PURPOSE: This report aims to present a case of the retinal nerve fiber layer (NFL) thinning, in which the progression of thinning was slowed following optic nerve-carotid artery decompression surgery.
OBSERVATIONS: A 41-year-old ophthalmologist incidentally noticed a generalized thinning of NFL in his left eye using optical coherence tomography (OCT). Five years earlier, the OCT showed only mild thinning. While visual acuity and visual fields were normal, the patient reported a blurriness. The MRI revealed an ectatic and tortuous left internal carotid artery compressing the left optic nerve.

A male patient in his 60s presented with a rare form of hyper-mature senile cataract, characterised by an empty capsular bag in the left eye and diminished vision over 2 years. Initial visual acuity was 20/40 in the right eye and finger counting at one metre in the left eye. A slit lamp examination revealed pseudophakia in the right eye and a fibrosed lens capsule in the left eye. A dilated examination confirmed an empty capsular bag with a fibrosed capsule and normal fundus. Ultrasound findings were anechoic, and anterior segment optical coherence tomography showed an intact lens capsule. The

OBJECTIVES: This study aims to compare the outcomes of cataract surgery using Patient reported outcome measures (PROM) between a general tertiary hospital setting and an outreach cataract centre. PROM reports the benefits of surgery from the point of view of the patient instead of a clinical evaluation, which reduces the biases by practitioners.
METHODS: Prospective Cohort Analysis. Patients were recruited at Hospital Selayang and The Hospital Selayang-MAIWP Cataract Care Centre (Pusat Pembedahan Katarak Hospital Selayang-MAIWP or PPKM in Malay) prior to their cataract surgery between 1 July

IMPORTANCE: The introduction of large language models (LLMs), such as Generative Pre-trained Transformer 4 (GPT-4; OpenAI), has generated significant interest in health care, yet studies evaluating their performance in a clinical setting are lacking. Determination of clinical acuity, a measure of a patient's illness severity and level of required medical attention, is one of the foundational elements of medical reasoning in emergency medicine.
OBJECTIVE: To determine whether an LLM can accurately assess clinical acuity in the emergency department (ED).
DESIGN, SETTING, AND PARTICIPANTS: This

Diabetic foot infections (DFIs) are common and represent the leading cause for hospitalization among diabetic complications. Without proper management, DFIs may lead to amputation, which is associated with a decreased quality of life and increased mortality. However, there is currently significant variation in the management of DFIs, and many providers fail to perform critical prevention and assessment measures. In this review, we will provide an overview of the diagnosis, management, and discharge planning of hospitalized patients with DFIs to guide hospitalists in the optimal inpatient care

BACKGROUND: Hyperlipidemia is an identified factor of premature vessel atherosclerosis. Lipemia retinalis is an unusual retinal manifestation of hyperlipidemia and is thought to be directly correlated with the serum triglyceride level.
CASE REPORT: This paper discusses the case of a 55-year-old patient with lipemia retinalis, which deteriorated his visual acuity. The patient had an extremely high serum cholesterol level (1053 mg/dl) and a very high level of triglycerides (1513 mg/dl). The normalization of serum lipids, reversion of retinal vessels alterations and visual acuity improvement was

A 69-year-old patient presented to us with traumatic mydriasis with irregular pupil measuring 7 mm, with superior loss of iris tissue and large inferior peripheral iridotomy and pseudophakia. The patient had history of blunt trauma 3 years ago in a fire cracker injury. He was operated elsewhere primarily after the trauma for cataract surgery with intraocular lens implantation and had suboptimal visual outcome with glare and photophobia. He presented to us with irregular pupil and inferior iridectomy with pseudophakia. The uncorrected visual acuity was 20/150 improving to 20/50 with glasses. He

We report a case of a 42-year-old woman who previously underwent femtosecond-assisted myopic laser-assisted in situ keratomileusis (LASIK) with uncorrected distance visual acuity (UDVA) 20/20- in both eyes (OU). The patient presented three months later following a motor vehicle accident (MVA), which she experienced blunt force trauma from airbag contact to the left eye (OS). In the emergency department, the patient complained of pain, decreased visual acuity, excessive tearing, and edema of the left eyelids. Slit lamp examination was consistent with a complete traumatic LASIK flap amputation O

BACKGROUND: Civilian prehospital tourniquet application (TA) has increased over time, and prehospital TA may be associated with decreased hemorrhagic shock and improved survival. However, tourniquets may be placed incorrectly or without indication. No national study has evaluated the effectiveness of bystander TA.
OBJECTIVES: This study sought to compare the success and short-term outcomes of prehospital TA by emergency medical services (EMS) or first responders vs. civilian bystanders, hypothesizing that civilian TA would lead to less improvement in patient acuity than EMS TA.
METHODS: The …
