids with antitubercular treatment. A literature search did not reveal any previous case report of an association between tuberculosis and autoimmune hemolytic anemia.

Retinal masqueraders of optic nerve disease.

Electrophysiologic testing of the visual system and ancillary imaging investigations may be crucial aids for correctly diagnosing patients with unexplained visual loss or isolated disc pallor. These patients may harbor a retinal disease process and not an optic neuropathy. A diverse range of retinal conditions may present in this manner, and misdiagnosis may lead to unnecessary neurologic investigation and subsequent delay in therapeutic intervention.

Splenic hemorrhage in a neonate of a mother on anticonvulsant therapy.

A 24-hour-old male infant was referred to the Eastern Maine Medical Center (EMMC) because of pallor, lethargy, and abdominal distention. Diagnostic abdominal paracentesis revealed free blood. At surgery hemisplenectomy removed the damaged part of the spleen followed by complete recovery of the infant. A liver-spleen scan, performed before discharge, revealed normal uptake in the remaining portion of the spleen.

[Familial lymphohistiocytosis. Anatomopathological study of 2 cases detected neonatally].

Two brothers presented with hemophagocytic reticulosis with neonatal onset. Early clinical and biological symptoms included pallor, hepatosplenomegaly and anemia, thrombocytopenia. Evolution was lethal in both cases, at 5 months and 13 days of age, respectively. Diagnosis was confirmed in both by the pathologic findings: diffuse lymphohistiocytic cellular proliferation with hemophagocytosis and atrophy of the lymphoid tissue. The diagnostic difficulties of this disease in the neonatal period are emphasized.

Black Holes in the Brain and Spine: A Dark Disease.

A 7-year-old girl presented with difficulty in walking and bilateral vision impairment since past 15 days. On examination, she had disc pallor, flaccid paraparesis with positive Babinski sign, and reduced sensations below clavicle. She was diagnosed as anti-aquaporin-4 (AQP-4)-positive neuromyelitis optica. This article emphasizes the importance of recognizing its classical neuroimaging findings distinct from other disorders.

Kwashiorkor-like zinc deficiency syndrome in anorexia nervosa.

This report deals with a 26-year-old white woman exhibiting signs of both Kwashiorkor (marasmus, pallor, hypopigmentation of hair and hepatomegaly) and acrodermatitis enteropathica (eczematous dermatitis predominantly on acral areas). Clinical and laboratory examinations excluded malabsorption syndrome and glucagonoma syndrome and revealed hypoproteinemia and marked zinc deficiency. Psychiatric examination disclosed anorexia nervosa. Substitution therapy led to rapid clearing of the skin lesions.

Macular ganglion cell complex measurement in bilateral retrobulbar optic neuropathy without a relative afferent pupillary defect.

PURPOSE: This study aimed to demonstrate the clinical usefulness of measuring the macular ganglion cell complex (GCC) for the early detection of axonal loss in eyes with bilateral retrobulbar optic neuropathies. PATIENTS AND METHODS: We retrospectively reviewed the medical records of three patients with bilateral toxic, ischemic, or infiltrative retrobulbar optic neuropathy. RESULTS: No relative afferent pupillary defect was detected in any patients. The results of the fundus examinations were unremarkable at the initial presentation except for slight optic disk pallor in the right eye of Ca…

Autoimmune Retinopathy.

The autoimmune retinopathies (AIRs) are a group of inflammatory-mediated retinopathies that present with unexplained visual loss (both central and peripheral), visual field defects, usually a ring scotoma, photoreceptor dysfunction as evident on electroretinography (ERG), and circulating autoantibodies against retinal antigens. The fundus may be normal or may show vascular attenuation, retinal atrophy with or without pigmentary changes or waxy pallor of the optic disc, and no or minimal inflammatory cells.

Anemia due to massive chronic foetomaternal hemorrhage.

We report a case of massive chronic foetomaternal hemorrhage. The labor course was uncomplicated. The newborn presented with pallor. tachypnea, and moderate hepatosplenomegaly. The initial hemoglobin was 6.5 g/dl. The Kleihauer-Betke stain on a maternal blood sample was 12%, which is equivalent to 540 ml of fetal blood in the maternal circulation. A clot in the umbilical vein was demonstrated sonographically. The possible association of foetomaternal hemorrhage with umbilical vein thrombosis is discussed.

Trauma-induced altitudinal hemianopsia.

A 55-year-old Caucasian female presented with bilateral inferior altitudinal visual field defects which resulted from trauma suffered in an automobile accident. In addition, there was a bilateral reduction in visual acuities and bilateral optic disc pallor. Diagnostic and historical data suggested that the primary site of damage was at the occipital cortex. A subsequent bilateral anterior ischemic optic neuropathy process was also thought to have occurred.

Measurement of the optical density of the optic nerve head II: short-time fluctuations of the optical density of the nerve head.

The fluctuation of pallor with increasing IOP could be due to either insufficient reliability of the method or to an unrecognized pulsating phenomenon. A better arrangement should eliminate imperfection of the method. Coupling the release with the ECG should reveal a relationship to the arterial pulse rate. Neither condition could be demonstrated as the reason for the fluctuation. The connection of the fluctuation and its possible relationship to central venous pressure is discussed. The margin of error is shown, which is 9% on average.
