BACKGROUND: 22q11 deletion syndrome (22q11DS) is characterized by conotruncal cardiac defects and hypoplasia of parathyroid glands and thymus, which result in variable hypoparathyroidism (HPT) and immune deficiency.
METHODS: To study the course of HPT and the spectrum of other associated manifestations we evaluated all patients with 22q11DS, confirmed by fluorescence in situ hybridization, and HPT who were under follow-up at the Calcium-bone clinic, The Hospital for Sick Children, Toronto. Patients were clinically assessed and their hospital records were reviewed.
RESULTS: Eighteen patients …

Current views on the molecular aspects of familial parathyroid gland diseases have been presented (familial primary hyperparathyroidism, hypoparathyroidism and psuedohypoparathyroidism). Their inherited mode and genetic abnormalities have been described. Particularly, the following genes: HRPT2, MEN1, RET, CASR, GNAS have been shown. Localization, structure, expression and structural changes (mutations) found in patients with familial parathyroid gland diseases have been presented. Attention has been paid to clinical and histopathologic symptoms, which should indicate the need to undertake gen

Bilateral neck exploration is the standard procedure of primary hyperparathyroidism. Preoperative localization is not mandatory. Intraoperative visualization methods help to avoid unsuccessful explorations and to reduce morbidity rates. In our study, the application of 5-Aminolevulinic acid (5-ALA), a precursor in the heme biosynthesis pathway, led to Protoporphyrin IX (PpIX) accumulation within the parathyroid gland (PTG). PpIX, a metabolite of 5-ALA, causes red fluorescence (635 nm) when stimulated by light of a defined wavelength. Largely invisible under conventional illumination, all PTGs

Neonatal hyperparathyroidism (NPHP) is exceedingly rare and often fatal. A neonate is presented with a serum calcium concentration of 33 mg/dL, an intrathyroid parathyroid gland, and a family history of hypocalciuric hypercalcemia (FHH). She underwent successful total parathyroidectomy. Six years later, the child is normocalcemic and developmentally normal, requiring calcium and calcitrol replacement. The results of this case support the concept that NPHP is associated with parathyroid hyperplasia and is part of a continuum that includes FHH.

We report a case of a 50-year-old man with hyperparathyroidism secondary to chronic renal failure who underwent extirpation of a mediastinal ectopic parathyroid gland by a transcervical approach under mediastinoscopy. This procedure provides an excellent approach to the mediastinal ectopic parathyroid gland, and is less invasive than median sternotomy or thoracotomy.

Hyperparathyroidism is a common finding in patients with renal failure. Phosphorus retention is known to be an important factor in the development of secondary hyperparathyroidism. Exciting new work has demonstrated that a high extracellular phosphorus concentration directly stimulates parathyroid hormone secretion and synthesis. Dietary phosphorus also modulates parathyroid function indirectly by decreasing calcitriol production, and it interferes with the calcaemic response to parathyroid hormone. The information available suggests that the control exerted by phosphorus is critical, via indi

Chromogranin A (CgA) is a major protein of the parathyroid gland that is costored and cosecreted with parathyroid hormone (PTH). CgA, widely distributed in endocrine and neuroendocrine cells in addition to parathyroid cells, appears to be a precursor of biologically active autocrine or paracrine peptides that include pancreastatin (CgA240-288) and parastatatin (CgA347-419), compounds that strongly inhibit stimulated secretion of PTH and costored CgA. The biosynthesis of CgA and PTH via gene expression and mRNA translation are noncoordinately altered by agents that affect parathyroid cell secre

The rate of occurrence of intrathyroidal parathyroid glands in patients treated surgically for hyperparathyroidism (HPT) varies across studies. Among 1200 consecutive patients who underwent surgery for HPT at the Lille Hospital, France, between December 1965 and July 1992, 43 (3.6%) had 47 histologically-proven intrathyroidal parathyroid glands, including 44 lower and three upper parathyroid glands. Fifteen patients had a thyroid gland abnormality. Of the 14 patients who had a preoperative ultrasound study, eight exhibited an abnormal image. Thirty-four of the 47 intrathyroidal parathyroid gla…

Since Sandström reported the first detailed description of the parathyroid glands of human beings in 1880, and Lever first described the ultrastructure of the parathyroid chief cells of rat in 1957, a large number of light and electron microscopic studies have been done on the parathyroid glands of numerous animal species under normal and experimental conditions. This review deals with the comparative morphology of the parathyroid glands in amphibians, reptiles, birds and mammals under normal conditions, and the problem concerning the effects of various experimental conditions on the parathyro…

Parathyroid carcinoma occurs in 0.1 to 5% as the cause of primary hyperparathyroidism (HPT). It is difficult to determine the true incidence, because parathyroid carcinoma is diagnosed too often due to unreliable histologic criteria. It is only justified to make the diagnosis, when a local recurrence or metastases with the clinical picture of a recurrent or persistent HPT occurred. Treatment of choice is the initial en-bloc resection, which may result in long disease-free intervals. Pharmacological treatment, chemotherapy, and radiation are mostly ineffective in the treatment of parathyroid ca

The concept that the PTH-calcium curve is representative of parathyroid function has been discussed. Comparisons of parathyroid function have been made between normal humans and hemodialysis patients and also between hemodialysis patients with different forms of renal osteodystrophy. From these comparisons, it is apparent that the magnitude of HPT is much greater in patients with renal failure than in normal humans, and as represented by the ratio of basal to maximal PTH, the parathyroid gland appears to be stimulated at basal serum calcium levels in hemodialysis patients. Similarly, based on

Primary hyperparathyroidism is caused by defects in the parathyroid gland. Investigations have implicated three interesting genes whose mutation can cause primary hyperparathyroidism. Familial hypocalciuric hypercalcemia is believed to be an atypical form of primary hyperparathyroidism with an inherited defect in calcium recognition expressed not only in all parathyroid chief cells (thus a polyclonal defect) but in some renal tubular cells as well. In typical primary hyperparathyroidism a monoclonal parathyroid tumor is usually the central cause. Either of two apparently different genes on the

The effects of a single oral dose of phosphate (780 mg of elemental phosphorus as 100 drops of Phosphoneurol 120 from DOMS-Adrian Laboratories) on parathyroid function and on calcium and phosphate metabolism were studied in 10 young adults. The parathyroid response was checked by measurement of PTH 1-84 and of nephrogenous cAMP before and after a 10 days therapy by phosphate (780 mg of elemental phosphorus twice daily). Before treatment a 16 per cent increase in PTH 1-84 (p less than 0.05) and a 34 per cent increase in nephrogenous cAMP (p less than 0.02) was obtained. After treatment the incr

From July 1973 to July 1984, 50 patients underwent parathyroidectomy because of secondary hyperparathyroidism due to chronic renal failure. One hundred fifty-eight excised parathyroid glands in 41 of these patients were submitted to the histopathological examinations in the relations to their clinical features which revealed the following findings. Inspite of secondary hyperparathyroidism, eleven of 41 patients showed four unevenly enlarged parathyroid glands with one or two glands weighing less than 100mg. The glands were divided into three adenomas, 101 diffusely and 54 nodulary hyperplastic…

The sonographic morphology of adenomas is studied on the basis of 7 sonographically identified and surgically confirmed adenomas of the parathyroid gland, taking the available literature into consideration. Information is given on differential diagnosis against normal anatomical structures and pathological processes. A correlation is established between sonomorphological and pathologico-anatomical findings.

The paper describes a case history of a 64-year-old woman who had suffered over no less than 2-year period from an unusual hormonally active tumour of the parathyroid gland associated with signs characteristic of Reklinghausen's disease. The tumour presented a node 4 cm in diameter. Histologically it showed pronounced atypia and polymorphism of the cells of the chief cell type and eosinophilic cells. Grimelius silvering revealed not numerous cells with argyrophil granularity in the cytoplasm. No mitoses were detected but tumour cells grew into the capsule of the node. No metastases were found
