Predictors of survival in parotid adenocarcinoma not otherwise specified: a National Cancer Database study of 3155 patients.

BACKGROUND: Parotid adenocarcinoma not otherwise specified (PANOS) is a common parotid cancer, but studies specifically on this subject are limited. METHODS: We conducted a retrospective review of the National Cancer Database. RESULTS: Ten percent of all parotid cancers were PANOS (n = 3155). Median age was 67 years. Most patients were men (62.8%) with high-grade histology (67.2%). Regional metastasis was common (35.9%), whereas occult nodal metastasis (20.2% overall) was less frequent in non-high-grade lesions (8.5% vs 31.6%; p < .001). Distant metastasis was rare (7.9%). Five-year overall

Bilateral myoepithelioma of the nasopharynx: a case report.

Myoepithelioma is a rare salivary tumour which is usually encountered in the parotid gland. Outside the parotid, myoepithelioma may arise from minor salivary glands of the hard palate and less frequently in bronchi or breast. In this report we describe the unique case of a metachronous bilateral nasopharyngeal myoepithelioma arising from the tubaric regions. Its microscopic features, immunophenotype, and the differential diagnosis together with a review of literature are discussed.

Bilateral mucosa-associated lymphoid tissue lymphoma of the parotid gland.

Mucosa-associated lymphoid tissue (MALT) tumors of the parotid gland are extranodal non-Hodgkin lymphomas. Stage I and II MALT tumors are us … tis, which are caused by irradiation. Radiotherapy does not produce a scar or an indentation at the parotid region, however, and results in a better cosmetic appearance. Therefore, we recommend open biopsy with facial nerve monitoring and subsequent irradiation in cases in which bilateral prominence of the parotid glands and suspicion of a MALT lymphoma are both …

Benign Lymphoepithelial Cyst: An Unusual Cause of Parotid Swelling in Two Immunocompetent Patients.

BACKGROUND: Lymphoepithelial cysts, which are benign and slow-growing tumours, usually involve the head and neck regions. Benign lymphoepithelial cysts (BLECs) are the most common cause of parotid swelling in human immunodeficiency virus (HIV)-positive patients and are less common in immunocompetent patients. CASE PRESENTATION: Here, we present two cases of immunocompetent patients with long-standing, progressively enlarging parotid swelling. Postoperative histopathological examination of these patients revealed features of BLEC. CONCLUSION: Wide surgical excision is the gold standard for tr

[Metastasizing basalioma of the temporal area or continuous extension? Surgical approach and results].

By means of histological sections the rare diagnosis of a basalioma of the temporal area with regional metastases in the parotid gland and in the neck could be verified. Subsequently the surgical treatment and the postoperative results are described. The operation consisted of an en bloc excision of the basalioma, exstirpation of the parotid gland and radical neck dissection. Since the tumor had also infiltrated the facial nerve, the nerve had to be sacrificed and reconstructed using the superficial peroneal nerve. In order to obtain immediately a satisfactory esthetic result in the face regio

The incidence of Frey syndrome and treatment with botulinum toxin in the Central Denmark Region.

OBJECTIVES: Frey syndrome (FS) is a common complication to large salivary glands surgery. This study primarily aims to describe the incidence of FS among parotid surgery patients in the Central Denmark Region. The secondary aim is to describe predisposing characteristics to syndrome development and the effect of treatment with botulinum toxin (Botox) injection. METHODS: This is a retrospective qualitative study spanning the years 2015-2020. Data on patients diagnosed with FS after parotid surgery with symptoms severe enough to require Botox was extracted from electronic patient records. Incid

Apparent diffusion coefficient mapping of the normal parotid gland and parotid involvement in patients with systemic connective tissue disorders.

PURPOSE: We hypothesized that a difference in restricted diffusion would exist in patients with connective tissue disorders (CTD) as compared with those without CTD. Our purpose was to determine whether the apparent diffusion coefficient (ADC) measurement could be used to identify parotid abnormalities in patients with CTD. METHODS: One neuroradiologist, who was unaware of patient histories, retrospectively measured the ADC values for the parotid glands in 121 patients who underwent clinically indicated brain MR imaging in which the parotid glands were sufficiently depicted. Facial Nerve Neurolymphomatosis That Extends to Both the Brainstem and Extracranial Regions.

A 73-year-old female developed right facial paralysis of House-Brackmann (H-B) grade III and was diagnosed with Bell's palsy. After three months of steroid therapy, she developed progressive hearing loss, and an MRI revealed a tumor in the right internal auditory canal. Within a few months, the right facial nerve palsy recurred, and the patient was treated with Gamma Knife radiosurgery. The tumor in the irradiated region disappeared, but new dysphagia was observed, and a right parotid gland tumor was detected for the first time. Tumors of the right parotid gland and the digastric muscle of the

Patterns of lymph node spread of cutaneous squamous cell carcinoma of the head and neck.

BACKGROUND: Among patients with cutaneous squamous cell carcinoma (SCC) of the head and neck, recent studies have shown that those with involvement of the parotid gland also have a high incidence of neck node involvement. Treatment of the neck by either surgery or radiotherapy is therefore recommended among patients with parotid SCC, even if clinical examination is negative. The aim of this study was first to analyze patterns of metastatic spread in the parotid and cervical lymph nodes and then to correlate the pattern of involved nodes with the primary cutaneous site in order to guide the app

First Bite Syndrome Following Rhytidectomy: A Case Report.

BACKGROUND: First bite syndrome (FBS) is a known complication of parotid gland resection, parapharyngeal space dissection, and cervical sympathetic chain injury. It can be described as severe cramping or spasms in the parotid region triggered by the first bite of a meal, with the pain lessening during each subsequent bite. Although dissection for a rhytidectomy is in the vicinity of the parotid parenchyma, face-lift is not typically characterized as a procedure that can lead to FBS. CASE DESCRIPTION: A 53-year-old female underwent a deep plane face-lift to address her goals of improving jowls…

Tissue-specific expression in the salivary glands of transgenic mice.

Using a DNA construct, named Lama, derived from the murine parotid secretory protein (PSP) gene, we have obtained salivary gland specific gene expression in transgenic mice. Lama is a PSP minigene and allows analysis of the PSP gene 5' regulatory region by transgenesis. We show here that the regulatory region included in Lama with 4.6 kb of 5' flanking sequence is sufficient to direct expression specifically to the salivary glands. The expression level in the parotid gland is only about one percent of the PSP mRNA level, while that of the sublingual gland is near the PSP mRNA level. This sugge

Benign lymphoepithelial lesions of the parotid.

Persistent generalized lymphadenopathy has been well described in patients with seropositivity to the human immunodeficiency virus (HIV). Moreover, isolated enlargement of the parotid gland and parotid lymphadenopathy have been noted much more frequently over the past few years. Histologically, these lesions demonstrate follicular hyperplasia, cystic dilatation of the ducts lined by pseudo-stratified squamous epithelium, and lymphocytic infiltrates. They are generally considered to be benign lymphoepithelial lesions of the parotid or hyperplastic periparotid lymph nodes. The relationship of th

Testicular seminoma metastasizing to the parotid gland: report of a case.

BACKGROUND: Among salivary glands, the parotid gland is the most common site for secondary neoplastic involvement, and carcinomas and melanomas are the two most common secondary tumors. METHODS: We present a case of testicular seminoma metastasizing to the parotid gland. The patient, who had a remote history of testicular seminoma, was initially seen with a parotid mass. A primary parotid lesion was anticipated, and excisional biopsy was performed. The diagnosis of testicular seminoma metastasizing to the parotid gland was established on the basis of histologic findings and was confirmed by i

Parotid gland involvement as an initial presentation of papillary thyroid carcinoma.

Papillary thyroid carcinoma (PTC) is the most differentiated malignant thyroid neoplasm. Local metastases of PTC commonly occur in the regional lymph nodes, while distant metastases are mainly to the lung and bone. The case of a patient with PTC who presented with swelling of the parotid gland and neck, mimicking a primary parotid neoplasm, is presented herein. This case is unique due to the unexpected initial presentation of PTC with no signs of disease in the thyroid gland.

Direct coaptation of extensive facial nerve defects after removal of the superficial part of the parotid gland: an anatomic study.

BACKGROUND: In the surgical repair of facial nerve paralysis, a tension-free end-to-end coaptation of the trunk or its branches with or without rerouting is functionally superior to grafting. Assuming that a lengthening of all branches of the parotid plexus can be attained by removal of the superficial part of the parotid gland and mobilization of the branches, we performed an anatomic study. METHODS: The parotid regions of 10 cadavers were dissected to investigate the length gained for the branches of the parotid plexus by this technique. Every branch at the upper and ventral border of the g…

Outcomes of treatment of mammary analogue secretory carcinoma of the parotid gland.

Mammary analogue secretory carcinoma (MASC) of the salivary gland is a newly-described, rare, malignant tumour. Few patients present with MASC of the parotid gland, so the exact characteristics, outcomes of treatment, and prognosis are unknown. The aim of this study was to analyse the clinical features and outcomes of treatment of MASC of the parotid gland in patients being treated at a single hospital. Five patients with histopathologically-confirmed MASC of the parotid gland between January 2015 and August 2018 were retrospectively enrolled. In all cases preoperative fine-needle aspiration c
